Essential thrombocythemia (ET) is one of the myeloproliferative neoplasms, characterized by persistent thrombocytosis, platelets >450,000/μL, and evident clonal abnormalities like JAK2 V617F, MPL, CALR mutation and not fulfilling WHO criteria for MDS, CML, PV, and IDA. Here we report a 24-year-old female who presented with headache and was found to have thrombocytosis with a platelet count of 2,141 × 103/μL, diagnosed as ET as per WHO criteria 2008; she required ICU admission and thrombocytapheresis with a favorable outcome.

SummaryCardiovascular diseases are the most common disorder in the developed countries. Invasive cardiological and cardiosurgical techniques are known therapies.Yet, patients with severe hereditary haemorrhagical diseases (haemophilia, rare deficiencies of coagulation factors) have an increased bleeding risk by the use of anticoagulants. Therefore, the treatment of these patients requires a concomitant therapy.This article shows eight patients with a severe bleeding diathesis and cardiosurgical interventions in the years 2006 to 2011. This case report shall demonstrate that an adequate therapy can be accomplished with the help of a good cooperation between haemostaseologists and colleagues of the cardioinvasive/ cardiosurgical disciplines.

A patient with a year-long history of metatarsalgia was found to have an elevated platelet count due to essential thrombocythemia, a benign myeloproliferative disease. Reduction of the platelet count with chemotherapy eliminated the foot pain. The authors recommend that a platelet count be part of the work-up of patients with metatarsalgia when the etiology of the foot pain is unclear.

In this care study I will explain the care that SVB received following his admission to the Post Anaesthetic Care unit (PACU) and summarise his discharge to the Intensive Care Unit (ICU). The limitations that this case report presents are that often the care received in the PACU can be quite short in terms of total length of stay in hospital. It is not uncommon that High Dependency patients are received for initial care into the PACU following surgery and to be further discharged and nursed in either a dedicated High Dependency Unit or to the Intensive Care Unit.
