Hiring Practices of Exercise Physiologist in Cardiac Rehabilitation Programs among Mid-Atlantic States in the United States

Purpose: The purpose of this survey was to identify factors that may be influencing the appreciation of exercise physiology as a discipline as demonstrated by hiring practices in regional clinical settings. Methods: A telephone survey was administered to 33 cardiac rehabilitation programs in 5 states in the Mid-Atlantic region of the United States (Kentucky, North Carolina, Tennessee, Virginia, and West Virginia). Results: The distribution of nurses and exercise physiologists (EPs) employed by the 33 facilities varied by state, but overall there were 86 nurses and 55 EPs working among the surveyed facilities. Of the 33 surveyed facilities, 12 (36%) reported a preference for hiring nurses over EPs; only 4 (12%) reported a preference for hiring EPs over nurses. The remaining facilities (n = 17; 52%) reported no preference (n = 12; 36%) or that the decision depends on a variety of factors (n = 5; 15%). Several common themes were identified from respondents. These included that, compared to nurses, EPs have greater expertise in exercise prescription and better understanding of safe exercise progression for patients. However, nurses were believed to possess greater assessment and clinical skill with an emphasis on emergency response preparedness and greater general patient education skills. Conclusion: Academic programs that prepare students for careers as EPs employed in clinical settings may benefit from additional coursework and internship site selection that focus on clinical assessment skills, emergency preparedness, and patient education to reinforce their work in an exercise science curriculum.

ABSTRACT The current state of the profession of clinical exercise physiology in the United States is that of evolution. Individuals now identified as clinical exercise physiologists (CEPs) first emerged in health care in the nascent cardiac rehabilitation programs in the late 1960s and have remained strongly identified in that role ever since. However, the profession has had difficulty expanding into other similar areas, largely due to the lack of standardized academic preparation. This contributes to uncertainty and confusion among other health care providers regarding appropriate roles and responsibilities for a CEP. Future directions for the profession of clinical exercise physiology include requiring certification candidates to graduate from accredited programs so the academic preparation becomes standardized. The American College of Sports Medicine (ACSM) is currently investigating modifying the eligibility criteria for the ACSM-CEP certification to address this requirement. The Clinical Exercise Physiology Association is currently assembling a writing team to draft a CEP scope of practice document for the support and endorsement of other professional organizations The last item is the strengthening or creation of liaisons with other professions to develop a collaborative care model that takes full advantage of the skillset CEPs bring to chronic disease management.

Introduction: Cardiac rehabilitation and patient education are the two parallel components in the management of coronary heart disease. Although, it is recommended strongly to join an outpatient cardiac rehabilitation program, the enrollment rates are always low. The role of structured patient education in increasing the enrollment in an outpatient cardiac rehabilitation program and its impact on the disability profile of conservatively treated medically stable postmyocardial infarction individuals, after joining and completing the rehabilitation program has not been studied. Aim: To study the impact of structured patient education and outpatient cardiac rehabilitation program on the disability profile of conservatively treated medically stable postmyocardial infarction individuals. Materials and Methods: The study is a mixed method design and will be conducted in two steps. Step 1: It is a qualitative study in which a structured patient education program will be developed based on the framework given by the working Group of Exercise Rehabilitation and Sports (GERS). Step 2: Implementation of the developed program and an outpatient cardiac rehabilitation program in an open-label non randomised clinical trial. Conclusion: The findings of this study will evaluate the need for the development of structured patient education programs for coronary artery disease patients, which may increase the enrollment rates in outpatient cardiac rehabilitation programs and thereby, reducing their disability.

Introduction: Cardiac rehabilitation (CR) is an effective but highly underutilized therapy for heart disease. Efforts are underway to increase CR referral and enrollment, but little is known about the potential capacity for growth in CR utilization in the United States. To address this concern, we estimated the current national capacity of CR programs across the United States and assessed obstacles to potential growth. Methods: We surveyed all CR Program Directors listed in the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) database in November of 2012. Respondents reported current enrollment levels, current program capacity, program capacity given reasonable expansion, and obstacles to growth. Results: Of 812 Program Directors in the AACVPR database, 290 (36%) completed the full survey. Respondents represented somewhat larger programs compared to non-respondents (4.0 vs. 4.6 full time employees, p = 0.01), but were otherwise similar. Current enrollment levels demonstrated a median [IQ range] of 140 [75,232] patients per year and a current estimated capacity of 192 [100,300] patient per year. Respondents estimated that programs could expand services by 25% to 240 [141,380] patients annually, assuming a significant increase in patient demand and a feasible increase in resources. We estimate that if programs filled to their current capacity, national CR utilization rates would increase from 34.7% to 46.4% (95% CI, 43.9 to 48.9) of eligible US patients each year. Given feasible program expansion, utilization could increase to a maximum of 58.4% (95% CI, 55.2 to 61.6) of qualifying patients. Capacity projections based upon absolute patient numbers and national statistics revealed concordant results. The most commonly cited obstacles to increasing patient participation are shown in the figure. As seen, the majority (88%) are controllable system-related factors unrelated to patient behavior. Conclusions: Even with substantial expansion of all existing CR programs, there is currently insufficient capacity in current CR programs to meet national service needs. Solutions to this problem will likely include the creation of new CR programs, and new policies that improve reimbursement for CR and also embrace new models of CR delivery.
