Aims and methodFollowing concerns over recruitment into general adult psychiatry, this study was undertaken to determine which factors influence senior trainees in psychiatry with regard to whether or not they choose a career in general adult psychiatry. The method used was by postal survey of all senior and specialist registrars in the UK.ResultsOf those responding, 32.6% had chosen a career in general adult psychiatry, while 38.9% had actively rejected such a career. Twenty-six per cent of respondents had chosen another speciality without actively rejecting general adult psychiatry. For those actively rejecting general adult psychiatry, the top three factors cited were poor resources (57.6%), high workload (43.2%) and poor working conditions (30.7%).Clinical implicationsThrough an awareness of why senior trainees reject general adult psychiatry as a possible career, resources can be targeted more specifically in the future at the factors responsible, and thereby improve recruitment into this speciality.

Aims and MethodThere have been major changes in the duration and the requirements of higher specialist training in psychotherapy in the UK. A postal survey was sent to all higher specialist trainees in psychotherapy to study their attitudes to, and experience of, these changes.ResultsEighty-nine per cent of trainees responded. The majority of trainees expressed concern about the reduction in length of training. There were gaps in provision of some essential training modalities. A substantial group of trainees did not have protected research time.Clinical ImplicationsPsychotherapy trainees believe the changes in training sacrifice depth for breadth. This highlights the ongoing debate about the future of psychotherapy within the NHS.

Aims and MethodA postal survey was sent to all consultant child and adolescent psychiatrists in the UK and Eire examining recruitment, retention, job satisfaction and job stress.ResultsA response was received from 333 (60%) child and adolescent psychiatrists. Sixty-one per cent indicated their service was inadequately resourced and 89% reported that their service failed to meet the College's minimum staffing requirements. Safe access to in-patient beds was not available to 71%. One hundred and thirty vacant posts were identified. Rates of psychological distress and burnout were high. Adequate services and the presence of a close, supportive colleague were associated with higher rates of satisfaction and lower rates of psychological distress and emotional exhaustion.Clinical ImplicationsA multi-faceted approach is suggested and recommendations are described under the headings of self-management, training, recruitment and commissioning.

Aims and MethodsTo ascertain current levels of access to home treatment for those with acute mental illness and future plans of trusts and purchasing authorities to provide such services. Also, to assess the attitudes of these organisations towards this form of treatment. A postal survey of all mental health trusts and purchasing authorities within the UK was carried out.ResultsOne hundred and seventy-two trusts and 82 health authorities returned questionnaires, representing a response rate of 75% and 67% respectively. Only 27 (16%) of trusts provided home treatment but 58 (40%) had plans to do so. All health authorities and 97% of trusts were in favour of the principle of providing home treatment.Clinical ImplicationsDespite the low levels of provision of home treatment trusts and health authorities were strongly in favourof it. There is likely to be a large increase in its availability over the next 12 months.

Aims and MethodDespite the Royal College of Psychiatrists' recommendation that higher specialist trainees in general adult psychiatry should use logbooks to record their training-related activities, none is currently available. We developed a logbook and conducted a postal survey of specialist registrars in general adult psychiatry in the West Midlands and Merseyside Training Schemes to elicit their perceptions of its usefulness and feasibility.ResultsAll of the 35 specialist registrars who completed the questionnaire felt that there was scope for a logbook and that this format was easy to use. Three-quarters of the respondents suggested that their training curriculum should be incorporated into the logbook and 71% felt that the use of logbooks ought to be mandatory. Despite all acknowledging the need for a logbook, only 14% were currently using one.Clinical ImplicationsLogbooks would greatly help higher specialist trainees to organise (identify, record and review learning objectives) the multifaceted aspects of their training. They would also help immensely in preparing for the Record of In-Training Assessment, appraisal and revalidation. There is clearly scope for a more ‘centrally’ produced (College) logbook for specialist registrars in general adult psychiatry, as indicated by the results of this survey. For logbooks to work in practice, they need to be simple and easy to use.

Aims and methodThe structure of academic training in psychiatry has changed in recent years and little is known about the trainees currently pursuing this career path. Two surveys were conducted of academic trainees in psychiatry and the heads of departments of psychiatry. These surveys aimed to identify the number of trainees currently in academic training, the nature of their positions and opinions about the current system of training in academic psychiatry.ResultsThere were 165 academic trainees identified, of whom 101 were not currently in academic clinical fellow (ACF) or academic clinical lecturer (ACL) posts. Academic trainees are located in a relatively small number of universities, with a quarter being based at one institution. In total, 60% of the trainees were in general adult psychiatry. Only 4.6% of respondents rated their academic training as excellent and just over half were certain that they wished to pursue an academic career in the future. Various challenges to academic training in psychiatry were identified by both the heads of departments and trainees.Clinical implicationsCurrent difficulties in academic training in psychiatry, such as lack of flexibility of the training pathway, need addressing to ensure the provision of high-quality research and teaching in psychiatry in the future.

AbstractEffective population screening for glaucoma would enable earlier diagnosis and prevention of irreversible vision loss. The UK National Screening Committee (NSC) recently published a review that examined the viability, effectiveness and appropriateness of a population-based screening programme for primary open-angle glaucoma (POAG). In our article, we summarise the results of the review and discuss some future directions that may enable effective population screening for glaucoma in the future. Two key questions were addressed by the UK NSC review; is there a valid, accurate screening test for POAG, and does evidence exist that screening reduces morbidity from POAG compared with standard care. Six new studies were identified since the previous 2015 review. The review concluded that screening for glaucoma in adults is not recommended because there is no clear evidence for a sufficiently accurate screening test or for better outcomes with screening compared to current care. The next UK NSC review is due to be conducted in 2023. One challenge for POAG screening is that the relatively low disease prevalence results in too many false-positive referrals, even with an accurate test. In the future, targeted screening of a population subset with a higher prevalence of glaucoma may be effective. Recent developments in POAG polygenic risk prediction and deep learning image analysis offer potential avenues to identifying glaucoma-enriched sub-populations. Until such time, opportunistic case finding through General Ophthalmic Services remains the primary route for identification of glaucoma in the UK and greater public awareness of the service would be of benefit.
