A preliminary study of group therapy intervention for bipolar disorder based on the integrative cognitive model

Abstract The main aim of this research was to investigate the effect of the 8-week group therapy intervention based on the integrative cognitive model (ICM) of bipolar disorder. The sample was composed of 10 patients with bipolar I and bipolar II diagnoses. The group sessions were held once a week for 1.5 hours. For the quantitative measures, the group participants were assessed during pre-test, post-test and 3-month follow-up using the Turkish versions of the Beck Depression Inventory, the Hypomanic Attitudes and Positive Predictions Inventory, the Psychological Well-being Scale and the Satisfaction with Life Scale. Quantitative analyses revealed that a reliable change was seen only in pre-test and post-test depression scores. However, this change could not be maintained at 3-month follow-up. For the qualitative analyses, data collected during semi-structured interviews were used. The thematic analyses results showed that the 8-week ICM-based group therapy was found to help participants make sound observations about their behaviors, decrease their feelings of loneliness and increase self-acceptance, and acceptance of the role of mood swings in their lives. Moreover, the results showed that participants found the following activities very beneficial: describing their emotions, evaluating the intensity of their emotions, identifying their life goals, coping with interpersonal conflicts, evaluating resources and going over strengths. In conclusion, the 8-week ICM-based group therapy had a positive impact on participants’ self-awareness, self-acceptance and self-perception. Key learning aims (1) To learn more about the integrative cognitive model and how it has been reconceptualized for group-based intervention. (2) To understand the structure and content of group CBT protocol for bipolar disorders based on the integrative cognitive model. (3) To consider possible advantages of group CBT for bipolar disorders during the recovery process.

Objective: To analyze new and reviewed findings to evaluate relations between treatment response and latency from onset of bipolar disorder (BD) to the start of mood-stabilizer prophylaxis. Method: We analyzed our own new data and added findings from research reports identified by computerized searching. Results: We found 11 relevant studies, involving 1485 adult patients diagnosed primarily with BD. Reported latency to prophylaxis averaged 9.6 years (SD 1.3), and follow-up in treatment averaged 5.4 years (SD 3.1). Greater illness intensity and shorter treatment latency were closely associated, resulting in a greater apparent reduction in morbidity with earlier treatment. However, this finding was not sustained after correction for pretreatment morbidity, and treatment latency did not predict morbidity during treatment. Therefore, assessments based on improvement with treatment, or without correction for pretreatment morbidity, can be misleading. Conclusions: Available evidence does not support the proposal that delayed prophylaxis may limit response to prophylactic treatment in BD and related disorders.

Background: The prevalence of diabetes is on the rise, and the lack of regular self-care activities can exacerbate this disease. Therefore, finding effective and short-term treatments is needed for these patients. Objectives: The purpose of this study was to investigate the effectiveness of acceptance-based emotion regulation group therapy in controlling diabetes in patients with type 2 diabetes. Methods: This experimental study included the three stages of pre-test, post-test, and six-month after follow-up, as well as a control group. The statistical sample consisted of 33 patients with type 2 diabetes referred to the Iranian Diabetes Association, of whom 16 patients were allocated to the intervention group and 17 to the control group. The study was conducted in Tehran in 2019 - 2020. Acceptance-based emotion regulation group therapy lasted 14 weeks, during which some aspects of acceptance and commitment therapy, dialectical therapy, and emotion-focused treatment were combined and provided to the patients. The questionnaires used were the Summary of Diabetes Self-care activities (SDSCA) and Diabetes Dependent Quality of Life scale. Also, a structured DSM-V clinical interview was performed, and glycosylated hemoglobin (HbA1c) was measured. To analyze the data, mixed design ANOVA was run in SPSS version 21. Results: The findings revealed that the mean difference between pre-test and post-test in the experimental group was significant for the variables of HbA1c, quality of life, and self-care while the mean difference between the post-test and follow-up was not significant for HbA1c (P = 0.17) and quality of life (P = 0.27), indicating the stability of the therapeutic effect after six months of the intervention. Based on the present findings, acceptance-based emotion regulation group therapy led to a decrease in HbA1c and an increase in self-care and quality of life in patients with type 2 diabetes. Conclusions: Our results showed that acceptance-based emotion regulation group therapy improved self-care, quality of life, and HbA1c in type 2 diabetic patients, so it can be used as a complementary intervention along with medical treatments.

Background and objectives: The aim of this study was examining the effectiveness of mindfulness-based cognitive group therapy on the quality of life and hope in the patients with coronary heart disease. Methods: The present study was quasi-experimental with pre-test, post-test and follow-up with control group. To fulfill the study, 30 patients were selected randomly and were assigned into experimental (n = 15) and control groups (n = 15). To collect the data, participants of both groups completed demographic information sheet, MacNew Heart Disease Health-Related Quality of Life Questionnaire and Miller Hope Scale, respectively of coronary heart disease questionnaire at pre-test, post-test and follow-up (2 months after the intervention). Participants of experimental group received eight 90-minute intervention sessions. Results: The results of the multivariate covariance analysis with repeated measures showed that mean score quality of life(P<0.05), hope (P<0.001) in participants of the experimental group had a significant difference compared to the participants of the control group at post-test and follow-up. Conclusion: results showed that mindfulness – based stress reduction treatment can be effective in improving life quality and hope in coronary heart disease. Implication for further research and possible clinical applications are discussed.

AbstractBackground.In recent years, several controlled studies could show that psychoeducational interventions have been effective for relapse prevention in bipolar disorders. We therefore established a cognitive-psychoeducational group intervention with 14 sessions providing information about the illness, early warning signs, cognitive and behavioural strategies for stress management and social rhythm. Additionally we offered a group intervention for the patients' relatives. The objective of this study was to describe the outcome associated with our psychoeducational intervention in bipolar patients and their relatives.Methods.Sixty-two bipolar patients attended 14 sessions (à 90 min) of cognitive-psychoeducational group therapy. Patients' knowledge of bipolar disorder and their satisfaction with the treatment were assessed using self-developed questionnaires before and after the group intervention. Additionally, 49 relatives of bipolar patients received two psychoeducational workshops of 4 hours each. We assessed demographic variables, burden, high expressed emotion and depressive symptoms of the relatives before and after the two workshops and at 1-year follow-up.Results.Patients significantly improved their knowledge of bipolar disorder. They also have benefited from the discussions and the exchange of useful coping strategies. Burden and high expressed emotions showed no significant reductions at post-assessment, however they were significantly reduced at 1-year follow-up. Relatives also felt significantly better informed about the illness.Conclusions.These findings show that psychoeducational interventions in bipolar patients and their relatives improve patients' and their relatives' knowledge of the illness and the burden of the disorder as well as high expressed emotions are reduced in relatives at 1-year follow-up.

Aims: Patients with high blood pressure usually experience high levels of stress that affect their emotion regulation and self-care behaviors. The current study aimed to examine the cognitive-minded group therapy on perceived stress, emotional, cognitive regulation, and self-care behaviors in patients with hypertension. Methods & Materials: The study has a quasi-experimental design with a pre-test, post-test, follow-up, and a control group. The study population was all people with hypertension referred to medical centers in Kish Island, Iran, in 2010 (March to June). The research sample consisted of 32 qualified candidates who were selected using the purposive sampling method. Then they were randomly assigned to the experimental and control groups. Perceived stress, emotional and cognitive regulation, and self-care questionnaires of patients with hypertension were used to collect information in the pre-test, post-test, and follow-up stages. The experimental group underwent mindfulness-based group therapy, but the control group did not receive any intervention. The obtained data were analyzed with repeated measures analysis of variance in SPSS v. 22. The significance level of the tests was considered 0.05. Findings: The results showed a significant difference between the experimental and control groups in the mean scores of positive stress (P=0.001), negative stress (P=0.001), positive emotion (P=0.001), negative emotion (P=0.001), medication regimen (P=0.003), diet (P=0.011), and disease management (P=0.026) in the post-test and follow-up. But there was no significant difference between the mean scores of food labels (P=0.195). Conclusion: The effectiveness of mindfulness-based group therapy on reducing stress and negative emotion and improving self-care of patients with hypertension suggests that in addition to the physical components of chronic diseases, the psychological components of patients be considered to prevent recurrence diseases and increase adherence to treatment in them.

Penelitian ini bertujuan untuk mengetahui apakah terdapat perbedaan tingkat self acceptance mahasiswi FISIPOL Unmul setelah diberi metode color therapy dengan media lipstick merah. Penelitian ini menggunakan pendekatan kuantitatif eksperimen dan desain yang digunakan adalah Static Group Comparison. Static Group Comparison merupakan desain eksperimen yang memberikan perlakuan hanya kepada satu kelompok. Subjek penelitian ini adalah 15 orang kelompok eksperimen. Data yang diperoleh kemudian dianalisis menggunakan uji analisis regresi linear berganda dengan bantuan program Statistical Package for Social Sciences (SPSS) 23.0 for Windows. Berdasarkan hasil uji paired t test diperoleh hasil: (1) tidak ada perbedaan self acceptance mahasiswi FISIPOL unmul setelah diberi color therapy red lisptick yang ditunjukan dengan hasil pre test-post test kelompok eksperimen, dengan t hitung = -0,180 (< t tabel = 2,145) dan 𝑝 = 0,860 (𝑝> 0.05). Dan hasil post test-follow up, t hitung = -0,327 (< t tabel = 2,145) dan 𝑝 = 0,748 (𝑝> 0.05); (2) tidak ada perbedaan self acceptance mahasiswi FISIPOL Unmul setelah diberikan materi tentang self acceptance dan beauty norms yang ditunjukan dengan hasil pre test- post test kelompok kontrol, dengan t hitung = 2.033 (< t tabel = 2,145) dan 𝑝 = 0,061 (𝑝> 0.05). Dan hasil post test-follow up t hitung = -1.047 (< t tabel = 2,145) dan 𝑝 = 0,313 (𝑝> 0.05).

Aim: The aim of this study was to compare the effect of reality therapy and cognitive-behavioral training on empathy and self-control of women with marital conflicts. Methods: The design of the present study was a quasi-experimental with pre/post-test and follow-up, and included a control group. The statistical population of the study included all women with conflict with their husbands who referred to counseling centers in 7th district of Tehran in 2020. Among them, 60 people were selected by purposeful sampling method and randomly divided into two experimental groups and a control group (twenty individuals in each group). Initially, all three groups underwent a pre-test, followed by 8 sessions of reality therapy for the first experimental group (Glasser, 2012) and 8 sessions of cognitive-behavioral training (Free, 1999) for the second experimental group. At the end, post-test was performed for all three groups and after three months, a follow-up test was performed to confirm the results. Data collection tools included the Sanai and Barati Marital Conflict Questionnaire, the Julif and Farrington Empathy Questionnaire, and the self-control Questionnaire of Tanji et al. Findings: The results of multivariate analysis of covariance showed that both reality group therapy and cognitive-behavioral training had a significant effect on empathy and self-control of women with marital conflict (P<0.01). cognitive-behavioral training has been more effective on self-control than reality group therapy. However, reality group therapy has been more effective on empathy than cognitive-behavioral training. Conclusion: Due to the effectiveness of both methods on research variables, the use of these trainings by psychologists and counselors in the psychological education program for people with marital conflict is recommended. Also, due to the greater impact of each training on one variable, it is suggested that paired training programs be adjusted based on these findings.

Abstract Background Bipolar disorders are serious illnesses with a chronic course and a high rate of relapse. Typically, bipolar disorders onset during adolescence or early adulthood, with patients experiencing significant personal and social costs as a consequence of their illness. Despite this, to date, there is limited (controlled) evidence regarding the effectiveness of psychotherapy during the critical stages of the disorder (e.g., early onset). Some preliminary studies suggest that targeted, tailored early interventions in particular may improve disease prognosis. The proposed study examines the effectiveness of group psychotherapy on relapse prevention, global adaptive functioning, and neuropsychological functioning in early-stage bipolar disorder. Methods In this multicenter randomized controlled trial (RCT), 300 patients with bipolar disorder are randomized to one of two group psychotherapies: Specific Emotional-Cognitive Therapy (SECT; intervention group) or Emotion-Focused Supportive Therapy (EFST; active control group). Each therapy comprises of a total of 48-h sessions (delivered once a month) over a period of 4 months. Assessments take place at baseline (t1); 6 months follow-up, i.e., post-intervention (t2); 12 months follow-up (t3); and 18 months follow-up (t4), whereby 18 months follow-up is the primary time point of interest. Discussion The goal of this study is to test the effects of an innovative, specific group therapy relative to an active control condition in terms of rates of relapse, global functioning, and neuropsychological functioning. Pending the outcomes of the trial, it will be possible to establish a firm evidence base for accessible group psychotherapy adjuvant to routine psychiatric care for individuals with bipolar disorder. AbstractBackgroundThe long-term outcomes of bipolar disorder range from lasting remission to chronic course or frequent recurrences requiring admissions. The distinction between bipolar I and II disorders has limited utility in outcome prediction. It is unclear to what extent the clinical course of bipolar disorder predicts long-term outcomes.MethodsA representative sample of 191 individuals diagnosed with bipolar I or II disorder was recruited and followed for up to 5 years using a life-chart method. We previously described the clinical course over the first 18 months with dimensional course characteristics and latent classes. Now we test if these course characteristics predict long-term outcomes, including time ill (time with any mood symptoms) and hospital admissions over a second non-overlapping follow-up period in 111 individuals with available data from both 18 months and 5 years follow-ups.ResultsDimensional course characteristics from the first 18 months prospectively predicted outcomes over the following 3.5 years. The proportion of time depressed, the severity of depressive symptoms and the proportion of time manic predicted more time ill. The proportion of time manic, the severity of manic symptoms and depression-to-mania switching predicted a greater likelihood of hospital admissions. All predictions remained significant after controlling for age, sex and bipolar I v. II disorder.ConclusionsDifferential associations with long-term outcomes suggest that course characteristics may facilitate care planning with greater predictive validity than established types of bipolar disorders. A clinical course dominated by depressive symptoms predicts a greater proportion of time ill. A clinical course characterized by manic episodes predicts hospital admissions.

Zusammenfassung. Ziel war es, die Effektivität zweier Interventionen zur Förderung der Lesekompetenz von Fünftklässlern zu untersuchen. Beide Treatments wurden von Regellehrkräften implementiert. Die eine Intervention war das reziproke Lehren, welches um Selbstregulationsprozeduren angereichert wurde (RT+SRL). Die zweite war eine von Lehrkräften konzipierte lesestrategiebasierte Unterrichtseinheit (Good Practice, GP). Zusätzlich gab es eine No-Treatment-Kontrollgruppe (KG0). Insgesamt nahmen an der Studie N = 244 Schüler teil. Im Rahmen eines Pre-, Post-, Follow-Up-Test-Untersuchungsplans kamen standardisierte Leseverständnisaufgaben, selbstkonstruierte Lesestrategieaufgaben und eine Selbstwirksamkeitsskala zum Einsatz. Kontrastierende Einzelvergleichsanalysen ergaben, dass sich die Schüler der Treatmentbedingung RT+SRL im Vergleich zu den Schülern der Kontrollgruppe zum Post-Test signifikant stärker im Leseverständnis, in der Lesestrategieanwendung und in der Selbstwirksamkeit verbesserten. Gleiches galt für die Lesestrategieanwendung zum Follow-Up-Test. Schüler der Bedingung GP konnten im Vergleich zu KG0-Schülern weder zum Post- noch zum Follow-Up-Test vorteilige Ergebnisse in den drei Kriteriumsmaßen erzielen.
