Of the quantitative approaches applied to inservice inspection, failure modes, effects,criticality analysis (FMECA) methodology is recommended. FMECA can provide a straightforward illustration of how risk can be used to prioritize components for inspection (ASME, 1991). But, at present, it has two limitations. One is that it cannot be used in the situation where components have multiple failure modes. The other is that it cannot be used in the situation where the uncertainties in the data of components have nonuniform distributions. In engineering practice, these two situations exist in many cases. In this paper, two methods based on fuzzy set theory are presented to treat these problems. The methods proposed here can be considered as a supplement to FMECA, thus extending its range of applicability.

PurposeThe purpose of this paper is to facilitate green supply chain (GSC) managers and planners to model and access GSC risks and probable failures. This paper proposes to use the fuzzy failure mode and effects analysis (FMEA) approach for assessing the risks associated with GSC for benchmarking the performance in terms of effective GSC management adoption and sustainable production.Design/methodology/approachInitially, different failure modes are defined using FMEA analysis, and in order to decide the risk priority, the risk priority number (RPN) is determined. Such priority numbers are typically acquired from the judgment decisions of experts that could contain the element of vagueness and imperfection due to human biases, and it may lead to inaccuracy in the process of risk assessment in GSC. In this study, fuzzy logic is applied to conventional FMEA to overcome the issues in assigning RPNs. A plastic manufacturer GSC case exemplar of the proposed model is illustrated to present the authenticity of this method of risk assessment.FindingsResults indicate that the failure modes, given as improper green operating procedure, i.e. process, operations, etc. (R6), and green issues while closing the loop of GSC (R14) hold the highest RPN and FRPN scores in classical as well as fuzzy FMEA analysis.Originality/valueThe present research work attempts to propose an evaluation framework for risk assessment in GSC. This paper explores both sustainable developments and risks related to efficient management of GSC initiatives in a plastic industry supply chain context. From a managerial perspective, suggestions are also provided with respect to each failure mode.

Service products are mostly produced and consumed simultaneously through interaction between customer and service providers. To prevent external failures in service operations, it is important to identify potential risks and take relevant actions to eliminate or reduce the occurrence. Therefore, risk assessment is vital to customer satisfaction in any service organization. Failure mode and effects analysis (FMEA) is an effective and useful tool for risk assessment. Although FMEA has been extensively studied in the manufacturing literature, there are a limited number of studies considering the application of FMEA in the hospitality industry. In traditional FMEA, the risk priority of failure modes is determined by generating a crisp risk priority number (RPN). However, it has been claimed in the literature that crisp RPN doesn't have a good performance in reflecting real-life situations. To overcome this shortcoming, a fuzzy hybrid FMEA method is developed. The proposed method has been tested on a case study in a five-star hotel to assess its applicability and benefits.

Gasifier system is one of the important components of coal gasification device. The technical characteristics of this system mainly lie in the following facts as huge technical scale and high complexity, and there is a dynamic correlation between the failure modes of gasification equipment. Traditional safety analysis methods such as fault tree and bow-tie diagram suffer from drawbacks as being static and ineffective in handling uncertainty, which hamper their application to risk analysis of process systems. This paper presents a newly developed model based on Dynamic Bow-Tie (DBT) and Dynamic Bayesian network (DBN) for quantitative dynamic risk assessment of gasifier system. In the meantime, in order to cope with the uncertainty of the failure data, fuzzy numbers and the defuzzification method are used to transform the experts' language into the failure rates. The results showed that dynamic risk assessment can solve the difficulties dealing with complex dynamic systems which have process variables and characteristics such as multiple, failure correlations, and noncoherence. And it also has important theoretical significance and application value for coal chemical industry to improve the scientificity of risk assessment.

Purpose The purpose of this paper is to proactively analyse and mitigate the root causes of the product and security risks. The case study approach examines the effectiveness of the fuzzy logic approach for assessing the product and process-related failure modes within global supply chain context. Design/methodology/approach The case study of a Printed Circuit Board Company in China is used as a platform for conducting the research. Using data triangulation, the data are collected and analyzed through interviews, questionnaires, expert opinions and quantitative modelling for some interesting insights. Findings Fuzzy logic approach for failure mode and effect analysis (FMEA) provides a structured approach for understanding complex behaviour of failure modes and their associated risks for products and processes. Today’s managers should conduct robust risk assessment during the design stage to avoid product safety and security risks such as recalls. Research limitations/implications The research is based on the single case study and multiple cases from different industry sectors may provide some additional insights. Originality/value The study attempts to mitigate the root causes of product and processes using fuzzy approach to FMEA in supply chain network.

To identify risk items, measure risk value objectively, and establish risk assessment matrix of airports is the major task of airport safety. This paper first extracts 14 risk items of airports from the International Civil Aviation Organization (ICAO) aviation accidents database and then applies Failure Modes, Effect and Criticality Analysis (FMECA) to define the decision factors of probability, severity and detectability of airport risks. This paper also designs a questionnaire and applies fuzzy logic to discover the importance of decision factors, to find out the threshold value of Risk Assessment Matrix, and to prioritize the airport risks. This paper uses Taiwan Taoyuan International Airport as a case study to demonstrate the modeling process and analyze the results.

It has significant impact on the safe operation of the power grid for large scale power transformer. The technique is complex, and the manufacture process is long. The users always take the manufacturing supervision on spot as the quality control method. The quality is determined during the manufacturing process. The quality defects may cause serious safe problems. It has important significance to strengthen the risk assessment and prevention. The quality defects of power transformer are classified based on the fault tree analysis theory. The quality defects of power transformer from the manufacturing supervision of State Grid Corporation of China (SGCC) in 2012 are analyzed as examples. The manufacturing process has multiple layers and factors. The index system for comprehensive evaluation of transformer is put forward. A risk assessment model for power transformer based on fuzzy synthetic evaluation is built. The example shows that the method can estimate the risk effectively. It can provide feasible decision basis for the risk management and maintenance.
