Incidence of benign thyroid adenoma is common, but adenoma causing extensive skin ulceration is a very unusual presentation. Here we are reporting a case of a 70-year-old female from Norshingdi district who presented to us with an ulcerated, neck swelling over the thyroid region for one year, which moves with deglutition and bleeds on touch with foul smelling discharge for one month and pain for two months. Biopsy from the ulcer revealed inflammatory lesion while subsequent histopathology following thyroidectomy under local anesthesia gave features of follicular adenoma. Postoperative period was satisfactory and follow up was excellent. Castleman disease is a rare clinicopathological disorder associated with lymphoproliferation. We report the case of a 54 year old gentleman initially presenting with dysphagia followed by bilateral cervical lymphadenopathy, and later diagnosed as multicentric Castleman disease. To the best of our knowledge no case of Castleman disease presenting as dysphagia along with neck swelling has been previously described in the literature. This case report highlights a very unusual presentation of this rare disease. It also provides insight into the fact that because of the lack of any specific presenting symptoms and distinguishing radiographic features, an accurate histopathologic diagnosis and careful staging become crucial to planning treatment for this disease. Tuberculosis has been reported in many parts of the human body but thyroid gland involvement is extremely rare and its true incidence is still unknown. Tuberculosis of the thyroid is a rare entity even in countries where tuberculosis is prevalent as an endemic problem. Tuberculosis of thyroid gland can present as cold abscess, multinodular goitre, acute abscess or generalized goitre. Clinically, these patients can be euthyroid, hypothyroid or hyperthyroid. Here we report an unusual presentation of recurrent neck swelling in an 8 year old girl turning out to be tuberculosis of thyroid gland.

A 19-year-old farmer was referred by his general practitioner as an emergency to our otolaryngology department complaining of marked breathlessness of a few hours duration. He gave a three-day history of painful swelling and hair loss in the beard area of the right side of the neck. His upper airway was compromised unless extension of the neck was maintained. Larynx and pharynx were normal. The acute symptoms settled with intravenous antibiotics and hydrocortisone. Culture of skin scrapings revealed a growth of Tricophyton verrucosum. The neck swelling subsided after a course of oral griseofulvin followed by terbinafine. Difficulty in breathing due to fungal infection of the neck has not been previously reported in the English literature.

Lateral neck swelling is a common presentation in ORL practice. The complexity of the neck structures warrants a thorough examination and investigation to narrow down the differential diagnosis. Neoplasm need to be ruled out especially if the neck swelling present in an adult. We report a case of a lateral neck mass being treated as lymphadenopathy before the patient defaulted follow up. The patient represented after 3 years and investigations performed. FNAC revealed sialadenosis but radiologically and intra-operatively consistently showed the mass located outside parotid gland. The final diagnosis of Whartin's confirmed after the HPE evaluation. The diagnosis dilemma is discussed.
