Repetitive task-oriented exercises are accepted in traditional gait rehabilitation and have given rise to driven gait orthoses, but both methods suffer from limited rehabilitation time for the patient. The presented device proposes a control strategy and implementation unique for a mobile rehabilitation exoskeleton as well as results from initial subject testing. This anthropomorphically designed device has knee and hip joints that are actuated in the sagittal plane using hydraulic actuators. The presented control strategy allows the user or therapist to directly specify the level of rehabilitation assistance desired between complete machine control and a zero impedance joint. The device was experimentally tested on three chronic stroke patients with noticeable gait improvements based on the metric of joint flexion. Other results of step time and step length are presented that do not demonstrate as clear improvements but these are believed to be a function of the limited patient testing time.

Abstract The syndrome of unilateral neglect, typified by a lateralized attention bias and neglect of contralateral space, is an important cause of morbidity and disability after a stroke. In this review, we discuss the challenges that face researchers attempting to elucidate the mechanisms and effectiveness of rehabilitation treatments. The neglect syndrome is a heterogeneous disorder, and it is not clear which of its symptoms cause ongoing disability. We review current methods of neglect assessment and propose logical approaches to selecting treatments, while acknowledging that further study is still needed before some of these approaches can be translated into routine clinical use. We conclude with systems-level suggestions for hypothesis development that would hopefully form a sound theoretical basis for future approaches to the assessment and treatment of neglect.

Eine Übersicht der bestehenden Literatur weist auf Unsicherheiten bezüglich der spezifischen Rolle der Pflegenden in der Rehabilitation von Hirnschlagpatientinnen und -patienten hin. Es existieren zwei unterschiedliche Begrifflichkeiten für die Rolle der Pflegenden, keine davon bezieht sich auf spezifische Rehabilitationsziele oder Patientenergebnisse. Ein anfänglicher theoretischer Beitrag der Rolle der Pflege in der Genesung vom Hirnschlag wird als Struktur unterbreitet, um die therapeutischen Aspekte der Pflege im Koordinieren, Erhalten und Üben zu vereinen. Bestehende Literatur untermauert diesen Beitrag. Weitere Forschung ist jedoch notwendig, um den spezifischen Inhalt und Fokus der Pflege in der Genesung bei Hirnschlag zu entwickeln.
