SummaryData from non-rapid-cycling bipolar (manic-depressive) patients who were receiving long-term treatment with lithium carbonate were analysed by the life table method to determine when lithium carbonate prophylaxis failures occurred. Forty-four of 96 patients failed to keep well in spite of maintenance lithium therapy. The analysis revealed an early, rapid failure rate during the first six months of treatment, which was followed by a slower rate of failure. Several clinical factors were assessed to determine if any of them predicted which patients would experience their initial failure in the early or late interval, but none of these factors, which included age, sex, age of onset, rate of affective attacks, family history, and the nature of the preceding episode, were found to have any predictive value regarding lithium prophylaxis failure. We found, however, that patients who had early failures tended to have a subsequent early failure in spite of continued maintenance with lithium carbonate.

There is no increase in the overall mortality of patients undergoing long-term lithium treatment compared with those receiving short-term treatment. Lithium causes a reduction in the incidence of suicide in patients suffering from manic depressive illness. Long-term treatment with lithium is more effective than treatment with imipramine or amitriptyline. Drug interactions may cause problems during long-term therapy with lithium. A reduction in plasma levels of lithium should be considered in stable patients on long-term prophylaxis. Discontinuation of therapy often results in a relapse. The toxicity of lithium is related to its effects on calcium transport.

Using the surface electromyography, we evaluated the effect of long- term intake of antidepressant chemical compounds on the masticatory muscles electrical activity at rest in twenty-one subjects. We compared the patient�s acquired data from the masseter and temporalis muscles with the default values for the resting activity of these muscles. We found significantly higher mean rest values concerning the amplitude of signals in all sites (p[0.05), except the left masseter. Considering the limits of the study, it appears that long-term treatment that uses chemically compounds with antidepressant effect may influence the masticatory muscles activity, manifested by hypertonicity at rest.

Ever since Kahlbaum's monograph 1863 the course and outcome of mental disorders have played important roles as criteria and validators of psychiatric classification. The prognosis is fundamental for doctor and patient when deciding whether to start long-term prophylactic medication and, at a later stage, whether to stop a successful long-term treatment. Course is a crucial factor in estimating the social consequences, costs, suicide risk, and mortality associated with mood disorders. The description of course includes the age of onset, episode length, recurrence of episodes, residual symptoms between episodes and outcome (remission, chronicity, death). These aspects are covered in this chapter.

The short and long-term effects of Lithium carbonate in 90 patients with episodic (68 cases) and chronic (22 cases) cluster headache are reported. The results are discussed in particular in relation to the response of the episodic forms, the development of tolerance, the ability of the substance to modify the natural history of the disease, and the side-effects from long-term treatment. On a évalué l'efficacité thérapeutique de l'administration, à court et à long terme, de lithium carbonate dans 90 patients atteints de formes épisodiques (68 cas) ou chroniques (22 cas) de céphalée en grappe. On discute les résultats en considérant en particulier la réponse des formes épisodiques, le développement des phénomènes de tolérance, la capacité du traitement d'interférer avec l'histoire naturelle de la maladie, l'apparition d'effects collatéraux dans les traitements prolongés. E’ stata valutata l'efficacia terapeutica della somministrazione, a breve e lungo termine, di litio carbonato in 90 pazienti affetti da forme episodiche (68 casi) o croniche (22 casi) di cefalea a grappolo. I risultati sono discussi con particolare riguardo alla risposta delle forme episodiche, allo sviluppo di fenomeni di tolleranza, alla capacità del trattamento di interferire con la storia naturale della malattia, alla comparsa di effetti collaterali in trattamenti prolungati.
