Abstract Objective To explore the clinical application of Ilizarov stretcher in the treatment of traumatic knee joint stiffness, and to compare the efficacy of soft tissue release around knee joint with CPM machine exercise. Methods In this study, 36 patients with ankylosis after operation for periarticular fracture from March 2012 to March 2017 were treated with Ilizarov stretcher after soft tissue lysis. The therapeutic effect was excellent. Results According to Judet method, the knee joint function was evaluated in the experimental group: excellent in 22 cases, good in 12 cases, fair in 2 cases, the excellent and good rate was 94.4%; in the control group, excellent in 3 cases, good in 18 cases, fair in 12 cases, the excellent and good rate was 63.6% Conclusions Ilizarov stretcher is an effective method for the treatment of traumatic knee joint stiffness, which has the advantages of short treatment time and good function of knee joint after operation.

AbstractObjectivesObjective of the study is to investigate whether Soft tissue mobilization (STM) can assist with static stretching to improve hamstring flexibly.MethodsThe design of the study was repeated measure design. The study was conducted at the physical therapy laboratory of Jamia Hamdard University, New Delhi. Participants included 78 healthy males with hamstring tightness, randomly assigned to either the control group (static stretching) or the experimental group (STM and static stretching). The experimental group received five sets of four different STM techniques, followed by two sets of 30-s static stretches 3 days per week over the course of 12 weeks. The control group received 5 min of sham ultrasound with an inactive probe prior to static stretching. Active knee extension test (AKE) was the outcome measure.ResultsBoth groups showed significant improvement in AKE compared with the baseline measurements. With ingroup analysis showed a significant difference in AKE across all measured time periods (weeks 4, 8, and 12) with pre-test in both groups (p<0.05). No significant difference in AKE improvement was found between groups (p>0.05).ConclusionThe results of this study show that STM prior to static stretching does not significantly improve hamstring flexibility among healthy individuals. Although this study cannot be generalized, the results may be useful for evidence-based practice in the management of hamstring tightness.

The purpose of the study was to investigate the regenerative processes in the knee joint of rabbits with experimental osteoarthritis after using of allogeneic bone marrow stem cells and a traditional treatment with the non-steroidal anti-inflammatory drug Meloxicam. For the experiment were used 27 male California rabbits (males). Three groups of animals were formed: a control group; the first experimental group treated by the traditional method; the second experimental group treated with allogeneic mesenchymal stem cells (MSC). Animals in the three groups were subjected to osteoarthritis of the knee joint by double injection of 3.44% retinol acetate into the joint cavity at a dose of 1 ml at intervals of 7 days. Tissue from the affected site was sampled for histological examination at 7, 14 and 28 days. The histological sections were stained with haematoxylin-eosin and examined under a microscope. It has been established that intra-articular administration of 3.5 × 106 cells of allogeneic MSCs in experimental osteoarthritis contributes to the restoration of the superficial layer of cartilage, as evidenced by the formation of columns of chondrocytes in the middle layer of articular cartilage and the appearance of isogenic groups of cartilage cells with basophilic cytoplasm in the matrix, uniform articular surface. The use of the traditional method of treating rabbits using the drug Meloxicam is accompanied by incomplete chondrogenesis: part of the chondrocytes is localized in typical chambers, in some cases chondrocyte chambers did not differentiate; articular cartilage had unequal thickness, cell placement was uneven.

Abstract Background: The argument presupposes that intra-operative soft tissue balance is associated with patient-reported outcome measures after total knee arthroplasty (TKA). Our aim was to assess the association between the extension-to-flexion gap and patients’ reported perception of knee joint function.Methods: This was a retrospective study of 60 cases of primary cruciate-retaining TKAs performed for the treatment of medial-compartment knee osteoarthritis, at a mean follow-up of 2.6 (range, 2.0–5.2) years. Knee perception was evaluated by asking patients whether they considered their knee joint as “natural” (grade I) or “artificial”, with or without restrictions (grades II–V). The following factors were compared between the two groups: age, sex, and the extension-to-flexion gap difference. A multiple logistic regression analysis was used to identify predictive factors of an artificial knee joint perception. A receiver operating characteristic curve analysis was used to identify cut-off values of predictive factors. Result: Compared to a natural knee joint perception (26 knees, 43%), an artificial perception (34 knees, 57%) was associated with a smaller gap difference at the distraction force of 20 lbf (p < .001), 30 lbf (p = .022), and 40 lbf (p = .038), a lower EuroQol 5-Dimension score (p = .029), and self-reported joint health (Knee injury and Osteoarthritis Outcome Score-Joint Replacement [KOOS-JR], p = .032). A gap difference <1.0 mm was a predictive of an artificial perception (odds ratio, 1.63; 95% confidence interval, 1.33–4.54; p < .001). A cut-off gap difference of 0.99 mm at 20 lbf predicted an artificial perception with a sensitivity of 81.5% and a specificity of 87.9%. Post-operative satisfaction (p < .001), KOOS-JR (p < .001), patient’s joint perception (p = .006), pain (p = .015), and EuroQol 5-Dimension (p = .032) differed between the two groups when the gap difference threshold was set to 1.0 mm.Conclusion: Careful balancing of soft tissue during TKA to achieve an extension-to-flexion gap ≥ 1.0 mm can improve patients’ perception of knee joint function and quality of life.

Background. The number of patients with soft tissue wounds does not tend to decrease, which is largely due to the growth of patients with injuries, ulcerative defects, skin tumors, comorbid pathology, including diabetes mellitus, antibiotic-resistant strains and other causes. New methods of local treatment of wounds based on the application of the latest achievements of science are constantly being introduced into the work of surgical departments. High efficiency is demonstrated by the use of sorbents and oxygen, which enhance the reparative processes in wounds.The aim of the study was to study in experimental conditions the effectiveness of the method of surgical treatment of soft tissue wounds, based on the use of jet oxygen sorption treatment (SCS).Materials and methods. The study was performed on 150 white Wistar rats in 5 groups of animals. In the 1st control group treatment was not carried out. In the 2nd and 3rd control groups, the wound surface was treated with a jet of air and oxygen, respectively; in the 4th control group, sorbent was applied to the wound surface. In the 1st experimental group conducted scso wound surface. The study of the effectiveness of SCS was carried out using objective, planimetric, histological and histochemical research methods. Results. The use of sorbent (4th control group) and the method of jet oxygen sorption treatment (1st main group) led to the most pronounced positive changes-acceleration of relief of the studied symptoms by 1.1-1.3 times and 1.2 1.5 times, respectively, compared with the data obtained in the 1st control group. Treatment of wounds with oxygen jet (3rd control group) had practically no advantages in comparison with treatment with air jet (2nd control group).Conclusion. Indirectly, the highest activity of reparative processes during the observation period in the 4th control and 1st main groups was confirmed by the average optical density of RNA and SH-groups, and their desire to normalize to 10 days could indicate the completion of processes of stratification in the wound area. In total, the average area of wounds was minimal in the 1st experimental group, where this figure was by the 3rd and 10th days-74.0% and 99.5% compared to the baseline data.

Abstract Backgroud: The objective of this study is to introduce posteromedial corner release with the knee in the figure-of-four position versus the conventional position for varus knee arthroplasty. Methods : From March 2015 to September 2019, 123 patients (139 knees) with varus knee were randomly and blindly allocated to experimental group (60 patients; 68 knees) and control group (57 patients; 65 knees). Patients in experimental group underwent posteromedial corner release with the knee in the figure-of-four position; and patients in control group with the knee in the conventional position. Time for soft tissue balancing was defined as the time from the start of spacer test to the end of balance test. Length of release was defined as the distance from the osteotomy surface of the tibial plateau to the farthest structures released. The rating system of Hospital for Special Surgery (HSS) knee score was used to evaluate the clinical results. Differences were considered statistically significant at p < 0.05. Results: The mean time for soft tissue balancing was 8.4±3.3 minutes and 8.4±3.3 minutes in experimental and control group, respectively ( p <0.05). The mean length of releasing posteromedial corner structures was 35.5±13.4 mm and 27.3±9.7 mm in experimental and control group, respectively ( p <0.05). HSS scores 5 years after surgery were 95.1±16.9 and 94.8±17.2 respectively ( p >0.05). Conclusion: During varus knee arthroplasty, the posteromedial corner can be released more extensively and thoroughly when the knee is placed in the figure-of-four position.

The objective of this article is to explore the effect of applying Zhifei Kangfu Decoction on the treatment effect of patients during the recovery period of children with pneumonia. The method of this research is to take patients who were treated in our hospital from December 2019 to December 2020 as an example to carry out the research work. The researchers selected all patients in the recovery period of pediatric pneumonia, and the number was selected as 100 cases, who were divided into two groups, and the treatment methods used are conventional western medicine and Zhifei Kangfu Decoction treatment, who were named the control group and the experimental group, and the clinical treatment effects of the two groups of patients are compared and analyzed. The effective rate and adverse reaction rate of children in the experimental group were 96.00% and 4.00%, respectively. The effective rate and adverse reaction rate of children in the control group were 82.00% and 30.00%, respectively. Asthma, cough relieving, and treatment time were shorter than those of the control group, and the difference in the data was P<0.05, which was statistically significant. The experimental group had better results. The treatment of children in the recovery period of pneumonia and the application of Zhifei Kangfu Decoction can promote the improvement of clinical efficacy, reduce the incidence of adverse reactions in children, and have a positive significance in promoting the recovery of children.

Abstract Aim To explore the application value of modified closed biopsy technique in puncture biopsy of rabbit VX2 transplanted bone tumor model.Methods VX2 tumor was transplanted into the bilateral tibia of 30 rabbits through the tibial plateau to make the model of VX2 transplanted bone tumor. Seven days after modeling, the proximal tibia biopsy was performed under the guidance of X-ray, and the biopsy specimen was examined pathologically. The left leg was biopsied with modified closed biopsy technique (experimental group) and the right leg was biopsied with hollow needle (control group). After 14 days of modeling, all rabbits were killed after X-ray examination around the puncture hole, and the soft tissue around the puncture hole was taken for pathological examination, and the expression levels of PCNA and CD34 in the tissue extract were detected by enzyme-linked immunosorbent assay (ELISA).Results By the end of the experiment, a total of 3 rabbits died, and finally 27 rabbits were included in the study. Tumor cells were detected in all the intramedullary specimens obtained by puncture biopsy. On the 14th day after modeling, X-ray showed that the occurrence rate of periosteal reaction and extraosseous high density shadow around the puncture hole was 14.81% (4/27) in the experimental group and 40.74% (11/27) in the control group. the difference was statistically significant (P<0.05). The pathological results of soft tissue around the puncture hole showed that the tumor cell metastasis rate was 29.63% (8/27) in the experimental group and 100% (27/27) in the control group, and the difference was statistically significant (P<0.05).The expression levels of PCNA and CD34 in the experimental group were lower than those in the control group (P < 0.05). Conclusion Both the modified closed biopsy technique and needle aspiration biopsy can provide sufficient biopsy tissue for the diagnosis of VX2 transplanted bone tumor in rabbits. At the same time, the improved closed biopsy technique has a certain application value in preventing local metastasis of tumor cells along the puncture channel.
