<p><em>Building a construction project is an activity which contains many dangerous elements.  It causes the construction industries have bad record in the terms of occupational safety.  Therefore, occupational safety is an aspect that must be improved any time because it is a very complex problem which covers issues in the terms of humanity, legal aspect, accountability and the image of the organization itself.  This study aims to (1) Determine the accuracy level of the Occupational Safety Management System Application in the Service Building (physical) Construction Project in Tidar Regional Hospital (RSUD) Magelang, (2) Identify and analyze the potential risks of work accident by using Fault Tree Analysis (FTA) method and planning mitigation to reduce the scope of construction in progress. This study is a quantitative analysis where the research is conducted by collecting the primary data through interview and observation, meanwhile the secondary data is in the form of data from the project itself. FTA is used to investigate the potential of workplace accidents by analyzing the direct causes to the basic causes of the accident itself.  The results of the study show that (1) The accuracy level of the application of Occupational Safety Management System which has been applied has an assessment rate of 97.29% and it is included in the level of satisfactory assessment.  (2)  The results of the analysis using FTA obtained several incidents that potentially can cause workplace accidents, incidents such as the workers who do not wear personal protective equipment, workers who act carelessly, and the lack of work experience are the frequent cases in the basic FTA incidents. </em></p>

In mid-2019, there was a construction accident during the underpass construction project in Yogyakarta. The underpass excavation wall in this project was collapsed and 2 vehicles were reported fallen. Although such accident sometimes occurs in some projects, it does not mean that these accidents are caused by the same source. Many efforts to prevent accidents have been done by various methods and one of them is an assessment by observation at construction sites referring to Government Regulation 05 2014 concerning SMK3 (Occupational Safety and Health Management System) in the Public Works Sector Construction. The details about the implementation of construction work are described in PUPR Decree No. 21 in 2019 in term of the Construction Safety Management System (SMKK) Guidelines. This research was conducted to identify factors and indicators of management consultants, supervisors and contractors who have a major impact and contribution to the underpass construction project in Kentungan, Yogyakarta. The percentage level of accuracy in the application of the construction safety management system based on the assessment of the contractor was 85.938% and for the supervisory consultant the percentage level of accuracy was 97.29%. Both percentages are included in the satisfactory category since the assessment ranges are between 85% and 100%. Based on these two assessments, an evaluation was carried out using a Fault Tree Analysis to determine the causality flow of construction accidents.

Occupational safety and health (OSH) issues have become a major concern to many corporations in Malaysia since the enactment of the Occupational Safety and Health Act 1994. While safety management system has been researched, only limited literatures have given attention to warehousing. The development of effective partnerships between customers and vendors to identify opportunities for enhancing safety management system. Employee safety behaviour in the workplace is crucial in approaching towards safety compliance and safety performance indicators. The main objective was to test the safety behaviour of the workers. Data were collected throughout the warehousing processes of a multinational electronics manufacturer in Malaysia. A questionnaire has been filled-up by the warehousing employees. This study proposes the theory of planned behaviour were to explain the linkage between customer - vendor partnership and safety behaviour. Respondents were questioned partnership with its customers to systematically improve safety behaviour. Usually most of the occupational accident or disease took place in the material handling operations and activities with cargo interface. The findings of this study show that customer-vendor partnership has a significant and positive relationship on safety behaviour. This approach to be examined for improved safety ownership and the possibility that enhancements would become an essential part of the vendor's processes, systems, and culture. This customer-vendor partnership approach will develop safety improvements for next heights of success. SPSS was applied for processing the data. It was found that information dissemination and collaboration with the client are important. Employees should participate in the safety program as required by the clients so as to obtain the merits of high performance workforce.

In recent years, with the rapid development of petroleum, chemical and energy industry around the Yangtze River, the variety and quantity transportation of dangerous chemicals has also dramatically increased through Yangtze River. This paper applies fault tree analysis method of specific LPG to obtain problems existing in the Yangtze river water transportation safety management and according to these series of problem, it puts forward some relevant management measures, hoping to minimum the possibility and damage of potential risk accident so as to the risk control measures more scientific, strict and meticulous.

Occupational Health and Safety (K3) is aimed as an effort to create a workplace that is safe, healthy, free from environmental pollution, so that it can reduce and or be free from work accidents and occupational diseases and can have an impact on increasing work efficiency and productivity. The need for implementation and Monitoring of the Occupational Health and Safety Management System at PT. Mujur Lestari must have a good corporate culture and be able to contribute to SMK3. For that PT. Mujur Lestari makes implementation and Monitoring to minimize the risk of work accidents for employees. The sample used in this study were employees of PT. Mujur Lestari, totaling 81 people. The data collection method used a questionnaire while the analytical method used was multivariate analysis. The results showed that there was an influence between knowledge on the Occupational Health and Safety Management System (SMK3) at PT. Mujur Lestari with a tcount greater than ttable, the effect of applying the Occupational Health and Safety Management System (SMK3) with a tcount greater than ttable, the effect of Monitoring on the Occupational Health and Safety Management System (SMK3) with a greater tcount from ttable and there is no relationship between the implementation and Monitoring of the Occupational Safety and Health Management System (SMK3). The conclusion shows that the knowledge variable has a positive and significant effect on the Occupational Safety and Health Management System (SMK3), the application variable has a positive and significant impact on the Occupational Safety and Health Management System (SMK3) and the Monitoring variable has a positive and significant effect on the Occupational Safety and Health Management System.
