Detection of Metaplastic Columnar Epithelium At the Esophagogastric Junction Using Chromoendoscopy with Alcian Blue

CONTEXT: Barrett's esophagus is characterized by the presence of goblet cells. However, when alcian-blue is utilized, another type of cells, called columnar blue cells, is frequently present in the distal esophagus of patients with endoscopic evidence of Barrett's esophagus. Cytokeratin 7 and 20 immunoreactivity has been previously studied in areas of intestinal metaplasia at the esophagogastric junction. However, the expression of these cytokeratins in columnar blue cells has not been characterized. OBJECTIVE: To compare the expression of cytokeratin 7 and 20 in goblet cells and columnar blue cells in patients with endoscopic evidence of Barrett's esophagus. METHODS: Biopsies from 86 patients with endoscopic evidence of Barrett's esophagus were evaluated. The biopsies were stained for cytokeratin 7 and 20. RESULTS: Goblet cells were present in 75 cases and columnar blue cells in 50 cases. Overall, cytokeratin 7 expression was similar in goblet cells and columnar blue cells (P = 0.25), while cytokeratin 20 was more common in goblet cells (P <0.001). In individuals with both cell types, however, cytokeratin 7 staining was the same in goblet and columnar blue cells in 95% of the cases, and cytokeratin 20 staining was the same in 77%. CONCLUSION: Goblet cells and columnar blue cells have similar immunohistochemical staining patterns for cytokeratins 7 and 20 in patients with endoscopic evidence of Barrett's esophagus.

The histological observations of this study which conducted on nasal cavity of black Iraqi goat revealed that the nasal cavity consisted of three regions (vestibular, respiratory and olfactory(.The vestibular region was narrow zone, lined with (non keratinized) stratified squamous epithelium, and its propria submucosa contained serous glands and blood vessels.The respiratory region was lined with ciliated pseudostratified columnar epithelia with goblet cells. The vascular propria submucosa had mucous glands, which were positive to periodic acdic shieff and Alcian blue stains. The olfactory region was lined with thick pseudostratified columnar epithelium devoid of goblet cells and contained, the propria submucosa serous simple acinus Alcian positive glands (Bowman's glands) and numerous nerve bundles. The histological results concluded that the nasal cavity of black Iraqi goat was good adaptive tooled for hot dusty environment.

Vestiges of the omphalomesenteric or vitello-intestinal duct and the pathologic implications attributed to these remnants have been treated in great detail by several investigators. Persistence of the omphalomesenteric duct is associated with such conditions as Meckel's diverticulum, umbilical fistula, mucosal polyps, and sinuses or cysts of the umbilicus. Remnants of the duct in the umbilical cord, although infrequent, are located outside of the triangle formed by the two umbilical arteries and the umbilical vein, are usually discontinuous and are often represented by a small lumen lined by cuboidal or columnar epithelium. This study will examine the ultrastructure of these cells.

The overall fine structure of the eye in Placopecten is similar to that of other scallops. The optic tentacle consists of an outer columnar epithelium which is modified into a pigmented iris and a cornea (Fig. 1). This capsule encloses the cellular lens, retina, reflecting argentea and the pigmented tapetum. The retina is divided into two parts (Fig. 2). The distal retina functions in the detection of movement and the proximal retina monitors environmental light intensity. The purpose of the present study is to describe the ultrastructure of the retina as a preliminary observation on eye development. This is also the first known presentation of scanning electron microscope studies of the eye of the scallop.

Developing blood vessels in the rat central nervous system exhibit several unusual luminal features. Hannah (1975) used high voltage electron microscopy to demonstrate numerous ridges of endothelium, some near junctional complexes. The ridges produced troughs (which may appear as depressions) in the endothelial surface. In some areas ridges extended over the troughs, removing them from direct contact with the luminal surface. At no time were the troughs observed to penetrate the basal laminae. Fingerlike projections also extended into the lumina.To determine whether any chemical specializations accompanied the unusual morphological features of the luminal surface, we added 0.1% Alcian blue (Behnke and Zelander, 1970) to the 3% glutaraldehyde perfusate (cacodylate buffer, pH 7.4). After Alcian blue had reacted with the luminal glycocalyces, the dye was dissociated with MgCl2 via critical electrolyte concentration method of Scott and Dorling (1965). When these methods are applied together, it is possible to differentiate mucopolysaccharides (glycosaminoglycans or GAG) with the electron microscope.
