The effect of short-term health promotion intervention on motor function in community-dwelling older adults

Physical activity (PA) can improve older adults’ gait performance and decrease fall risks, yet adherence to PA programs is low. The purpose of this study was to evaluate the efficacy of a game-centered mobile health promotion application (app) to improve gait performance in community-dwelling older adults. Participants (N = 38; age 72.42 ± 12.58) from four independent living facilities were randomly assigned to experimental (n = 20; app with exercise) or control (n = 18; app without exercise) conditions. Each condition completed a 10-week intervention using an inclusive evidence-based health promotion program that combines bingo and exercise, Bingocize®. The investigators collected baseline/post gait assessments 1 week prior and 1-week post intervention. Data were analyzed using a mixed-model ANOVA (p < 0.05). There were no main effects; however, significant interactions (group × time) were observed, and interactions for gait speed (>5 cm/s) were observed in the experimental condition. Research supports this increase as a meaningful clinical change in gait speed, which may potentially reduce the risk of falls.

The COVID-19 pandemic has impacted the ability to evaluate motor function in older adults, as motor assessments typically require face-to-face interaction. This study tested whether motor function can be assessed at home. One hundred seventy-seven older adults nationwide (recruited through the MindCrowd electronic cohort) completed a brief functional upper-extremity assessment at home and unsupervised. Performance data were compared to data from an independent sample of community-dwelling older adults (N=250) assessed by an experimenter in-lab. The effect of age on performance was similar between the in-lab and at-home groups for both the dominant and non-dominant hand. Practice effects were also similar between the groups. Assessing upper-extremity motor function remotely is feasible and reliable in community-dwelling older adults. This test offers a practical solution in response to the COVID-19 pandemic and telehealth practice and other research involving remote or geographically isolated individuals.

Background: Older adults often overestimate their motor performance, which represents a serious safety hazard. The cause of this self-overestimation is to date, not yet fully established. Thus, the present study aimed to reveal the factors associated with self-overestimation by focusing on motor function.Methods: This study included 105 community-dwelling older adults [20 males, median (25, 75 percentile) age: 73.00 (69.50, 77.50)]. Participants were assessed for errors in their self-estimation using a two-step test. They estimated the two-step distance that could be reached with maximum effort. Thereafter, they performed the actual two-step action. Participants were comprehensively assessed for motor function by various tests (i.e., 10-meter Walking Test, Timed Up and Go Test, postural stability, and muscle strength). They were then divided into two groups (the self-underestimation or self-overestimation group) and their motor performances were compared. Multiple linear regression analysis was then utilized to investigate the relationship between self-estimation error and motor function.Results: Significant differences were found between the two groups regarding age, weight, actual two-step distance, and the time required for the Timed Up and Go Test and 10-meter Walking Test (p &lt; 0.05). The regression analysis showed that self-estimation error was significantly related to the result of the 10-meter Walking Test (beta = 0.24, p = 0.011).Conclusions: The self-overestimation of motor performance, which is likely to lead to several dangers (i.e., falling or obstacle collision), was related to walking ability. Consequently, the results showed that the 10-meter Walking Test would assist in detecting the self-overestimation of motor performance.

Quality of life (QOL) and mortality are true endpoints of epidemiological or medical research, especially for community-dwelling older adults. Nutritional status and activities of daily living (ADLs) are associated with QOL and mortality. Good oral health status supports a good nutritional status. The aim of this study was to elucidate the complex structure of these important health-related factors. We surveyed 354 healthy older adults at the age of 85. Nutritional status was evaluated by the serum level of albumin. QOL, ADLs, self-assessed chewing ability, serum albumin level, and mortality during the 15 year follow up period were analyzed. Self-assessed chewing ability was associated with QOL and ADLs. Self-assessed chewing ability for slight-hard foods was associated with mortality in men. However, it was not associated with the serum albumin level. The serum albumin level was associated with mortality in women. These results indicate that maintaining good oral function is not enough. Nutritional instruction in accordance with oral function is indispensable for health promotion in older adults. When planning health promotion strategies for older adults, different strategies are needed for men and women.
