Beckground/Aim. Neuroendrocine lung tumors are histologically heterogenous group of cancers with different clinical progression. In non-small cell lung cancer (NSCLC) neuroendocrine differentiation exists in 10-30% of patients. The aim of this study was to determine the frequency and influence of neuroendocrine differentiation on survival of treated patients with advanced non-small cell lung cancer (NSCLC). Methods. A clinical trial included 158 patients (74% males and 26% females), with the diagnosis of NSCLC, determined by histological verification. The patients were treated by combined chemo - and X-ray therapy in stage III (without pleural effusion) or chemotherapy only in stage III (with pleural effusion) and stage IV. Chemotherapy was conducted until progression of the disease, but no more than six cycles. When the progression had been noted in stage III (without pleural effusion), the treatment was continued with X-ray therapy. Neuron specific enolase, chromogranin A, as well as synapthophysin expression in tissue examples were determined by immunohistochemical analysis with monoclonal mouse anti-human-bodies. Survival was assessed within a year and two years follow-up examination. Results. A total of 53 patients (34%) had NSCLC with neuroendocrine differentiation, confirmed rather in large cell lung cancer and lung adenocarcinoma (66.7% and 40%, respectively). Neuron specific enolase, chromogranin A and synapthophysin expression was noted in 45 (28.5%), 34 (21.5%) and 33 (20.1%) patients, respectively. The one year and two years follow-up survival periods were confirmed in 39% and 17% of patients respectively. The median survival time in the patients with the neuroendocrine expression as compared to those without the expression was 15.6 vs 10.8 months; one year survival time with the expression compared to those without the expression achieved in 62% vs 27% of the patients, (p < 0.001); a two - year survival time noted in 30% of the patients (p = 0.000). One year follow-up survival time was longer in the patients with neuron specific enolase and chromogranin A expression lung cancer (p < 0.001). Synapthophysin expression was not statisticaly significant for survival (p > 0.05). Conclusion. The results of this study suggest that almost the third of the advanced NSCLC has neuroendocrine differentiation. The median survival time of the treated patients is longer when the tumor is associated with neuron specific enolase and chromogranin A expression. .

Objectives: To identify the relevance of neuroendocrine differentiation in non-small cell lung cancer and its correlation with different pathological features. Materials and Methods: A total number of 30 cases of per cutaneous CT guided biopsies of primary non-small lung cancer were collected in the pathology department of Misr University for Science and Technology Giza, Egypt and private practice in the time period from January 2018 till December 2020. Immunohistochemical study for neuroendocrine differentiation was performed using mono clonal antibodies against synaptophysin, chromogranin A and CD56. For all selected cases, clinical and pathological data such as age, gender, histologic types, grade and clinical stage were collected, tabulated and statistically analyzed with the results of neuroendocrine markers expression. Cases with incomplete pathological data and cases with histologic picture of neuroendocrine differentiation were excluded. Results: A total number of 30 cases of primary non-small lung cancer were enrolled in this study. The median age of patients was 61.5 years. There were 21 (70%) males and 9 (30%) females. Regarding neuroendocrine markers, positivity for either marker was identified in 23.3% of non-small cell lung cancer. Chromogranin A was positively expressed in 9 (30%) of cases, synaptophysin was positively expressed in 7 (23.3%) of cases and CD56 was positively expressed in 5 (16.7%) of cases. Only 2 cases (6.7%) showed co-expression of two markers. It was found that there was a high significant relation between chromogranin A expression and clinical stage. Chromogranin A expression was significantly higher in stage III than stage I and II (P<0.001). There was no statistical significant difference between synaptophysin, chromogranin A and CD56 expressions and the rest of the studied pathologic data. Conclusion: A considerable number of non-small cell lung cancer has neuroendocrine differentiation for at least one neuroendocrine marker despite absence of morphologic features. Much less number of cases showed expression of two markers. A reasonable panel of neuroendocrine markers is recommended to detect this differentiation which may have a clinical impact and optimize an alternative therapeutic option.

Introduction: Corticosteroids are used to treat immune-related adverse events (irAEs) associated with nivolumab. However, patients with non-small-cell lung cancer who are administered corticosteroids before the initiation of nivolumab treatment are commonly excluded from clinical trials. The appropriate timing for corticosteroid administration in relation to nivolumab treatment, effects of corticosteroids on the efficacy of nivolumab, and resulting adverse events are not clearly understood. In this study, the effects of differences in the timing of corticosteroid administration on nivolumab efficacy and the resulting adverse events were examined. Methods: A retrospective study was conducted with 109 patients who were treated with nivolumab at Sapporo Minami-Sanjo Hospital between December 2015 and March 2018. Results: Of the 109 patients treated with nivolumab, 12 patients were administered corticosteroids before the first cycle of nivolumab (pre-CS), and 33 patients were administered corticosteroids after the first cycle of nivolumab (post-CS). These 2 groups were compared with the control group comprising 64 patients who were not administered corticosteroids (non-CS). The objective response rate in the post-CS group was significantly higher than that in the non-CS group, and the disease control rate in the pre-CS group was significantly lower than that in the non-CS group. The overall survival time and progression-free survival time in the pre-CS group were significantly shorter than those observed in the non-CS group; however, these did not differ from those in the post-CS group. Conclusions: These results suggest that corticosteroids administered to patients with non-small-cell lung cancer after initiation of nivolumab treatment did not affect the disease prognosis. Thus, corticosteroids can be administered immediately for rapid treatment of irAEs.
