Abstract Background There is little information on the relationships of fasting blood glucose (FBG) including high normal blood glucose and impaired fasting glucose (IFG) with endothelial function. The purpose of this study was to evaluate the relationship between FBG level and flow-mediated vasodilation (FMD) in detail using a large sample size. Methods This study was a cross-sectional study. We measured FMD in 7265 subjects at 31 general hospitals. The subjects were divided into four groups based on FBG levels: <5.55 mmol/L, 5.55–6.05 mmol/L, 6.11–6.94 mmol/L, and ≥ 6.99 mmol/L or known type 2 diabetes. The subjects were also divided into six groups based on FBG levels: <5.00 mmol/L, 5.00-5.22 mmol/L, 5.27–5.50 mmol/L, 5.55–6.05 mmol/L, 6.11–6.94 mmol/L, and ≥ 6.99 mmol/L or known type 2 diabetes. Results FMD decreased in relation to increase in FBG level. There was a significant difference in FMD between the FBG of < 5.55 mmol/L group and the other three groups (6.7 ± 3.1% vs. 5.9 ± 2.8%, 5.7 ± 3.1%, and 5.1 ± 2.6%; p < 0.001, respectively). After adjustment for confounding factors, the odds of having the lowest quartile of FMD was significantly higher in the FBG of 5.27–5.50 mmol/L, 5.55–6.05 mmol/L, 6.11–6.94 mmol/L, and ≥ 6.99 mmol/L or known type 2 diabetes groups than in the FBG of < 5.00 mmol/L, group. Conclusions These findings suggest that FBG of 5.55–6.05 mmol/L and FBG of 6.11–6.94 mmol/L are similarly associated with endothelial dysfunction and that a pre-IFG state (FBG of 5.27–5.50 mmol/L) is also a risk for endothelial dysfunction compared with FBG of < 5.00 mmol/L. ObjectivesHDLs have many diverse functions. The goal of this study was to determine the association of HDL cholesterol (HDL-C) and apolipoprotein A-I (apoA-I) with the development of type 2 diabetes (T2D). In particular, this study determined the association between the ratio of HDL-C to apoA-I (HA) and incident T2D.Design and methodsA total of 27 988 subjects with impaired fasting glucose (IFG) (18 266 men and 9722 women) aged 21–91 years (mean age 40.7 years) were followed for a mean duration of 2.81 years.ResultsStudy subjects were divided into quartiles according to the baseline HA ratio. Age, male sex, current smoking, BMI, waist circumference, and high-sensitivity C-reactive protein decreased across the quartiles, and all metabolic profiles, including blood pressure, fasting glucose, insulin resistance as determined by homeostasis model assessment of insulin resistance, and lipid measurements such as total cholesterol, LDL cholesterol, non-HDL-C, and apoB, improved as the HA ratio increased. In addition, incident cases of T2D decreased as the HA ratio increased, independent of age, sex, BMI, current smoking, systolic blood pressure, HbA1c, fasting serum insulin, family history of diabetes, and serum triglyceride concentrations (HR (95% CI) of fourth quartile vs first quartile; 0.76 (0.67–0.86), P<0.0001).ConclusionsA higher HA ratio was associated with favorable metabolic profiles and a lower risk of T2D development in subjects with IFG.

IntroductionPeripheral arterial tonometry (PAT) is an operator-independent and non-invasive measurement method to assess microvascular endothelial function in the fingertips. PAT-derived measures of endothelial function were associated with type 2 diabetes in cross-sectional studies. However, longitudinal studies are lacking. The study aims to investigate the association of two PAT-derived endothelial function parameters reactive hyperemia index (RHI) and mean baseline amplitude (MBA) with follow-up glucose and insulin parameters and the development of (pre)diabetes and type 2 diabetes.Research design and methodsThe study included 673 participants initially without diabetes (328 men and 345 women) aged 52–71 years from the prospective population-based Cooperative Health Research in the Region of Augsburg F4/FF4 cohort study conducted in Southern Germany (baseline examination F4: 2006–2008; follow-up FF4: 2013–2014). An oral glucose tolerance test was performed at baseline and follow-up to define type 2 diabetes, prediabetes, fasting glucose, fasting insulin, 2-hour glucose, homeostasis model assessment of insulin resistance (HOMA-IR), homeostasis model assessment of beta-cell function and hemoglobin A1c.ResultsIn multivariable adjusted logistic/linear regression models, a 1 SD increase in baseline RHI was inversely associated with incident type 2 diabetes (OR 0.69 (95% CI 0.48 to 0.97)) as well as with fasting insulin (β −0.069 (95% CI −0.131 to −0.007)) and HOMA-IR (β −0.072 (95% CI −0.133 to −0.010)) at follow-up in participants with initial normoglycemia. A 1 SD increase in baseline MBA was positively associated with incident (pre)diabetes (OR 1.62 (95% CI 1.25 to 2.11)) and fasting glucose (β 0.096 (95% CI 0.047 to 0.146)) at follow-up in participants with initial normoglycemia.ConclusionsMicrovascular endothelial dysfunction seems to be involved in the development of early derangements in glucose metabolism and insulin resistance and could thereby trigger the development of prediabetes and type 2 diabetes.
