The problem of prevention and treatment of vascular diseases of the brain is becoming more and more important every year due to the prevalence of the disease, the severity of disorders, high mortality and disability. Therefore, the issues of organizing medical care for such a contingent of patients are very relevant.

INTRODUCTION: The results obtained from the analysis of age dynamics are insufficient for development a program of reducing morbidity and, ultimately, mortality, since a need of medical care already indicates a problem; so prevention and screening programs should surpass the emergence of these problems. AIM: To analyze the age dynamics of the need for outpatient medical care for conditions that determine the basic value of the need in women with diseases of the circulatory system. MATERIALS AND METHODS: We used data on 780,000 visits during one year to medical organizations that provide medical care on an outpatient basis. We studied the probability of an episode and the dynamics of the frequency of episodes depending on age in six five-year age groups: 31 to 35 years, 36 to 40 years, 41 to 45 years, 46 to 50 years, 51 to 55 years, and 56 to 60 years. The dynamics of the frequency of episodes was modeled by a mathematical function with the largest value of the approximation coefficient. The dynamics of the frequency of episodes was modeled by a mathematical function with the largest value of the approximation coefficient. RESULTS: The dynamics of the need for outpatient medical care for hypertensive diseases [Diseases characterized by high blood pressure] (I10 – I15), ischemic heart diseases (I20 – I25) and vascular diseases of the brain (I60 – I69) is characterized by an increasing trend in demand indicators throughout the studied age period. The greatest increase in indicators is observed in the age groups of 31–40 years for ischemic heart diseases and vascular diseases of the brain and 41–50 years for hypertensive conditions. The greatest increase in demand is observed for coronary heart diseases (I20 – I25). The total amount of resources required increases more than 350 times over the 30-year age period, despite the stable value of the average duration of treatment. The increase in the total amount of resources required for vascular diseases of the brain is 30 times. The lowest growth gradient is registered in relation to hypertensive conditions (10-fold growth). As a critical age period, it is most appropriate to consider the age of a woman under 35 years. CONCLUSION: Mathematical modeling is optimally performed using power functions for hypertensive conditions and vascular diseases of the brain and exponential functions for ischemic heart diseases. All the proposed models are characterized by high approximation coefficients.

The aim of the study is to improve the organization and provision of surgical care to civilians with gunshot wounds to the chest in the context of armed conflict on the example of the Chechen campaign.Materials and methods. The work is based on the analysis of medical care provided to 106 wounded with gunshot wounds to the chest from the civilian population in the period from 1991 to 2000. Medical care was provided on the basis of the surgical hospital of the city hospital No. 9 in Grozny. Results and discussion. Found that providing medical care to wounded with gunshot wounds of the chest when massive flow in terms of actual hostilities rendered by the forces and means of civil hospitals, has a certain specificity. It is due to the fact that in such conditions, in contrast to military medical organizations, there are no stages of evacuation, often the wounded are delivered by non-specialized and unsuitable vehicles on destroyed roads, while they are not given or given insufficient pre-medical care. In such conditions, the workload of medical personnel of civil medical institutions associated with the provision of medical care increases. In this regard, in our opinion, it is necessary to develop measures aimed at improving the algorithm aimed at minimizing various errors that occur in civilian medical institutions during the mass admission of wounded.

Fifteen cases of presumed cocaine intoxication were evaluated in the emergency room (ER) at a city hospital over a four-day period. This series is unique in that many of these patients were from a similar area of the city, in some cases had the same street address, were regular abusers of cocaine, and presented to the ER with similar symptoms of tachycardia, dilated pupils, marked confusion, bizarre and sometimes violent behavior, psychosis, and hallucinations. Many of these symptoms were present several hours after drug use. Samples of a white powder presumed by the patients to be cocaine were obtained from two patients and analyzed by gas-liquid chromatography. Neither sample contained cocaine, but rather revealed atropine, benzocaine, and procaine. The signs and symptoms of cocaine, amphetamine, and atropine intoxication are reviewed and the problems of drug analysis and differential diagnosis of drug intoxication are discussed.

Abstract Twenty-four patients with petroclival meningiomas were operated upon at the neurosurgical clinic of the City Hospital of Hannover between 1978 and 1987. Seventeen were women and seven men; the mean age was 45 years. Symptoms were usually present for more than 2 years before the diagnosis was made. The most common symptom was disturbance of gait; the most common preoperative sign was cranial nerve deficit, mainly of the 7th and 8th nerves. Preoperative neuroradiological evaluation included computed tomography and four-vessel cerebral angiography. Fifteen patients (62%) had a tumor larger than 2.5 cm in its major diameter. The surgical approaches used were the retromastoid, pterional, subtemporal, and combined retromastoid-subtemporal. We developed a modification of the retromastoid-subtemporal approach with preservation of the transverse sinus and used this in the last 2 patients. There was no postoperative death; 11 patients (46%) suffered postoperative complications, mainly in the form of cranial nerve deficits, often reversible. “Total” tumor removal was achieved in 17 patients (71%). Twenty patients (83%) were independent at the time of discharge from the hospital. With accurate neuroradiological evaluation, careful choice of the surgical approach, and sound application of microsurgical techniques, petroclival meningiomas may be “totally” and safely resected in a significant number of patients.

Most violence connected with sleep disorder is assumed to be related to sleep walking. It is less well known that other sleep disorders can also give rise to violence. The role of narcolepsy in car accidents is mentioned. Sleep drunkenness can lead to confusion resulting in violent behaviour especially on forced awakening. This condition is associated to sleep apnea. Primary or central sleep apnea is caused by disorders of the brain stem affecting the respiratory center. Secondary or upper airway sleep apnea can be caused by virtually any condition that results in cessation of the airflow due to occlusion of the upper airway. The author describes one patient who engaged in assaultive behaviour on forced awakening following earlier alcohol consumption. The pathomechanism of violent behaviour generated by a combination of sleep apnea and respiratory pathology is described. The differential diagnosis, prevention and treatment is outlined. The use of polysomnography in diagnosis and the potentially dangerous effects of drugs with respiratory depressing effects is highlighted.

In recent years, cerebral vascular diseases have been increasingly detected in young patients. It is due not only to better physicians’ knowledge about this pathology, but also to the improvement of its diagnosis methods. Modern neuroimaging techniques allow us to clarify the nature of hemorrhage, to determine the volume and location of intracerebral hematoma, and to establish the degree of concomitant edema and dislocation of the brain. However, despite the high accuracy of the research, it is not always possible to establish the cause that led to a brain accident, which greatly affects the tactics of management and outcomes in this category of patients. A special feature of the structure of cerebrovascular diseases of young people is the high proportion of hemorrhagic stroke, the causes of which are most often arterio-venous malformations. Meanwhile, there are a number of other causes that can lead to hemorrhage into the brain substance. These include disorders of blood clotting, and various vasculitis, and exposure to toxic substances and drugs, and tumor formations (primary and secondary). All these pathological factors outline the range of diagnostic search in young patients who underwent hemorrhagic stroke. Diagnosis of these pathological conditions with the help of modern visualization techniques is considered to be easy, but this is not always the case. In this article, the authors give their own clinical observation of a hemorrhagic stroke in a young patient, which demonstrates the complexity of the diagnostic search in patients with this pathology.
