We treated a 65-year-old Japanese man with a giant penile lymphedema due to chronic penile strangulation with a rubber band. He was referred to our hospital with progressive penile swelling that had developed over a period of 2 years from chronic use of a rubber band placed around the penile base for prevention of urinary incontinence. Under a diagnosis of giant penile lymphedema, we performed resection of abnormal penile skin weighing 4.8 kg, followed by a penile plasty procedure. To the best of our knowledge, this is only the seventh report of such a case worldwide, with the present giant penile lymphedema the most reported.

AbstractA 78-year-old Japanese man with a 5-day history of fever (∼38°C) and decreased appetite was admitted to our hospital. Transesophageal echocardiography revealed aortic valve vegetation. Streptococcus constellatus was detected from a blood culture. An antibiotic sensitive to this strain was administered for 6 weeks, and the patient has been well for 6 months without any sign of relapse. A review of all documented cases of infective endocarditis due to S. constellatus revealed nonspecific initial symptoms, especially coughing, and complications of abscess formation and septic embolisation. Clinicians should carefully consider the choice of antibiotic agents in the treatment of infective endocarditis due to S. constellatus, because penicillin-resistant strains have been documented in some cases.

AbstractObjective:We report an extremely rare case of maxillary haemangioma.Method:Case report and review of the literature concerning haemangioma arising from the nasal cavity and paranasal sinuses.Results:Maxillary haemangioma is rare and sometimes requires wider resection than nasal haemangioma if a large tumour is found. We present a case of maxillary haemangioma in a 37-year-old Japanese woman, which was completely resected by pre-operative embolisation and endoscopic sinus surgery.Conclusion:Our findings suggest that if a large maxillary haemangioma is diagnosed pre-operatively, the treatment of choice is pre-operative embolisation followed by endoscopic sinus surgery, in order to avoid the surgical complications associated with wide resection.

The first Japanese case of fibrolamellar carcinoma (FLC) of the liver is described. The 17-year-old Japanese male affected died of the disease 25 months after palliative operation. The diagnosis of FLC had not been confirmed before autopsy. Pre-operative diagnosis of FLC is important to surgeons. Acccording to the literature, FLC is rare in Asia. The endemic discrepancy may suggest that the carcinogenic factors are different from those of general hepatocellular carcinoma.
