<p class="abstract">Trans-nasal Endoscopic access to the frontal sinus is in vogue now a days with advancements imaging modalities and availability of angled instrumentation. The external osteoplastic flap approach to the frontal sinus is still applicable in failed endoscopic procedures, laterally placed pathologies or eroded anterior or posterior tables. We discuss an intriguing patient with chronic dull ache over the forehead being treated with a mini osteoplastic flap technique. There is no single approach that has been able to fulfil the criteria for the modality of choice in terms of excellent results, no recurrence or residual disease, minimal morbidity, short hospital stays, uneventful long-term postoperative course, and good cosmesis in cases of extensive disease involvement. The main objective of this study was to study the outcome of external frontal sinus osteoplastic flap approach in terms of intraoperative disease clearance for extensive frontal sinus lesions beyond the scope of endoscopic excision.  </p>

Abstract Background: Pulmonary sequestration (PS) is a rare congenital pulmonary pulmonary malformation. In this study, we reported a case of PS treated with aberrant feeding artery mutilation in our center.Case Presentation: A 29 –year-old female patient presented to our center in March 2020 due to repeated hemoptysis for more than 15 years, she received CTA examination which showed an aberrant feeding artery from the thoracic aorta into the left lower lobe of the lung and was therefore diagnosed with left lower lung pulmonary sequestration. Given that she complained with no infectious symptoms, she was treated with thoracoscopic feeding artery mutilation in June 2020 . She and experienced an uneventful postoperative course and was discharged 2 days postoperatively. On the postoperative follow-up of 4 months, thise patient’s physical condition was excellent and free of any symptoms such as hemoptysis or cough.Conclusion: Surgery is still the first choose for the treatment of PS. When pulmonary lesion is small-sized,for PS patients with non-infectious symptoms, aberrant feeding artery mutilation can be attempted as an alternative to intravascular technology for the treatment of PS. Future large studies are required to further verify its long-term efficacy and scope of application.

Introduction: Infantile endodermal oculomotor nerve cyst (EONC) is an extremely rare entity. There are very few pediatric cases reported in the literature, and as expected, oculomotor palsy is the most common presenting symptom. To date however, the risk of recurrence of these lesions following surgical intervention is unclear due to a lack of long-term radiological follow-up. Case Presentation: We present a case of a 13-month-old male patient with an EONC and detail his surgical fenestration and postoperative course. Somewhat surprisingly, re-expansion occurred within 6 months and remained stable 2 years later. Discussion: A surgical approach to fenestration of an EONC in an infant is possible and should be performed by an expert neurosurgeon. Early recurrence is underreported in the current literature, and we encourage longer term radiological surveillance of these lesions after surgery to optimize primary and recurrent management in the future.

Spermatocytic Seminoma (SS) is less common than the Classic variant, as its incidence ranges between 1.3% and 2.3% of all seminomas. Generally SS is diagnosed in men older than 50 years. The Anaplastic variant of Spermatocytic Seminoma is characterized by an earlier onset when compared to SS, but a benign behavior in spite of its histological patterns similar to Classic Seminoma. We reported the first case of bilateral, largest and synchronous Anaplastic Spermatocytic Seminoma, in a patient treated with radical orchifunicolectomy alone and with long-term follow-up. The currently available data show that Anaplastic SS reveals a clinically benign behavior, and no distant metastases have been reported so far. A close surveillance after surgery could be considered a valid option in the management of this rare testicular neoplasm.

ABSTRACT:Background:Tension pneumocephalus, the accumulation of intracranial gas under pressure, is a rare but potentially life-threatening condition which can complicate craniofacial surgery, trauma, or cranial tumor. It presents as an acute or subacute expanding mass lesion.Case Report:We present a case of a 40-year-old male who developed tension pneumocephalus as a consequence of a previously detected but untreated frontal sinus osteoma. Despite prompt decompression and repair of the fistulous connection between the sinus and the intracranial compartment, the patient suffered permanent frontal lobe damage with significant neurocognitive sequelae and seizures.Conclusions:This case illustrates that tension pneumocephalus can be a dangerous entity with potential for early mortality and long-term morbidity. We recommend, therefore, early treatment and close follow up of destructive lesions involving the posterior frontal sinus wall.
