The aim of this study was to evaluate whether there is any correlation between periodontal disease and mortality contributing factors, such as cardiovascular disease and diabetes mellitus in the elderly population. A dental evaluation was performed by a single examiner at Tufts University dental clinics for 284 patients. Periodontal assessments were performed by probing with a manual UNC-15 periodontal probe to measure pocket depth and clinical attachment level (CAL) at 6 sites. Causes of death abstracted from death certificate. Statistical analysis involved ANOVA, chi-square and multivariate logistic regression analysis. The demographics of the population sample indicated that, most were females (except for diabetes mellitus), white, married, completed 13 years of education and were 83 years old on average. CAL (continuous or dichotomous) and marital status attained statistical significance (p<0.05) in contingency table analysis (Chi-square for independence). Individuals with increased CAL were 2.16 times more likely (OR=2.16, 95% CI=1.47-3.17) to die due to CVD and this effect persisted even after control for age, marital status, gender, race, years of education (OR=2.03, 95% CI=1.35-3.03). CAL (continuous or dichotomous) was much higher among those who died due to diabetes mellitus or out of state of Massachusetts. However, these results were not statistically significant. The same pattern was observed with pocket depth (continuous or dichotomous), but these results were not statistically significant either. CAL seems to be more sensitive to chronic diseases than pocket depth. Among those conditions, cardiovascular disease has the strongest effect.

⋄ Objective Loss of residual renal function (RRF) in peritoneal dialysis (PD) patients is a powerful predictor of mortality. The present study was conducted to determine the predictors of faster decline of RRF in PD patients in Taiwan. ⋄ Methods The study enrolled 270 patients starting PD between January 1996 and December 2005 in a single hospital in Taiwan. We calculated RRF as the mean of the sum of 24-hour urea and creatinine clearance. The slope of the decline of residual glomerular filtration rate (GFR) was the main outcome measure. Data on demographic, clinical, laboratory, and treatment parameters; episodes of peritonitis; and hypotensive events were analyzed by Student t-test, Mann–Whitney U-test, and chi-square, as appropriate. All variables with statistical significance were included in a multivariate linear regression model to select the best predictors ( p < 0.05) for faster decline of residual GFR. ⋄ Results All patients commencing PD during the study period were followed for 39.4 ± 24.0 months (median: 35.5 months). The average annual rate of decline of residual GFR was 1.377 ± 1.47 mL/min/m2. On multivariate analysis, presence of diabetes mellitus ( p < 0.001), higher baseline residual GFR ( p < 0.001), hypotensive events ( p = 0.001), use of diuretics ( p = 0.002), and episodes of peritonitis ( p = 0.043) independently predicted faster decline of residual GFR. Male sex, old age, larger body mass index, and presence of coronary artery disease or congestive heart failure were also risk factors on univariate analysis. ⋄ Conclusions Our results suggested that diabetes mellitus, higher baseline residual GFR, hypotensive events, and use of diuretics are independently associated with faster decline of residual GFR in PD patients in Taiwan.

Abstract Objectives The purpose of this study was to analyze and to compare retrospectively the characteristics, the treatment process, and the prognosis of cracked teeth by comparing recent data with data from 10 years ago. Materials and Methods Sixty-eight cracked teeth from March 2009 to June 2010 (2009 data) and 185 cracked teeth from March 2019 to June 2020 (2019 data) were analyzed. The characteristics of cracked teeth and the treatment method depending on probing depth, caries, and symptoms, and prognosis depending on pocket depth and apical lesions were analyzed using R version 3.3.3 (R Foundation for Statistical Computing, Vienna, Austria) and T&F version 3.0 (YooJin BioSoft, Korea). To compare proportions, the two-sample proportion test was performed. The distribution of proportions within the samples from 2009 and 2019 data was analyzed using the Chi-square test or binomial test. A p-value <0.05 was considered to indicate statistical significance. Results Significantly fewer cracked teeth received root canal treatment before crown in 2019 than in 2009 (p = 0.032). In both 2009 and 2019, symptomatic cracked teeth and those with deep periodontal pockets (>6 mm) were significantly more likely to receive root canal treatment. In both years, cracked teeth with a probing depth less than 6 mm or without an apical lesion were significantly more likely to be asymptomatic at 3-month and 6-month follow-ups (p < 0.001). Conclusion Cracked teeth with a deep periodontal pocket or symptoms had a higher likelihood of endodontic treatment, and the presence of a deep periodontal pocket or apical lesion was associated with a higher risk of persistent symptoms. Therefore, clinicians should consider these factors when planning treatment and predicting patients’ prognosis.

A hipertensão arterial sistêmica (HAS), diabetes mellitus (DM), sedentarismo e síndrome metabólica afetam a saúde indígena. Este estudo objetivou identificar hipertensos e diabéticos na etnia Xavante, Mato Grosso. A amostra foi de 50 indígenas, caracterizados quanto ao sexo, idade, escolaridade, renda, estado civil e número de filhos. Fez-se a anamnese e avaliação clínica, com antecedentes de DM e/ou HAS, uso de medicações tradicionais ou não. Usaram-se tensiômetro digital de pulso e um glicosímetro capilar. Fez-se a análise com os testes qui-quadrado, Pearson, Kruskal wallis, t de Student, Coeficiente de correlação de Pearson e ANOVA. Um terço dos participantes recebiam até um salário mínimo; 22% eram compatíveis com pré-diabetes e 26% diabéticos; a HAS esteve em uma média de 122,5 (±14,5) por 79,8 (±9,4) mmHg. Houve relação significativa entre a renda, estado civil, idade e presença de filhos, com os dados clínicos e os aspectos multifatoriais de risco.Descritores: Indígena, Hipertensão Arterial, Diabetes Mellitus. Tracking chronic disease in an indigenous communityAbstract: Systemic arterial hypertension (SAH), diabetes mellitus (DM), physical inactivity and metabolic syndrome affect indigenous health. This study aimed to identify hypertensive and diabetic people from Xavante ethnic group in Mato Grosso - Brazil. The sample consisted in 50 indigenous people, characterized by gender, age, educational level, income, marital status and number of children. It was made the anamnesis and a clinical evaluation, asking for precondition on DM and/or SAH, and the use or not of traditional medicine. It was used a digital wrist blood pressure monitor and a capillary blood glucose meter. It was performed a chi-square test, a Pearson test, a Kruskal Wallis test, a Student's t test, a Pearson's correlation coefficient and an ANOVA tests. One third of the participants received up to one minimum wage; 22% of them were compatible with pre-diabetes and 26% were diabetic; the average the SAH was 122.5 (± 14.5) by 79.8 (± 9.4) mmHg. This study examined the prevalence of highly and minimally leukotoxic Actinobacillus actinomycetemcomitans in patients with periodontal disease. Pooled subgingival plaque samples from 136 patients with some form of periodontal disease were examined. Subjects were between 14 and 76 years of age. Clinical examinations included periodontal pocket depth (PD), plaque index (PI) and bleeding index (BI). The obtained plaque samples were examined for the presence of highly or minimally leukotoxic A. actinomycetemcomitans strains by the polymerase chain reaction (PCR). Chi-square and logistic regression were performed to evaluate the results. Forty-seven subjects were diagnosed with gingivitis, 70 with chronic periodontitis and 19 with aggressive periodontitis. According to chi-square there was no significant correlation detected between PD (chi2 = 0.73), PI (chi2 = 0.35), BI (chi2 = 0.09) and the presence of the highly leukotoxic A. actinomycetemcomitans. The highly leukotoxic A. actinomycetemcomitans strains were correlated with subjects that were 28 years of age and younger (chi2 = 7.41). There was a significant correlation between highly leukotoxic A. actinomycetemcomitans and aggressive periodontitis (chi2 = 22.06). This study of a Brazilian cohort confirms the strong association between highly leukotoxic A. actinomycetemcomitans strains and the presence of aggressive periodontitis.

Abstract Background: The pathogenesis of non-traumatic osteonecrosis of the femoral head (ONFH ) is related to the interruption of blood supply caused by lipid metabolism and hypercoagulability. The purpose of this study was to explore the relationship between clinical biochemical parameters and non-traumatic ONFH. Methods: The basic information and biochemical indexes of 1292 patients with non- traumatic ONFH and 1880 healthy controls were collected. SPSS software (version 22.0) was used to process and analyze the data. T-test was used for quantitative analysis. Chi-square test was used for categorical variables.p< 0.05 were the index with statistical significance. Results: In the population sample, TC (p= 0.00004), LDL (p= 0.014) and PLT (p= 0.000005) levels were statistically significant between the two groups.In men, levels of TC (p = 0.004), LDL (p= 0.011), and PLT (p= 0.00005) were statistically significant between the two groups.In women, TC (p= 0.001) and PLT (p= 0.048) levels were statistically significant between the two groups.There were differences in TC (p= 0.00001) and PLT (p= 0.031) levels between the case group and the control group in samples aged less than 45 years.There were differences in LDL (p= 0.00002) and PLT (p= 0.022) levels between the two groups in samples older than 45 years.Compared with the control group, patients with alcohol-induced ONFH had HDL (p = 0.002).LDL (p= 0.00002);The level of PLT (p= 0.0001) was significantly different.HDL (p = 0.005) was found in alcohol-induced ONFH patients younger than 45 years of age.The PLT level (p=0.045) was different from that of the control group.There was a difference in LDL (p= 0.000003) levels between control and alcohol-induced ONFH patients older than 45 years.The older the onset age, the TC in vivo;HDL;LDL;ApoA1;ApoB;The lower the PLT level.With the prolongation of the onset time, the PLT level in the patient's body is decreasing continuously.Conclusion: The changes of biochemical indexes are closely related to the occurrence of non-traumatic ONFH. Our research can provide a new direction for the prevention of ONFH.

Objective: This study aims to determine the association between The quick Sequential Organ Failure (qSOFA) score and the mortality of Ludwig's Angina. Material and Methods: Secondary data from the medical records of Ludwig’s Angina patients in the period January 2019 - December 2020. Data included demographic feature, qSOFA data, Outcome patient, comorbidity diseases and complications before treatment. The number of samples was determined using the total sampling method. This study was enrolled into analytical observational study. Results: There were 49 medical records that were included in the inclusion criteria, 6 medical records were excluded because the data were missing. There were more men 36 (73,5%) than women 13 (26,5%). There were 12 (24,5%) high qSOFA and 37 (75,5%) low qSOFA. There were 12 (24,5%) patient outcome dead and 37 (75,5%) outcome alive. The results of Chi-square analysis show qSOFA and confounding Diabetes Mellitus (DM) respectively were statistically significant association with Ludwig's Angina mortality. The results of multivariate logistic regression analysis, shows that there is a significant association between qSOFA and Ludwig's Angina mortality (p: 0.018) with an OR of 20.9 (CI 95% : 1,673 – 263,367). Diabetes mellitus comorbidity factors were also significantly associated with the mortality of Ludwig's angina. Conclusion: There is a significant association between qSOFA and Ludwig's Angina mortality. Keywords: Ludwig’s Angina, Mortality, qSOFA, Sepsis.

Diabetes mellitus is a risk factor for development of periodontal disease with progressive periodontal destruction seen in diabetic patients. Advances in oral and periodontal disease diagnostic research are moving towards methods whereby periodontal risk can be identified and quantified by objective measures such as biomarkers. The aim of this study was to identify the presence of beta glucuronidase activity as an enzymatic biomarker of periodontal tissue destruction in patients of Diabetes mellitus associated with Periodontitis. β Glucuronidase activitywas estimated in saliva, GCF (Gingival Crevicular Fluid) and serum in healthy subjects and patients with Diabetes mellitus, Periodontitis and Diabetes associated periodontitis. Enzyme activity was compared with clinical parameters like Probing pocket depth, Plaque Index and Gingival Index. It was maximally raised in diabetic cases with periodontitis. Enzyme activity was found to be highest in crevicular fluid compared to saliva and serum. However, serum Glucuronidase correlated significantly with clinical indices. β-glucuronidase may be employed on routine basis as a chair side test for screening and diagnosis of patients with periodontitis in diabetics.

Introduction: Smile line is a major factor considered in esthetic treatment planning.Objective: To assess the position of the smile line both in natural and forced position in relation to age and gender in Nepali population sample.Materials & Method: A cross sectional study was done in 220 adults. DSLR camera was used to take photograph of the participants. Two different photographs were taken of each participant in natural and forced smile position in standardized format. The smile lines were determined and classified according to Liebart et al. Data analysis was done by SPSS 20.0 software. Chi square test was performed to analyze statistical significance between the gender and age groupsResult: Maximum individuals had low type of natural smile (59.1%) and average type of forced smile (40.0%). Very few individuals had very high type of natural smile (1.4%) and very high type of forced smile (15.9%). Among all, 41% had visibility of the periodontium during normal smile while on forced smile 79.1% had visibility of the periodontium.Conclusion: Females have significantly more periodontal visibility when compared to males during forced smile in Nepali population sample. This concept of smile line should be utilized for treatment planning to achieve desirable facial esthetics in many fields of dentistry including Orthodontics, Prosthodontics, Periodontics, Restorative Dentistry.
