Patients’ assessment of medical services quality – a determinant of changes in medical entity management on the example of Poland

PurposeThe article’s primary goal is to identify areas requiring improvement in the activities of healthcare entities, suggest directions for future changes, and indicate the strengths and weaknesses of the clinic’s operation based on patients’ opinions. Subjectively expressed opinions of patients are treated as acceptance of the current state of affairs or the need to introduce changes in a given area.Design/methodology/approachThe empirical research was based on information obtained from questionnaire surveys on patients’ opinions about services provided by medical entities. The hypothesis was verified by research conducted in 23 (out of 50 possible) the most dynamically developing non-public healthcare institutions in one of the regions of Poland. The conducted research was based on a proprietary survey using questions on qualitative and quantitative scales.FindingsThe results of empirical research allowed us to identify areas requiring improvement and to propose future directions of changes in the surveyed units. The suggested changes should significantly improve efficiency in the organisation and management of a health facility, focused on medical effectiveness and patients’ health effectiveness.Originality/valueFrom a broader perspective, research results may become a starting point for further considerations on changes in the organisation and management of healthcare facilities. Using the study’s conclusions in practice may positively affect the improvement of the functioning of healthcare facilities, their better reputation and contribute to increasing competitiveness in the medical services market.

Abstract Introduction. Family medicine remains the primary type of medical services in Poland and it is supposed to treat both individual patients and the society as a whole. Due to the growing commercialization of the health service, most primary healthcare centers have transformed into non-public healthcare facilities. The public ones (called SPZOZ in Polish) account only for a small fraction of the whole number of primary healthcare facilities. The quality of medical services provided by such facilities, as patients see it, remains one of the key elements determining the development of family medicine centers. Aim. The aim of this paper was to assess patient satisfaction levels regarding the healthcare services they received in two primary healthcare institutions, both of the NZOZ and SPZOZ type, in a small town located close to Lublin. Material and methods. An anonymous survey was filled out by 30 patients of both a public and non-public healthcare center located in Niemce (Niemce Commune, Lublin District). The quality of services was assessed using an original questionnaire in the form of a poll. Results. The results obtained indicate a clear relationship between one’s trust to the physician, diagnosis accuracy and visiting the particular center again, in order to continue the treatment. For older subjects, it was nurses’ kindness and politeness that was the most important. The elderly appreciated the kindness and politeness of the nurses in particular. No significant differences were found between the institutions in respect of the overall perception of satisfaction with services. In terms of infrastructure assessment, the majority of positive feedback was provided for NZOZ. Conclusions. The findings above suggest that it is essential to conduct surveys on a regular basis, in order to check patients’ assessment of the service quality in various institutons.

Purpose – The purpose of this paper is to extend the body of knowledge on healthcare facilities management (FM) by investigating the risks associated with outsourcing of FM services in hospitals. Design/methodology/approach – The paper relied on two strands of methodology common with pragmatic research. Questionnaire survey (QS) used data from 208 respondents representing ten hospitals while three out of the ten hospitals involved in the QS were selected based on their willingness to enter the case study (CS) interview. Data collected were analysed using descriptive and inferential statistics for QS and using narrative techniques by discussing themes, sub-themes for the CS. Findings – Findings established 24 out of the 35 risk factors as critical, four factors as somehow critical, and five factors as not critical. Besides, nine risk factors were found to be significantly loaded on the five risk categories. The rank analysis also revealed that the top five critical risk factors are: inexperience and lack of requisite skills; possibility of fraud by vendor; financial failure of chosen vendor; vendor opportunism; and fall in morale of employees. Originality/value – The study provides an unambiguous contribution to exiting body of knowledge on outsourcing risks as it relates to healthcare FM. It reinforces the theory that risks exist in any form of relationship but developed a distinct body of factors associated with outsourcing of FM services particularly from the context of Nigeria’s public healthcare sector.

Purpose This study aims to let researchers sort out the past, look to the future and boost further management research. Design/methodology/approach This paper takes the exploration of the application of AR in the field of management research as the starting point. Through the combing and induction of AR literature to accurately grasp the essential characteristics of AR, this paper analyzes the application prospects and possible difficulties of AR in the field of management research in China, puts forward specific suggestions and explores the specific path of the transformation of management academic research to practice from the perspective of research methods. Findings In research methods, empirical research and case studies have become the mainstream research methods, but there is a passive research limitation of “study what happens.” Action research is different from the standpoint of empirical research that is independent of the research object. It no longer simply treats managers as the “object of information acquisition,” but as the subject of participating in the research. Management should be oriented towards the transformation of practice, and action research (AR) that can organically integrate researchers and practitioners is just a “bridge” to break the gap between academia and business circles to establish a cooperative relationship. Originality/value This paper takes the exploration of the application of AR in the field of management research as the starting point. PurposeThe purpose of this paper is to explain the direct and moderating effects of perceived transparency on citizen satisfaction with public healthcare services.Design/methodology/approachThis paper extends the classic framework of service quality, value and satisfaction by adding perceived transparency as an essential component, applies service-dominant logic to develop hypotheses regarding the effects of transparency and empirically tests the extended framework with secondary household survey data on China’s public healthcare.FindingsThe classic framework of service quality, value and satisfaction that originated from the private sector is robust in public service. Perceived transparency has a positive impact on perceived value and citizen satisfaction, but the positive effects of service quality and perceived value on citizen satisfaction were weakened with the increasing level of perceived transparency.Research limitations/implicationsPerceived transparency is a double-edged sword regarding citizen satisfaction: although transparency increases perceived value and satisfaction by enhancing citizens’ service co-creation and inspiring citizen confidence, increased transparency raises citizens’ expectations for service products, which subsequently dampens the positive effects of service quality and perceived value on citizen satisfaction.Practical implicationsIn a context with higher transparency, local authorities must learn to manage citizen expectations that influence satisfaction, and increase the probability of satisfaction by avoiding too high citizen expectations.Originality/valueThis paper highlights the importance of transparency in enhancing public service co-creation, and contributes a novel perspective on the effects of transparency in the formation of citizen satisfaction, which may serve as the starting point for studying the role of transparency in public affairs.

Purpose The field of returns management has become an important supply chain management topic. The purpose of this paper is to develop a conceptualization of returns management orientation (RMO), a topic that has been largely ignored by the academic community. Design/methodology/approach A series of interviews with experts and a review of the literature were used to develop the construct. Empirical survey data were used to validate the proposed RMO measurement scale. Findings Statistical results support the validity and reliability of the new RMO construct. Different analyses show that the proposed conceptualization satisfies the criteria of the tests for unidimensionality, construct validity, internal consistency reliability, and nomological validity. Furthermore, the results of this study confirm the positive relationship between a firm’s RMO and returns management performance. Research limitations/implications The present study provides a starting-point for further empirical research on returns management from a behavioral perspective. Practical implications RMO has a significant impact on returns management performance. The proposed RMO measurement scale provides a useful tool for companies to evaluate their management’s emphasis or recognition of the returns management function. Originality/value While the field of returns management has gained increased attention from both academics and practitioners, empirical research efforts on this topic are still limited. In order to address this issue, this study takes a behavioral perspective and follows a well-established approach to operationalize the construct of RMO.

Purpose – Based on theoretical input from punctuated equilibrium theory and the exploration vs exploitation paradigm, a set of innovation drivers and propositions is developed. The purpose of this paper is to present a starting point for a more comprehensive investigation of companies’ innovation trajectories towards balance. Design/methodology/approach – The paper builds on a thorough analysis of the literature and suggests the notion of “strategic inflection points” (SIPs). These points are supposed to mark the transgression between innovation types as reflected in respective performance enhancements. Findings – The propositions developed in this paper serve as a basis for further understanding and for subsequent empirical research. Practical implications – Managerial implications relate to the understanding that companies need to balance innovation types in order to increase performance. Originality/value – This paper discusses a variety of innovation types and suggests the notion of SIPs.

Purpose The centrality of ethics and corporate social responsibility (CSR) to the corporate marketing perspective serves as a point of differentiation for the field within the broader marketing discipline. Currently, there is a lack of clarity around the ‘transparency’ construct, which is an integral if ill-defined dimension of ethics and CSR in marketing. A shared understanding of the transparency construct is thus a significant gap within corporate marketing theory. Addressing that gap is the purpose of this paper. Design/methodology/approach The approach in this paper is conceptual. In developing a detailed definition of transparency, the paper draws on core papers in corporate marketing theory as well as organisational transparency. Findings Rawlins’ (2009) multi-layered definition of the transparency construct is identified as appropriate for adoption in the corporate marketing context. Each of the six layers of his definition is analysed to understand what is implied and what the application of the construct means for corporate marketing practice. The implications are that the application of transparency in corporate marketing requires that a positive and proactive approach to information-sharing is adopted; the default position is to share information with stakeholders; both good and bad news are shared; the criteria – accuracy, timeliness, balance and unequivocality – are applied to all information prior to releases; an organisation commits to empowering stakeholders; and there is recognition of an obligation to account to stakeholders. Research limitations/implication The paper is conceptual in nature and does not apply the definition of the transparency construct to empirical data. It is likely that empirical research will lead to further refinements and amendments. The paper should therefore be considered as a starting point for this empirical work. Practical implication The paper provides a detailed definition of the transparency construct, which includes a discussion of what the application of the transparency construct implies and what it means for the practice of corporate marketing. The definition and its practical application are summarised in table form as a guide for both researchers and practitioners of corporate marketing. The table may serve as a guide for evaluating current organisational performance and for embedding transparency in corporate marketing practice. Originality/value This study appears to be the first paper to address the gap in the corporate marketing literature in relation to the transparency construct. This conceptual paper therefore provides a foundation for further empirical research into the application of the transparency construct in corporate marketing.

PurposeGlobally, several studies have shown that hospital building is charged with multiple inherent risks because a large number of users are vulnerable in tragic events. Thus, the need for the fire safety management plan (FSMP) has been proved as an instrument to mitigate fire and related risks in healthcare facilities. In Malaysia, FSMP regarding public healthcare building is yet to be explored in-depth. Therefore, this paper explores the information necessary to develop the FSMP framework for public hospital buildings.Design/methodology/approachThe paper’s objectives were accomplished via a combination of five face-to-face interviews and observations of five selected public hospitals in Pulau Pinang, Malaysia. The five key participants were across the five public hospitals and collated data analysed through thematic analysis with the assistance of MAXQDA 2018.FindingsFindings show that fire safety stakeholders practice system, fire safety action plan and fire risk management were the three main variables that promote fire safety programme and will improve FSMP for Malaysia’s public hospital buildings.Research limitations/implicationsThis paper’s data collection is limited to Penang, Malaysia, and a qualitative research approach was used, but this does not deteriorate the strength of the findings. Future studies are needed to consider validating findings from this paper via a quantitative approach.Practical implicationsThe suggested framework can be employed by Malaysia’s public hospital authorities as a guideline to mitigate fire hazards in the country’s healthcare facilities.Originality/valueThis paper is encouraging hospital operators and other key stakeholders to improve on their FSMP for healthcare buildings across Malaysia as part of the study implications.

PurposeThe aim of this research is to improve the responsiveness of the healthcare network of a large city to a major earthquake, by applying a combined methodology to reduce human suffering and death.Design/methodology/approachScenario analysis, a non-linear programming (NLP) model, and the analytical network process are sequentially applied to find the “best location pattern”.FindingsWhen considering the occurrence of major earthquakes in cities with high population density, as a rule of thumb, the location of healthcare facilities should prioritize areas characteristically overcrowded and/or that were built based on poor standards of seismic resistance.Research limitations/implicationsThe proposed research design does not include a cost criterion in the set of decision variables involved. Furthermore, the results derived from the NLP-model are restricted by the input simulation data.Practical implicationsThe performance of the “best location pattern” is compared with the current location of healthcare facilities in terms of their distances to the affected zones. Metropolis areas worldwide with similar conditions to the city under consideration could be benefited from applying the general methodology for relocation of healthcare facilities described in this research.Originality/valueThis research implements a diverse combination of methodologies to examine the problem of relocating of healthcare facilities in a large city in the wake of an assumed earthquake. In addition, to the best of authors' knowledge, this is the first study of its kind that proposes improvements in the responsiveness of the healthcare facilities' network in the city in question.

Purpose The purpose of this paper is to highlight the ontological implications of combining network and system ontology to conceptualize industrial networks as the empirical manifestations of complex adaptive economic systems. Design/methodology/approach This paper contributes with a systematic discussion on how network and system ontology can be combined to produce better understandings of business networks. It also provides a review of the state-of-the art research literature on the topic as a starting point for the discussion. Findings Findings indicate that networks may be enclosed in each other constituting sub- and supra-networks comprising increasing complexity. In these cases, sub-networks that are black-boxed can be seen as entities in themselves producing inputs and outputs to the supra-network. Networks, once they become black-boxed, can assume the functions of generative mechanisms within a wider supra-network. Research limitations/implications This research is conceptual in nature and needs to be complemented with empirical research. In addition, the literature review used one database complemented with papers from the IMP journal. A wider search could reveal additional research that can be of relevance for the development of the field. Originality/value This paper addresses the ontological and methodological issues arising from a mixed system and network ontology. These issues are commonly ignored or dealt with indirectly in extant literature. For any accumulation of knowledge in the field to be possible, the explication of a mixed ontology is important as it have conceptual and methodological consequences. Adopting such a mixed ontological position provides an ontology in line with empirical research of business practice.
