Tobacco and Marijuana Co-use Behaviors among Cigarillo Users

Objectives: In this study, we examine the frequency of 'simultaneous use' (smoking tobacco in close temporal proximity to smoking marijuana) and substitution (smoking a cigarillo in place of a marijuana blunt – a cigar wrapper filled with marijuana) and their association with nicotine dependence (ND) among young cigarillo users. Methods: The sample consisted of 686 cigarillo and blunt co-users who responded to a cross-sectional, Web-based survey of 14-28-year-olds recruited via social media. We used bivariate analyses and multiple linear regression to examine the association of simultaneous use and substitution with participant demographics, tobacco product use, marijuana use, and ND. Results: Most users reported simultaneous use (72.4%) and substitution (67.9%). Chasing was the most common form of simultaneous use (63%). Simultaneous users were more likely to use tobacco daily and to smoke cigarettes, and reported a lower age of initiation to marijuana and tobacco than non-simultaneous users. The most common reason for substituting (73%) was the lack of availability of marijuana. Both practices were strongly and independently associated with greater ND, adjusting for covariates. Conclusions: Simultaneous use and substitution are common co-use behaviors among cigarillo users, and may play a role in promoting increased tobacco use and nicotine dependence.

Background. Waterpipe nicotine dependence and its association with depressive symptoms and dual usage among adolescents are currently not examined in the literature. Adolescents are a vulnerable population that is susceptible to depression and initiation of tobacco use. We aim, in this novel study, to assess the association between depressive symptoms and waterpipe nicotine dependence among adolescents in Jordan, evaluate the association between waterpipe smoking status (waterpipe smoker vs. dual user) and waterpipe nicotine dependence, and assess the internal validity of the Waterpipe Nicotine Dependence Scale (WNDS). Method. A cross-sectional study among adolescents of grade 9th to 12th in Jordan was conducted through multistage cluster random sampling. The self-reported Arabic Youth Tobacco Use Composite Measure Questionnaire (YTUCM) was used to collect the surveys that include demographic information, smoking status, and the WNDS to assess waterpipe nicotine dependence and depressive symptoms. Multiple linear regression and the t-test were used to analyze the data. Findings. One thousand three hundred and three surveys were collected, of which 1082 were included in the study (443 males and 639 females). 64.9% of the sample were nontobacco users, while 20.1% were waterpipe- (WTP-) only smokers, 11.4% were dual users, and 3.7% were cigarettes-only users. After adjusting for weights, 66.6% were nonsmokers, 19.2% were WTP-only smokers, 10.2% were dual users, and 3.9% were cigarettes-only smokers. Using multiple linear regression, depressive symptoms were significantly associated with WTP nicotine dependence (β 0.618), upon adjusting for confounders. Furthermore, dual users were associated with higher WTP nicotine dependence (β 12.034) compared to WTP-only smokers after adjusting for confounders. Cronbach’s alpha for the WNDS was 0.955. Conclusions. Our study shows that there is a statistically significant association between depressive symptoms and WTP nicotine dependence and higher dependence among dual users compared to WTP-only smokers. The WNDS can be a useful tool to assess WTP nicotine dependence with high internal consistency. However, a longitudinal study is needed to further understand the association and temporality between the depressive symptoms and WTP nicotine dependence. Additionally, research is needed to shorten the WNDS while maintaining high internal consistency and assess the external validity of the WNDS and the short- and long-term consequences of dual usage.

ObjectivesJapan is currently the biggest market of heated tobacco products (HTPs) in the world. Little is known about nicotine dependence among HTP users. Thus, the objective was to assess the association of type of tobacco use and time-to-first-use, a marker of nicotine dependence.MethodsA cross-sectional analysis of the 2019 data from an internet cohort study was conducted. The analytical sample consisted of 2147 current (≥1 day use in the past 30 days) HTP and/or conventional cigarette users, aged 25+ years. Marginal structural binomial regression was used to estimate nicotine dependence prevalence ratios (PRs) for each category of tobacco use (exclusive daily cigarette, exclusive HTP (≥1 day), dual HTP+daily cigarette, dual HTP+non-daily cigarette), relative to exclusive, non-daily cigarette smoking.ResultsUsing a 5 min cut-off for time-to-first-use, the prevalence of nicotine dependence was higher among dual users of HTP and daily cigarettes (PR=1.38; 95% CI: 1.05 to 1.82) and exclusive, daily cigarette users (PR=1.48; 95% CI: 1.15 to 1.91), relative to exclusive, non-daily cigarette users. However, nicotine dependence among exclusive HTP users, and dual HTP+non-daily cigarette users, did not differ from that of exclusive, non-daily cigarette users. When using 15 and 30 min cut-offs, all types of users, including exclusive HTP, had higher levels of nicotine dependence relative to exclusive, non-daily cigarette users.ConclusionsRegardless of HTP use, daily cigarette users had higher prevalence of nicotine dependence compared with non-daily cigarette users. Exclusive HTP users had similar (or potentially higher) dependence compared with exclusive, non-daily cigarette users. Longitudinal studies are needed to interrogate the public health implications of growing HTP use worldwide.

Background‘Ice’ e-cigarette flavours—marketed as a combination of fruity/sweet and cooling flavours (eg, ‘blueberry ice’ or ‘melon ice’)—recently entered the US market. The prevalence and correlates of ice flavoured e-cigarette use in young adults are unknown.MethodsThis cross-sectional study of a Los Angeles, California, USA, cohort analysed data from the past 30-day e-cigarette (current) users (n=344; M (SD)=21.2 (0.4) years old) who completed web-based surveys from May–August 2020. The exposure variable was self-reported e-cigarette flavour used most often in the past month (menthol/mint, fruit/sweet or ice). Outcomes included self-reported combustible tobacco use, vaping dependence symptoms, frequency of use and device type used.ResultsAmong current e-cigarette users, 48.8% reported using ice flavours most often, 33.7% predominately used fruit/sweet and 17.4% used menthol/mint. Using primarily ice-flavour was associated with reporting more past-30-day vaping days (vs menthol/mint: b=4.4, 95% CI (1 to 7.7); vs fruit/sweet: b=3.6, 95% CI (0.8 to 6.4)) and more episodes per vaping day versus fruit/sweet users (b=2.4, 95% CI (0.5 to 4.3)). Ice-flavour users were less likely than menthol/mint users to use JUUL/cartridge-based rechargeable (OR=0.1, 95% CI (0.03 to 0.45)) and more likely than sweet/fruit users to use disposable non-cartridge (OR=3.9, 95% CI (2.1 to 7.4)) devices than refillable/rechargeable tank/pen or other devices. Ice users had greater odds of past 30-day combustible tobacco use versus menthol/mint users (OR=2.7, 95% CI (1.3 to 5.7)) and vaping dependence symptoms than versus sweet/fruit users (OR=2.6, 95% CI (1.5 to 4.4)).ConclusionYoung adult use of ice flavoured e-cigarettes may be common and positively associated with combustible tobacco use, nicotine vaping frequency and dependence and use of disposable e-cigarette devices. Further study of the prevalence, determinants and health effects of ice flavoured e-cigarette use is warranted.

Background: India is one of the countries where tobacco use is highly prevalent. Tobacco use in the form of smoking is related to occurrence of multiple health related problems in smokers and also general population.Methods: A community based cross sectional study was conducted among 360 adults residing in a rural area of Tiruchirappalli. Cluster sampling was used to select the study participants. A pretested semi-structured questionnaire was used to obtain information on tobacco use, exposure to environmental tobacco smoke (ETS), knowledge on its health effects and protection measures. Data was entered and analyzed using Epi info 7.2. The results were presented in the form of mean and percentage. Chi-square was used to find the association between the parameters and ETS. A p value of less than 0.05 was taken as significant.Results: Mean age of the study group was 38.5±10.8 years. Proportion of males and females was 53.9% and 46.1% respectively. Two hundred and fifty five (70.8%) were non-smokers and 29.2% have ever smoked some form of tobacco product. Thirty seven percent of the non-smokers were exposed to Environmental Tobacco Smoke in the past 15 days and exposure was higher among males (46.7%) than females (31.3%) (χ2 value=6.3; p=0.014). Public place was the most common place of exposure (25.5%) to ETS. Significantly higher proportion of smokers had correct knowledge on ill effects of ETS than smokers (χ2 value=12.3; p= 0.0004).Conclusions: Exposure to environmental tobacco smoke was significantly high in the study population. Either strict enforcement of laws or banning cigarette sale completely would help reducing the exposure to cigarette smoke which is an important risk factor for multiple non communicable diseases.

Information flows can affect people’s apprehension of the pan- demic and related behaviors. The aim is to explore how people access and assess COVID-19 information and whether sociodemographic factors are associated with these processes. Quantitative data from 928 persons from 29 countries who participated in an international web-based survey was ana- lyzed with descriptive and analytic statistics. Television, newspapers and social media were common sources to search for certain types of COVID-19 information. Age, educational level, employment status and sex were as- sociated with differences in terms of reported sources, types, and reliability assessments of retrieved information. Sex and education levels were associ- ated with differences pertaining to main sources and kinds of information searched. Younger respondents reported more frequent uses of social media than older respondents. Older respondents assessed information from tra- ditional media as more reliable information from social media. The current results show trends about where and how information is accessed and assessed during the pandemic. Some associations with sociodemographic factors were found, but the study’s cross-sectional design prevents causal inferences. Further research on consumption patterns of COVID-19 infor- mation and its effects on consumers is motivated, as this may in turn affect people’s attitudes and behavior relating to the pandemic.

Purpose: To investigate variations of psychographic profiles in adult tobacco users to inform message tailoring. Design: A cross-sectional design used data from the Simmons 2015 National Consumer Study. Setting: Data were voluntarily provided by US families through a mail survey on media, products, and services, brands, and attitudes. Subjects: US adult tobacco users (N = 4,609). Measures: Participants answered questions about general opinion/attitudes and provided demographic and tobacco use information. Analysis: A factor analysis was conducted to determine the “best” latent psychographic factor structure based on model fit, factor loadings, and interpretability. A structural equation model was then applied to assess the associations between demographics, tobacco product use, and latent psychographic factors. Results: We identified 9 latent psychographic factors: (1) helplessness, (2) happiness, (3) achievements, (4) religion, (5) interest in art and culture, (6) conscience, (7) conformity, (8) family indulgence, and (9) creativity. Endorsement of these factors varied by demographics and tobacco product use. E.g. low income tobacco users showed stronger endorsement for “helplessness” (Adjusted Standardized Regression Coefficient [ASRC]: 0.42; 95% CI: 0.33, 0.51) and “religion” (ASRC: 0.24; 95% CI: 0.14, 0.33). Less educated tobacco users showed stronger endorsement for “conformity” (ASRC: 0.16; 95% CI: 0.07, 0.24). Young adults had significant positive associations for “achievements” (ASRC: 0.57; 95% CI: 0.48, 0.67). Conclusion: Psychographic profiles of tobacco users vary by demographics and product use. Tailored anti-tobacco media campaigns to specific disparity groups matching their psychographic profiles may improve message effectiveness and reduce tobacco use disparities.

ObjectiveThe goal of this study is to examine the cross-sectional prevalence of use and 3-year longitudinal pathways of cigar use in US youth (12-17 years), young adults (18-24 years), and adults 25+ (25 years or older).DesignData were drawn from the first three waves (2013–2016) of the Population Assessment of Tobacco and Health Study, a nationally representative, longitudinal cohort study of US youth and adults. Respondents with data at all three waves (youth, n=11 046; young adults, n=6478; adults 25+, n=17 188) were included in longitudinal analyses.ResultsWeighted cross-sectional prevalence of past 30-day (P30D) use was stable for adults 25+ (~6%), but decreased in youth (Wave 1 (W1) to Wave 3 (W3)=2.5% to 1.2%) and young adults (W1 to W3=15.7% to 14.0%). Among W1 P30D cigar users, over 50% discontinued cigar use (irrespective of other tobacco use) by Wave 2 (W2) or W3. Across age groups, over 70% of W1 P30D cigar users also indicated P30D use of another tobacco product, predominantly cigar polytobacco use with cigarettes. Discontinuing all tobacco use by W2 or W3 was greater in adult exclusive P30D cigar users compared with polytobacco cigar users.ConclusionsAlthough the majority of P30D cigar users discontinued use by W3, adult polytobacco users of cigars were less likely to discontinue all tobacco use than were exclusive cigar users. Tracking patterns of cigar use will allow further assessment of the population health impact of cigars.

Introduction. A substantial proportion of cancer patients continue to smoke after their diagnosis but few studies have evaluated correlates of nicotine dependence and smoking rate in this population, which could help guide smoking cessation interventions. Aim. This study evaluated correlates of smoking rate and nicotine dependence among 207 cancer patients. Methods. A cross-sectional analysis using multiple linear regression evaluated disease, demographic, affective, and tobacco-seeking correlates of smoking rate and nicotine dependence. Smoking rate was assessed using a timeline follow-back method. The Fagerström Test for Nicotine Dependence measured levels of nicotine dependence. Results. A multiple linear regression predicting nicotine dependence showed an association with smoking to alleviate a sense of addiction from the Reasons for Smoking scale and tobacco-seeking behavior from the concurrent choice task (p<.05), but not with affect measured by the HADS and PANAS (p>.05). Multiple linear regression predicting prequit showed an association with smoking to alleviate addiction (p<.05). ANOVA showed that Caucasian participants reported greater rates of smoking compared to other races. Conclusions. The results suggest that behavioral smoking cessation interventions that focus on helping patients to manage tobacco-seeking behavior, rather than mood management interventions, could help cancer patients quit smoking.

Background: Tobacco product are products made entirely or partly of leaf tobacco as raw material, which are intended to be smoked, sucked, chewed, or snuffed. Tobacco use, among the adolescents is a growing public health problem the world has ever faced. More than 7 million deaths occur from tobacco use every day. Literature has shown that the prevalence of the tobacco uses was high among adolescents. Methodology: A cross-sectional descriptive study design was used.150 respondents were taken as sample size. Probability sampling technique and self- administered questionnaire modified from GYTS, Version 2011 was used as a research instruments. Analysis was done using SPSSversion 16. Findings: The study revealed that maximum 57.3% were male, majority 71.3% were from nuclear family, more than fifty 62.7% respondent's family uses tobacco products and majority75.8% tobacco is used by the respondent’s father. Majority 73.3% uses the hookah, more than half 64% gets tobacco from shop, and almost half 46.7% initiated tobacco product as experiment/curiosity and majority 74.7% uses in café. There is statically association found on frequency of tobacco used by the respondents with sex, residence of the respondent, time spend by the parents and initiated age of the tobacco. Greater strength of association on sex and initiated age of the tobacco use where lower strength of association on residence of the respondents and time spend by family. Conclusion: About 15-20% of each higher secondary school students involve on the use of tobacco products. Gender, living place, time spend by the parents, initiated age of tobacco use, increase café cultural, easy availability and accessibility were the common factors for the use of tobacco among higher secondary students. So buying and selling tobacco products everywhere must be prohibited and adolescents should be restricted to use tobacco products in café or public places. Introduction: Nicotine dependent individuals among the tobacco users are in greater risk of having tobacco related disease and psychiatric co-morbidities. There is little information about the tobacco use and nicotine dependence among psychiatric patients in developing countries including Nepal. This study was done to estimate the prevalence of tobacco use, the level of nicotine dependence among the users and the psychiatric disorder specific prevalence among tobacco users admitted in psychiatry ward.Material And Method: A cross-sectional study was conducted in In-patient setting of Department of Psychiatry, BPKIHS. Ninety cases (size) were enrolled. The Fagerström Test for Nicotine Dependence (FTND) was applied to assess the level of nicotine dependence and the Mini International Neuropsychiatric Interview (M.I.N.I.) for diagnosis of Psychiatric disorders.Results: The prevalence of tobacco use was 62.2%. Tobacco use was more common among males (p value< 0.05).Smokeless form was the commonest form of tobacco. Majority of tobacco users had moderate nicotine dependence and had started the use in adolescence and in their 20s. Psychotic disorders were the most common diagnosis among the admitted patients.Conclusion: Tobacco use is common in patients with psychiatric disorders. Nicotine dependence should be looked into for holistic management. Treatment approaches should be tailored in tobacco users with comorbid psychiatric disorders, as this specific population has high prevalence of tobacco use and higher nicotine dependence. J Psychiatrists’ Association of Nepal Vol. 6, No. 2, 2017, Page: 54-59
