Abstract: Heart failure is the major cause of morbidity and mortality worldwide. Heart failure is defined as heart’s inability to pump the blood to supply oxygen and nutrients to the body’s tissues. Functional limitation and psychological distress such as depression caused by this chronic condition will affect the quality of life of patients. This study aimed to determine the relationship of the level of depression and quality of life of patients with chronic heart failure. This was an observational analytical study with a cross sectional approach. Samples were 38 patients with chronic heart failure obtained by using consecutive sampling method. The level of depression was measured by using the Beck Depression Inventory-II meanwhile the quality of life was measured by using the Short Form-36. The results showed that of the 15 respondents with minimal depression, 4 respondents had poor quality of life and 11 respondents had better quality of life; of the 16 respondents with mild depression, 13 respondents had poor quality of life and 3 respondents had better quality of life, meanwhile 5 respondents with moderate depression and two respondents with severe depression had poor quality of life. Conclusion: There was a significant relationship between the level of depression and the quality of life. The higher the level of depression the poorer the quality of life of patients with chronic heart failure.Keywords: chronic heart failure, levels of depression, quality of life  Abstrak: Gagal jantung merupakan penyebab utama morbiditas dan mortalitas di seluruh dunia. Gagal jantung didefinisikan sebagai ketidakmampuan jantung memompa darah untuk memenuhi kebutuhan oksigen dan nutrisi jaringan tubuh. Keterbatasan fungsional dan distres psikologis seperti kejadian depresi yang disebabkan kondisi kronis ini akan mempengaruhi kualitas hidup pasien. Tujuan penelitian ini yaitu untuk mengetahui hubungan tingkat depresi dan kualitas hidup pada pasien gagal jantung kronik. Penelitian ini menggunakan metode analitik observasional dengan pendekatan potong lintang. Sampel penelitian adalah 38 pasien gagal jantung kronik yang diambil menggunakan teknik consecutive sampling. Tingkat depresi diukur menggunakan kuesioner Beck Depression Inventory-II dan kualitas hidup diukur menggunakan kuesioner Short Form-36. Hasil penelitian menunjukkan dari 15 responden dengan depresi minimal terdapat empat responden memiliki kualitas hidup kurang baik dan 11 responden memiliki kualitas hidup baik, kemudian dari 16 responden dengan depresi ringan terdapat 13 responden memiliki kualitas hidup kurang baik dan tiga responden memiliki kualitas hidup baik, sementara lima responden dengan depresi sedang dan dua responden dengan depresi berat memiliki kualitas hidup kurang baik. Simpulan: Terdapat hubungan bermakna antara tingkat depresi dan kualitas hidup, dimana semakin tinggi tingkat depresi maka semakin rendah kualitas hidup pasien gagal jantung kronik.  Kata kunci: gagal jantung kronik, tingkat depresi, kualitas hidup

The aim of the present study was to assess whether EPC (endothelial progenitor cell) number/function might be an explanatory factor for the observed relationship between Type D personality (a joint tendency towards negative affectivity and social inhibition) and poor cardiovascular prognosis. We also assessed whether the effect of a single exercise bout on EPC number/function was affected by Type D personality. A total of 35 sedentary men with CHF (chronic heart failure; left ventricular ejection fraction ≤45%) underwent CPET (cardiopulmonary exercise testing) and personality assessment with the 14-item Type D scale. CD34+/KDR (kinase insert domain-containing receptor)+ cells were quantified by flow cytometry before and immediately after CPET. Migration of early EPC towards VEGF (vascular endothelial growth factor) and SDF-1α (stromal-cell-derived factor-1α) was investigated. Type D (n=10) and non-Type D (n=25) patients were comparable with regards to demographics, disease severity and Framingham risk factor score. Circulating EPC numbers were reduced by 54% in Type D compared with non-Type D patients (0.084±0.055 and 0.183±0.029% of lymphocytes respectively; P=0.006). Exercise led to a 60% increase in EPC in Type D patients, whereas the EPC number remained unchanged in the non-Type D group (P=0.049). Baseline migratory capacity was related to disease severity, but was not different between Type D and non-Type D patients. Exercise induced a highly significant enhancement of migratory capacity in both groups. In conclusion, reduced EPC numbers might explain the impaired cardiovascular outcome in Type D patients. The larger increase in circulating EPCs observed in these patients suggests that acute exercise elicits a more pronounced stimulus for endothelial repair.

ABSTRACT Background Malnutrition is common in patients with chronic heart failure (CHF) and is associated with adverse outcome, but few data exist. Objectives The objective of this study was to compare the agreement and classification performance of 6 malnutrition tools in patients with CHF. Methods We evaluated the performance of 6 malnutrition tools: COntrolling NUTritional Status Index (CONUT), Geriatric Nutritional Risk Index (GNRI), Prognostic Nutritional Index (PNI), Malnutrition Universal Screening Tool (MUST), Mini Nutritional Assessment–Short Form (MNA-SF), and Subjective Global Assessment (SGA), in 467 consecutive patients with CHF who attended our clinic for follow-up. We used Venn diagrams and Kappa statistics to study the agreement of different tools. Because there is no “gold standard” for malnutrition evaluation, for each of the malnutrition tools, we used the results of the other 5 tools to produce a standard combined index for evaluating at least moderate malnutrition. Subjects were considered as having at least moderate malnutrition if so identified by ≥3/5 tools. We evaluated the sensitivity, specificity, and predictive values of different tools in identifying significant malnutrition as defined by the combined index. Results Men comprised 67% of patients, median age was 76 years, and median N-terminal pro-B-type natriuretic peptide (NTproBNP) was 1156 ng/L. The prevalence of any degree and at least moderate malnutrition ranged between 6–60% and 3–9%, respectively, with CONUT classifying the highest proportion of subjects as malnourished. Malnourished patients tended to be older and have worse symptoms, higher NTproBNP, and more comorbidities. CONUT had the highest sensitivity (80%), MNA-SF and SGA had the highest specificity (99%), and MNA-SF had the lowest misclassification rate (2%) in identifying at least moderate malnutrition as defined by the combined index. Conclusions Malnutrition is common in patients with CHF. The prevalence of malnutrition varies depending on the tool used. Among the 6 malnutrition tools studied, MNA-SF has the best classification performance in identifying significant malnutrition as defined by the combined index.
