The search for the etiologic agents of periodontal diseases started in the Golden Era of medical bacteriology, when the etiologic agents of many bacterial infections were isolated and characterized. After the initial enthusiasm in establishing the infectious nature and the true agents of periodontal diseases, this concept was virtually ignored for the next four decades. Until the early 1970s treatment regimens based on the non-specific plaque hypothesis were directed towards a non-specific reduction in plaque amount. Later, the specific plaque hypothesis established the role of some microorganisms such as A. actinomycetemcomitans, P. gingivalis, T. forsythensis, T. denticola, P. intermedia and F. nucleatum in different forms of periodontal diseases. It was recently suggested that these suspected periodontal pathogens seem to not act alone and interactions between species, especially the balance between pathogenic and beneficial species affect both progression of disease and response of tissues to periodontal therapy. Nowadays it is well established that one of the goals of therapy is to control such periodontal pathogens. Among the most commonly used therapies to treat periodontal infections are scaling and root planing (SRP), supragingival plaque control and periodontal surgeries. Many studies confirmed the reduction of "red complex" species by SRP, and apically repositioned flap can lead to an additional beneficial effect in the subgingival microbiota by decreasing levels of "red" and "orange complexes" species. Furthermore, the level of plaque control maintained by the patients has been considered a crucial step in preventing recurrence of destructive periodontitis.

Periodontal diseases are recognized as bacterial infections, and some forms are associated with specific organisms, such as Actinobacillus actinomycetemcomitans in juvenile periodontitis, and Bacteroides gingivalis and others in adult periodontitis. The source of the periodontal organisms, whether they are part of the indigenous or resident flora and overgrow to become opportunistic oral pathogens, or whether they are exogenous oral pathogens, is important to determine. The chain of periodontal infection, microbial agent(s) and their transmission, and host response are reviewed with respect to the role of A. actinomycetemcomitans in localized juvenile periodontitis and B. gingivalis in adult periodontitis. The present data lead us to hypothesize that some periodontal organisms may be exogenous pathogens. Prevention of periodontal diseases may be influenced by the knowledge of whether various forms are caused by opportunistic organisms or exogenous pathogens. If exogenous pathogens are responsible, prevention can be directed to intercepting transmission, thereby preventing colonization. On the other hand, if the organisms are opportunistic pathogens, prevention might be directed at interfering with initial acquisition of the flora earlier in life, as well as suppressing them to low levels consistent with health. For those exogenous periodontal infections, attempts at eradication and prevention of re-infection are likely to be effective. If the organisms are part of the indigenous flora, there is little hope of complete elimination of the organism. Criteria for distinguishing exogenous periodontal pathogens from opportunistic periodontal pathogens include the prediction that exogenous pathogens would be transient members of the oral flora associated with periodontal disease, likely to be comprised of one or a few clonal types, and intrinsically virulent. In contrast, opportunistic periodontal pathogens would likely be members of the indigenous flora and would overgrow. They would likely be comprised of many clonal types, and have an intrinsically low level of virulence.

Abstract Dental researchers have attempted to understand the microbial nature of oral diseases over the past 120 years. Their view of plaque and its constituent microorganisms has shifted from a specific plaque hypothesis to a non-specific plaque hypothesis and back again to a theory of specific periodontal pathogens in plaque. Changes in the way plaque and its microorganisms are viewed affect the strategies used to prevent and control periodontal diseases. In recent years, dental researchers have begun to view plaque as a biofilm. This shifting view of plaque has important implications for future efforts in prevention and treatment. This article describes the various ways that dental professionals have viewed plaque throughout the years and highlights the current view of plaque as a biofilm and the ramifications for periodontal therapy.

ABSTRACT Periodontitis is an inflammatory condition of supporting tissues of teeth, for which several risk and susceptibility factors are proposed. Periodontal disease results when balance between host factors and etiologic agents is disrupted. Bacteria have a primary role in the initiation of periodontal disease, and a range of host related factors influence the clinical presentation and rate of progression of disease. Genetic variations that modify immunological reactions identify the disease susceptibility in various individuals. Many studies have proved the effect of various single or composite nucleotide polymorphisms to susceptibility, progression or severity of periodontal diseases. Despite these studies, association between periodontal disease and candidate genes is still not clear. The reports of familial nature of chronic periodontitis are less frequent as compared to aggressive periodontitis. The striking familial aggregation of trait in aggressive periodontitis is consistent with significant genetic etiology. In this paper, an attempt has been made to summarize recent views on various genes involved in the pathogenesis and progression of aggressive periodontal disease. Data were identified by searches of the Medline, and Pubmed. Articles published in English were selected, and most up-to-date or relevant references were chosen.

Background: Different host proteins play a central role in cell response during bacterial infections, the Bcl-2-Associated X protein (BAX) and Vascular Cell Adhesion Protein 1 (VCAM-1) are often reported in infective primary events during cell injury. Objective: The aim of this study is to evaluate the predictive value of these two proteins as biomarkers of oral bacterial infection, with particular emphasis on the tongue, which plays an important role in microbial homeostasis in the mouth. Methods: Twenty-nine patients were recruited and divided according to the Periodontal Index (CPI), 4 of them were severely compromised periodontal patients. Oral hygiene, gingival tissues and plaque presence were evaluated clinically. The laboratory analysis carried out on tongue tissue included: total bacterial genomes, proportion of specific periopathogens and BAX -VCAM-1 expression rate, while Reactive Oxygen Species (ROS) were measured in saliva. Results: Neither tongue microbiological status nor salivary ROS level corresponded with the state of disease. VCAM-1 mRNA expression rate was comparable in all patients but, on the contrary, BAX expression resulted high in periodontally-compromised patients and appears related to periodontal status in the analyzed subjects. Conclusion: This preliminary work suggests that the BAX protein is a possible candidate in a prognostic marker study for oral diseases started by periodontal bacteria. For example, none of the evaluated clinical and microbiological parameters could predict the presence, prognosis or recurrence of periodontal diseases. This biomarker could be a valuable tool in determining the risk, diagnosis and prognosis of this human illness.

The diagnosis of periodontal disease is mainly based on use of clinical and radiographic evidence. In this study we employed a quantitative PCR analysis of Aggregatibacter actinomycetemcomitans and Treponema denticola as species strongly involved in periodontal diseases, burden in periodontal pockets to detect the main sampling factors that interfere with qPCR results. From 22 patients with advanced periodontal disease, subgingival plaque was comparatively collected by paper points and periodontal Gracey curettes. Samples were collected from the same situs in presence of gingival bleeding and absence of bleeding. The concordance and agreement of results between samples were assessed. The present study demonstrates that subgingival plaque sampling with sterile absorbable paper points is often accompanied by gingival bleeding resulting in quantification biases of periodontal pathogens.

The aim of this study was to evaluate the clinical and microbiological effects of subgingival instrumentation (SI) alone or combined with either local drug delivery (LDD) or photodynamic therapy (PDT) in persistent/recurrent pockets in patients enrolled in supportive periodontal therapy (SPT). A total of 105 patients enrolled in SPT were randomly treated as follows: group A (n = 35): SI +PDT and 7 days later 2nd PDT; group B (n = 35): SI+LDD; group C (n = 35): SI (control). Prior intervention, at 3 and 6 months after therapy, probing pocket depths, clinical attachment level, number of treated sites with bleeding on probing (n BOP), full mouth plaque and bleeding scores (gingival bleeding index, %BOP) were recorded. At the same time points, 8 periodontopathogens were quantitatively determined. All three treatments resulted in statistically significant improvements (p < 0.05) of all clinical parameters without statistically significant intergroup differences (p > 0.05). Several bacterial species were reduced in both test groups, with statistically significantly higher reductions for LDD compared to PDT and the control group. In conclusion, the present data indicate that: (a) In periodontal patients enrolled in SPT, treatment of persistent/recurrent pockets with SI alone or combined with either PDT or LDD may lead to comparable clinical improvements and (b) the adjunctive use of LDD appears to provide better microbiological improvements for some periodontal pathogens than SI alone or combined with PDT.

Background: Spontaneous pneumomediastinum or mediastinal emphysema is relatively a rare disease that occurs in viral and bacterial infections with the benign entity. Case: The patient was a 57-year-old man who, after a week of self-isolation at home, coming to the COVID-19 triage center of the hospital with severe shortness of breath. He was admitted to the Intensive Care Unit (ICU) due to acute respiratory failure. The patient was treated as the protocol designed and respiratory support with high flow nasal oxygen and Non-Invasive Positive Pressure Ventilation (NIPPV). Spontaneous pneumomediastinum was developed during viral pneumonia infection. The known risk factors included age, male sex, and abnormal laboratory finding. All the biochemical and hematological findings such as lymphopenia, thrombocytopenia, raised CRP, LDH, and ferritin were detected in our cases. They indicate a possible prognosis for the development of acute respiratory failure and adverse clinical outcomes. Conclusion: Spontaneous pneumomediastinum has usually a benign outcome in COVID-19 Pneumonia and its prognosis is related to background conditions in patients.

Objective: the aim of this study was to evaluate, through literature review, clinical aspects (plaque index, gingival index, probing depth, gingival recession and clinical attachment level) and microbiological (qualitative and quantitative presence of periodontal pathogens) in smokers patients and the response to periodontal therapy. Material and Methods: it was selected scientific articles published between the years 1978 and 2014. We were selected scientific articles from MEDLINE, PUBMED and LILACS databases. Results: there are controversies between tobacco and clinical and microbiological parameters. Conclusion: based on the evaluated studies we can conclude that there are significant clinical and microbiological differences in smoking patients and impaired response to periodontal therapy.

Pseudomonas aeruginosa and Staphylococcus aureus are bacteria pathogens that cause a myriad of infections affecting various sites in the body including the eyes, ears, lungs, skin, heart, bones, and blood amongst others. These bacteria can be disseminated via the blood to other parts of the body away from the primary site of infection and consequences vary from mild to severe with death occurring in certain instances. Both bacterial infections can occur individually, as well as in co-infection resulting in even worse outcomes. P. aeruginosa and S. aureus exhibit multidrug resistance against current antibiotic treatment regimens, which accentuates the challenge in managing the infections caused by these bacteria. To prevent the looming era of untreatable bacterial infections, alternative treatment regimens that are cost effective and accessible are needed. To explore novel treatment options, twenty-five organic compounds comprising fatty acids and their derivatives were screened for antibacterial activity in broth microdilution assay to determine the minimum inhibitory concentration and minimum bactericidal concentration against both P. aeruginosa and S. aureus. Five candidates (N–nonanoic acid, butyric acid, heptanoic acid, palmitoleic acid, and isopropyl myristate) were effective against P. aeruginosa. Seven candidates (N–nonanoic acid, palmitoleic acid, tridecanoic acid, sebaic acid, undecanoic acid, monolaurin, and monocaprin) were effective against S. aureus. Candidates such as N–nonanoic acid and palmitoleic acid were effective against both P. aeruginosa and S. aureus, demonstrating that the same fatty acids show potential to be used against both Gram negative and Gram positive bacterial infections.

Aim. The study aims at enhancing the therapeutic effect and reducing the duration of treatment by using a solution of titanium dioxide in ozonated distilled water for antiseptic treatment of periodontal tissues. Material and Methods. Treatment results of local inflammatory changes of periodontal tissues in 128 patients (73 female, 55 male aged 21-65 years) were analyzed and classified by the severity of periodontal disease as: gingivitis, primary and moderate periodontitis. Schemes of medical actions differed in pharmacotherapy. In order to ascertain the most effective approach, the results of conventional and developed treatment complexes were compared. Results and Discussion. Digital indicators of the periodontal tissues condition following treatment presented common positive dynamic in all patients. However, significantly better results were obtained by using the developed method. According to the dynamics of bleeding index values, the proposed treatment regimens allow achieving more effective control over the inflammatory process. Digital indicators of hygiene showed that a better effect was reached after the suggested method - the dynamics of reducing the depth of periodontal pockets demonstrated clearer and statistically better results. Changes in the pH of the gingival fluid in a severe inflammatory condition indicated a considerable improvement after using the suggested treating method. X-ray data demonstrated the stability of digital values in the observation groups before and after treatment, which was interpreted as a sufficient control of destructive processes. Conclusion. Mathematical analysis of the presented data demonstrates the benefits of the developed method for the treatment of periodontal diseases, especially in non-surgical treatment regimens. The performed research confirms greater effectiveness of the therapeutic method in initial lesions, and thus it proves the efficiency of complex control for preventive purposes
