Avascular necrosis of the metacarpal head, known as Dieterich disease, is rare. The underlying pathogenesis of the disease is not clearly understood, and there are few cases reported in the literature. Nonoperative treatment with rest and nonsteroidal anti-inflammatory drugs is often successful, but surgical management is sometimes indicated. The case outlined here describes a novel application of the known technique of metacarpophalangeal joint denervation to relieve pain while maintaining joint mechanics and grip strength.

Avascular necrosis of the metacarpal head is a rare disease. We herein report a case with varying degrees of lesions in the third and fourth metacarpal heads of the right hand and the third metacarpal head of the left hand. The patient was a 37-year-old male right-handed mechanical worker who presented with persistent dull pain in the right hand after labor work for more than a year. The 3 lesions in this patient were treated differently based on their clinical imaging manifestations. The neurologic function of the right hand recovered by the 18-month follow-up; only a slight limitation remained in the right middle finger. This is the first report of 1 patent who received 2 different treatment methods simultaneously and both provided a satisfactory clinical result.

The avascular necrosis of the metacarpal head is a rare disease. Therefore, although a variety of surgical treatments were performed in the sporadically published case reports, the optimal surgical treatment is still controversial. Open wedge osteotomy of the metacarpal neck was first reported in 2002. However, no other reports using the same procedure have been published. We presented 13-year-old girl case of the third metacarpal head necrosis having been treated by the open wedge osteotomy of the metacarpal neck. Four years later, she had no pain and gained full range of motion of the metacarpophalangeal (MP) joint. The plain X-rays and magnetic resonance images demonstrated sufficient bone remodeling of the metacarpal head. The open wedge osteotomy of the metacarpal neck is a useful option for the metacarpal head necrosis, especially when the dorsal articular surface of the head remains intact.

A case of avascular non-traumatic necrosis of the metacarpal head in a 36-year-old woman is reported. Treatment was by curettage of the necrotic bone and packing with cancellous grafts.
