Relaparotomy in the treatment of postoperative complications of abdominal surgery remains a difficult problem of modern surgery.To evaluate the prognostic value of expression clusters of determination on immunocompetent cells of patients, the dynamics of HLA-DR + molecules and cytokines in the blood of patients with abdominal sepsis caused by severe peritonitis, as well as abscesses and phlegmons for postoperative period.Materials and methods.The study was conducted in 40 patients diagnosed with abdominal sepsis in the period from 2017-2020, which was treated in medical institutions of Chernivtsi and Ternopil in Ukraine, and which were divided into control and research groups. The control group consisted of 17 patients without signs of abdominal sepsis and acute surgical pathology. Determination of the expression of determination clusters such as CD11a, CD162, CD95, CD16 on immunocompetent cells, and the study of the dynamics of expression of HLA-DR + molecules and the content of cytokines IL-2, IL-4, IL-6 in the blood of patients was performed in the postoperative period. Both groups were representative by age, sex, comorbidities, risk factors. Determination of experimental parameters in the blood of patients was performed in the postoperative period (the results were evaluated before surgery, on the first, third, seventh, and on the fourteenth day of the postoperative period).Research results and their discussionIn patients with abdominal sepsis caused by severe peritonitis, the expression of HLA-DR + molecules on immunocompetent cells increases, which to some extent indicates an intensification of γ-interferon synthesis. At the same time, there is a sharp decrease in the content of IL-2 in the blood - the main regulator of a specific immune response.

Acute pancreatitis is one of the most pressing problems of modern pancreatology, according to the authors ranks first among surgical pathologies.The aim of this study was to evaluate the dynamics of cytokine profile in patients with abdominal sepsis caused by pancreatic necrosis.Materials and methods.The study was conducted in 12 patients diagnosed with abdominal sepsis caused by pancreatic necrosis, in the period from 2017 to 2020 who were treated in medical hospitals in Chernivtsi and Ternopil. The control group consisted of 17 patients without signs of abdominal sepsis and acute surgical pathology. Both groups were representative by age, sex, comorbidities, risk factors. Determination of experimental parameters in the blood of patients was performed in the postoperative period (the results were evaluated before surgery, on the first and tenth days of the postoperative period).Research results and their discussion.In patients with abdominal sepsis, a violation of the mechanisms of cell adhesion and costimulatory-cooperative interaction of immunocompetent cells was observed in the preoperative period, as indicated by the low level of CD11a + and CD162 + expression on them. The decrease in the content of IL-6 in the blood with a slight progressive increase in the level of IL-4 corresponds to a moderate imbalance of cytokine regulation of the immune response.

PURPOSE: To evaluate the effect of Terazosin and Propranolol on the prevention of necrosis induced by nicotine, in a random skin flap. METHODS: This study utilized 32 adult male Wistar-EPM rats divided, at random, into four groups of eight animals each. All the 32 animals received nicotine (2 mg/kg/day) subcutaneously, for one week before and one week after flap elevation. CG (Control) group received distilled water (0.2 ml/day) by gavage and saline (0.5 ml) intraperitoneally, for seven days in the postoperative period. TG (Terazosin) group received terazosin hydrochloride (3 mg/day) by gavage and saline, intraperitoneally, for seven days in the postoperative period. PG (Propranolol) group received propranolol (1.5 mg/day) intraperitoneally and distilled water, by gavage, following the stablished pattern. TPG (Terazosin + Propranolol) group received both drugs. On the seventh postoperative day, the distal necrotic area of the flaps was determined via the paper template method. Blood and skin samples were collected in order to allow determination of Malondialdehyde (MDA) levels RESULTS: The control group had a mean value of 39.5 % of necrosis; the Terazosin group 25.1 %; the Propranolol group 34.5 % and the Terazosin + Propranolol group 26.2 % of necrosis. MDA levels in the serum and in the skin samples behave similarly, with an exception regarding Propranolol group in this case. CONCLUSION: Terazosin is effective in the prevention of necrosis in this animal model and Propranolol is not effective in this case.

Aim. To create a mathematical model for identification of the relationship between the health of medical workers and the social and occupational factors, as well as a number of laboratory parameters. Methods. In an oncological hospital conducted was a cross-sectional study of the immune status of medical workers, principally the content of subpopulations of T and B lymphocytes. The model «level of health of medical workers» is based on an examination of 96 members of the medical staff who work on radiation emitting equipment, which were divided into five groups according to the nosological forms of identified diseases. The control group consisted of 98 individuals of the same medical facility, who are office workers. Results. In the individuals, who are exposed to radiation, revealed was a reduction in the functional activity of T lymphocytes, as well as an increase in the activity of natural killer cells. In the first and fifth groups (risk of tumors and diseases of the nervous system) noted was a sharp decrease in the expression of the antigens of the major histocompatibility complex system, and a reduction of the number of HLA-DR cells. In the first and second groups (risk of tumors and respiratory diseases) a decrease in the number of cells with receptors for transferrin CD71 occurred. Conclusion. The presented method of mathematical interpretation of the immunological parameters of blood will make it possible to determine the appurtenance of new study objects to a particular risk group, that is, to identify a predisposition to various diseases.

ABSTRACT The fractional determination of 17-ketosteroids in the daily urine was performed in nine cases of hyperemesis gravidarum and in four control cases, in the first trimester of pregnancy both before and after corticotrophin administration. The excretion of total 17-KS is similar in the two groups. Only in the hyperemesis group does the excretion of total 17-KS increase significantly after corticotrophin administration. The fractional determination reveals no difference between the two groups of patients with regard to the values of the fractions U (unidentified 17-KS), A (androsterone) and Rest (11-oxygenated 17-KS). The excretion of dehydroepiandrosterone is significantly higher in the hyperemesis group than in the control group. The excretion of androstanolone seems to be lower in the hyperemesis group than in the control group, but the difference is not statistically significant. The differences in the correlation between dehydroepiandrosterone and androstanolone in the two groups is significant. The high excretion of dehydroepiandrosterone and low excretion of androstanolone in cases of hyperemesis gravidarum is a sign of adrenal dysfunction.

Aim. To study the influence of deproteinized dialysate from blood of milk-fed calves on the type of exudative discharge and dynamics of inflammatory reaction after hernia repair with plastics of the anterior abdominal wall with hernioendoprosthesis of polypropylene. Materials and Methods. The study involved 59 patients being on stationary treatment in the surgical department of Kursk Regional Clinical Hospital. Patients were hospitalized for the herniation of small or medium dimension. The patients were divided to two groups: the main (n=30) and control (n=29) groups. After endoprosthetics, the patients of the control group were given complex conservative treatment. The patients of the main group, besides standard treatment, were administered deproteinized dialysate from blood of milk-fed calves intravenously in drips 10 ml + 200 ml of 0.9% sodium chloride solution within 7 days. For cytological examination and determination of the type of cytograms, the traumatic discharge was collected and analyzed using the method of M.F. Kamaev and M.A. Palthsev. Results. Cytomorphometric examination was conducted on the third, fifth and seventh day after endoprosthetics with the aim of studying dynamic changes. Determination of cell composition and also of its changes characteristic of each studied period, was necessary for obtaining further information characterizing inflammatory process in the region of placement of the endoprosthesis. After endoprosthetics in patients who were administered hemodialysate, the inflammatory reaction was less pronounced than in patients who did not receive the preparation. This was associated with a more dynamic change of stages of the inflammatory process. In patients of the main group who received deproteinized dialysate, regenerative type of inflammation first appeared on the fifth day and made 6.9%; by the seventh day the share of patients with the regenerative type rose to 17.5%, while in the patients of the control group no regeneration stage was observed in both periods. Conclusion. Analysis of the efficiency of influence of hemodialysate on the inflammatory reaction in plastics of the anterior abdominal wall with polypropylene endoprosthesis evidences faster course of all stages of inflammation and reduction of its intensity by 10%.

Topicality: The result of surgical treatment depends largely on the chosen method of management of the postoperative period, the main purpose of which is to reduce the effects of surgical trauma. Despite the wide variety of drugs for local and systemic use, the question of pharmacotherapy of the operated sinus remains open to this day. The main properties that a modern drug should possess are safety and high clinical efficacy. Aim: to study the clinical efficacy of topical application of a complex preparation based on a saline solution containing sodium hyaluronate and dexpanthenol in patients with chronic sinusitis after functional endoscopic sinus surgery (FESS). Materials and Methods: Clinical and laboratory studies were conducted on 47 patients with chronic rhinosinusitis who underwent surgery using the FESS technique. Patients were divided into 2 groups according to the principle of the postoperative period. Patients of research group (n=21), except for traditional therapy, was used locally NASOMER (a preparation based on a water-salt solution containing sodium hyaluronate and dexpanthenol); to the control group (n=26) patients entered, in the postoperative period received traditional therapy. Traditional therapy included a toilet of the nasal cavity, the use of short-course topical decongestants, irrigation of the nasal cavity with water-salt solutions. Criteria of clinical effectiveness: data of endoscopy of the nasal cavity, rhinomanometry, activity of the mucociliary transport system. The effectiveness of functional research methods in the postoperative period was determined on the 7th and 14th day of treatment. Results: On the 3rd day of the study, an increase in the swelling of the nasal cavity was observed in patients of both clinical groups, correlated with difficulty in nasal breathing. On the 7th day, a decrease in edema was observed in patients of both groups, but in the experimental group, the index of nasal breathing, according to rhinomanometry, was significantly (p<0.05) different from the control group, and was respectively 1.54±0.14 and 2.04±0.19 kPa/l*s. On the 14th day of the study, no significant difference was observed between these indicators. When studying the activity of the ciliated epithelium of the mucous membrane of the nasal cavity, it was proved that patients of the experimental group on day 7 after surgery showed a statistically significant difference in this indicator compared to the control group (17.8±1.0 and 22.7±2.1 min. respectively). Conclusions: The use of NASOMER after surgical intervention in patients with chronic rhinosinusitis contributes to more effective treatment compared with traditional therapy, as evidenced by the rapid recovery of the main functional parameters according to active posterior rhinomanometry and mucociliary clearance. Based on the results of the studies performed, the use of NASOMER for pharmacotherapy in the postoperative period is recommended for patients who have undergone rhinosurgical interventions with the aim of reducing the period of postoperative rehabilitation as an effective anti-inflammatory and wound-healing agent.

Relevance: Modern endonasal surgery allows to remove polyps from all affected paranasal sinuses, following the principles of minimal invasiveness. Minimal traumatic of surgical intervention gives the best results, accompanied by less progression of the disease. Minimal invasiveness of surgical intervention and its obligatory combination with postoperative medical treatment are common practice in many countries. One of the drugs that actively effect the restoration of the mucous membrane of the nasal cavity and paranasal sinuses in the early postoperative period is Nazomer, which includes sodium hyaluronate and dexpanthenol in saline solution. The purpose of the study is to investigate the effectiveness of the drug Nazomer in patients with polyposis rhinosinusitis after endoscopic polyposynosotomy. Results and discussion: The main group consisted of 30 patients with polyposis rhinosinusitis, who were prescribed Nasomer in addition to standard treatment in the postoperative period. The control group included 30 patients who underwent standard treatment in the postoperative period. As criteria for clinical efficacy, data from endoscopic examination of the nasal cavity and indicators of anterior rhinomatometry, measured by the «Optimus» device, were selected. In the main group, the index of nasal breathing according to rhinomatometry was better than the results of patients in the control group on the 3rd and 5th day of the postoperative period by 26% and 24%, respectively. Conclusion: The use of the drug Nazomer in the postoperative period in patients with polyposis rhinosinusitis contributes to more active restoration of respiratory function of the nasal cavity compared with the control group, according to rhinomatometry, up to 26%. Based on the results obtained, the drug Nazomer is an effective anti-inflammatory and regenerative agent in the postoperative period in patients with polyposis rhinosinusitis.

Objective: To evaluate the efficacy and safety of patient-controlled analgesia through prolonged epidural analgesia after joint replacement of the lower extremities. Material and methods. We analyzed the postoperative period of 213 elderly and senile patients who were operated on for degenerative-dystrophic and traumatic injuries of the joints of the lower extremities. All patients underwent total joint replacement (164 - THA and 49 - TKA). The age of patients is from 65 to 90 years (average age was 78 ± 8 years) with a physical status of ASA 3 and above. All examined patients were divided into 2 groups. 63 patients comprised the main group, which in the postoperative period underwent patient-controlled analgesia (PCA) through prolonged epidural analgesia. The control group consisted of 150 patients, for the anesthesia of which in the postoperative period only standard systemic multimodal analgesia was used Conclusion. Patient-controlled analgesia is an alternative to traditional analgesic regimens. This method should be one of the main methods after surgical anesthesia for joint replacement of the lower limb in elderly and senile patients.

Railway bogie is the basic element that determines the force, kinematic, power and other parameters of the rolling stock, and its movement in the railway track has not been studied enough. Classical calculation of the kinematic and dynamic parameters of the bogie's motion with the determination of the position of its center of rotation, the instantaneous axes of rotation of wheelsets, the magnitudes and directions of all forces present a difficult problem even in quasi-static theory. The paper shows a simplified method that allows one to explain, within the limits of one article, the main kinematic and force parameters of the bogie movement (installation angles, clearance between the wheel flanges and side surfaces of the rails), wear and contact damage to the wheels and rails. Tribology of the railway bogie is an important part of transport tribology, the foundation of the theory of wheel-rail tribosystem, without which it is impossible to understand the mechanisms of catastrophic wear, derailments, contact fatigue, cohesion of wheels and rails. In the article basic questions are considered, without which it is impossible to analyze the movement of the bogie: physical foundations of wheel movement along the rail, types of relative motion of contacting bodies, tribological characteristics linking the force and kinematic parameters of the bogie. Kinematics and dynamics of a two-wheeled bogie-rail bicycle are analyzed instead of a single wheel and a wheelset, which makes it clearer and easier to explain how and what forces act on the bogie and how they affect on its position in the rail track. To calculate the motion parameters of a four-wheeled bogie, it is represented as two two-wheeled, moving each on its own rail. Connections between them are replaced by moments with respect to the point of contact between the flange of the guide wheel and the rail. This approach made it possible to give an approximate estimation of the main kinematic and force parameters of the motion of an ideal bogie (without axes skewing) in curves, to understand how the corners of the bogie installation and the gaps between the flanges of the wheels and rails vary when moving with different speeds, how wear and contact injuries arise and to give recommendations for their assessment and elimination.

Abstract This study describes changes in haematological parameters, cytokine profile, histopathology and cortisol levels in Swiss mice experimentally infected with Angiostrongylus costaricensis. Twenty-eight Swiss mice were divided into two groups (G1 and G2) of 14 animals each. In each group, eight animals were infected orally with ten third-stage larvae of A. costaricensis and six were used as a control group. The mice of groups G1 and G2 were sacrificed 14 and 24 days after infection, respectively. Samples were collected for histopathological and haematological analyses and determination of the cytokine profile and cortisol levels. Granulomatous reaction, eosinophilic infiltrate and vasculitis in the intestinal tract, pancreas, liver and spleen were observed with varying intensity in infected animals. Our results showed that the mice developed normocytic and hypochromic anaemia, and that the histopathological lesions caused by the experimental infection influenced increases in cortisol, neutrophil and monocyte levels. In addition to this, we detected increased interleukin-6 and tumour necrosis factor alpha levels in the infected animals.
