Educational attainment is among the most substantial protective factors against cigarette smoking, including during pregnancy. Although Minorities’ Diminished Returns (MDRs) of educational attainment, defined as weaker protective effect of education for racial and ethnic minority groups compared to Non-Hispanic Whites, has been demonstrated in previous studies; such MDRs are not tested for cigarette smoking during pregnancy. To better understand the relevance of MDRs to tobacco use during pregnancy, this study had three aims: firstly, to investigate the association between educational attainment and cigarette smoking in pregnant women; secondly, to compare racial and ethnic groups for the association between educational attainment and cigarette smoking; and thirdly, to explore the mediating effect of poverty status on such MDRs, among American adults during pregnancy. This cross-sectional study explored a nationally representative sample of pregnant American women (n = 338), which was taken from the Population Assessment of Tobacco and Health (PATH; 2013). Current smoking was the outcome. Educational attainment was the independent variable. Region and age were the covariates. Poverty status was the mediator. Race and ethnicity were the effect modifiers. Overall, a higher level of educational attainment (OR = 0.54, p < 0.05) was associated with lower odds of current smoking among pregnant women. Race (OR = 2.04, p < 0.05) and ethnicity (OR = 2.12, p < 0.05) both showed significant interactions with educational attainment on smoking, suggesting that the protective effect of educational attainment against smoking during pregnancy is smaller for Blacks and Hispanics than Non-Hispanic Whites. Poverty status fully mediated the above interactions. In the United States, highly educated pregnant Black and Hispanic women remain at higher risk of smoking cigarettes, possibly because they are more likely to live in poverty, compared to their White counterparts. The results suggest the role that labor market discrimination has in explaining lower returns of educational attainment in terms of less cigarette smoking by racial and ethnic minority pregnant women.

AbstractObjectivesTo examine the relationship between cigarette smoking and a range of nutrient intakes and blood status indices in older people.DesignNational Diet and Nutrition Survey: cross-sectional survey of nationally representative sample of people aged 65 years and over.SettingMainland Britain during 1994/95.Subjects1191 people (619 male, 572 female) aged 65 years and over, of whom 920 were living in private households and 271 were living in institutions.ResultsCigarette smoking was inversely correlated with intakes of antioxidants and other micronutrients after adjustment for age, sex and domicile. Cigarette smoking was also inversely correlated with a number of antioxidant micronutrient status indices including plasma vitamin C and the carotenoids (but not vitamin E status indices), and with other micronutrient status indices, including plasma pyridoxal phosphate, red cell and serum folate, after adjustment for age, sex, domicile and the corresponding nutrient intake. Previous cigarette smoking or cigar/pipe smoking was not generally associated with lower nutrient intakes or status indices, however, both current and previous cigarette smoking was associated with increased concentrations of acute phase indicators. Further adjustment for total energy intake and/or sociodemographic, health and drug usage variables attenuated only a few of the associations observed.ConclusionOlder people who smoke cigarettes are at increased risk of suboptimal antioxidant and other micronutrient intakes and status, but the lower intakes found in cigarette smokers only partly explain their reduced blood indices.

Objective. To examine the relationship between maternal cigarette smoking during pregnancy and children's intellectual functioning during the first 4 years of life. Design. Prospective follow-up of participants in a randomized trial of pregnancy and infancy nurse home visitation. Setting. Semirural community in Upstate New York. Participants. 400 families in which the mothers registered before the 30th week of pregnancy and had no previous live births. Eighty-five percent of the mothers were either teenagers (&lt;19 years at registration), unmarried, or poor. Analysis limited to whites who comprised 89% of the sample. Main results. Children in the comparison group whose mothers smoked 10 or more cigarettes per day during pregnancy had Stanford-Binet scores at 3 and 4 years of age that were 4.35 (95% CI: 0.02, 8.68) points lower (after controlling for a wide range of variables) than their counterparts whose mothers did not smoke during pregnancy. Conclusions. The results of this study add to the increasingly consistent evidence that maternal cigarette smoking during pregnancy poses a unique risk for neurodevelopmental impairment among children and provide an additional reason for pregnant women not to smoke cigarettes.
