Esthetics is an increasing concern for patients, particularly in anterior teeth. The current approaches to manage the esthetic demands include options such as prosthodontic rehabilitation, orthodontic treatment, and direct restorations. Direct restorative procedures for anterior teeth have become a reliable alternative, since current composite materials can mimic teeth’s esthetics and function perfectly. This article describes two clinical cases of anterior esthetic rehabilitations where the direct reshaping of the four maxillary incisors was performed using the buccolingual layering technique with the help of a silicone index. In Case 1, a diastema closure was also performed, whereas in Case 2, the replacement of old restorations was also carried out. It was possible to achieve excellent final esthetic results in both cases, recovering the smile symmetry, and both patients were satisfied. The literature supports the use of direct restorations in the anterior region with very good long-term prognosis. It is a minimally invasive approach that is reversible, repairable, cost-effective, and when combined with layering techniques and adequate restorative materials, it is possible to achieve good esthetic and functional outcomes. However, regular maintenance appointments are mandatory, and all cases should be carefully planned to ensure predictable and long-lasting results.

ABSTRACT Smile is an integral part of dentistry. Patients come with the prime aim of achieving esthetics. The dentist may preserve or alter the relationships of the visible components of the oral region to give the desired esthetic result. But, it is our responsibility to attain this result through a scientifically based and thoroughly planned treatment. The appearance of gingival tissue surrounding the teeth plays an important role in the esthetics of anterior region. Abnormities in symmetry and contour can significantly affect the harmonious appearance of natural or prosthetic dentition. There are a variety of cases of teeth wear, wherein interdisciplinary intervention is essential. Also the factor causing severe wear of natural teeth must be identified and eliminated, or reduced before attempting restorative treatment to improve long-term prognosis of restorative treatment. This article deals with a case of anterior teeth wear, stressing upon diagnosis, treatment planning and progression of treatment with regards to perio-prostho esthetics. Objective: The objective of the present study was to present, through two clinical cases, the direct stratification technique and the sequence of finishing and polishing of composite resin restorations on fractured anterior teeth. Case reports: In the first case, an 11-year-old female patient fractured the upper central incisors. In the second, a 6-year-old female patient fractured the left upper central incisor. A detailed description of the layering technique, materials, and instruments available was presented, not only for the restoration itself but also for finishing and polishing. In both cases, the tooth fragment was lost, so it was decided to use the technique of direct restoration with composite resin. Discussion: The composite resin restores the tooth structure with excellent optical results, long-term predictability is obtained, in addition to being a conservative way to restore the aesthetics and function of fractured anterior teeth. Conclusion: The technique performed provided a satisfactory aesthetic and functional result that, consequently, improved the patient’s self-esteem.

Accidental injury to anterior region with fractured and mobile teeth can be an awful experience to young female patient. Treating such patient requires knowledge judgment and skill of the operator. In the present case report multiple horizontal root fracture in cervical third region with mobile undetached crown was successfully stabilized by a very simple procedure using hedstrome endodontic file as an endodontic implant. In a two year follow-up period the tooth was completely asymptomatic clinically as well as radiographically. Immobilization of fractured root by endodontic instrument with MTA considered to be a simple ,reliable and cost effective method to save a tooth that would have been difficult to restore with good long term prognosis. Background: Long-term survival and functional outcomes should influence admission decisions to intensive care, especially for patients with advanced disease. Aim: To determine whether physicians’ predictions of long-term prognosis influenced admission decisions for patients with and without advanced disease. Design: A prospective study was conducted. Physicians estimated patient survival with intensive care and with care on the ward, and the probability of 4 long-term outcomes: leaving hospital alive, survival at 6 months, recovery of functional status, and recovery of cognitive status. Patient mortality at 28 days was recorded. We built multivariate logistic regression models using admission to the intensive care unit (ICU) as the dependent variable. Setting/participants: ICU consultations for medical inpatients at a Swiss tertiary care hospital were included. Results: Of 201 evaluated patients, 105 (52.2%) had an advanced disease and 140 (69.7%) were admitted to the ICU. The probability of admission was strongly associated with the expected short-term survival benefit for patients with or without advanced disease. In contrast, the predicted likelihood that the patient would leave the hospital alive, would be alive 6 months later, would recover functional status, and would recover initial cognitive capacity was not associated with the decision to admit a patient to the ICU. Even for patients with advanced disease, none of these estimated outcomes influenced the admission decision. Conclusions: ICU admissions of patients with advanced disease were determined by short-term survival benefit, and not by long-term prognosis. Advance care planning and developing decision-aid tools for triage could help limit potentially inappropriate admissions to intensive care.

Four case reports are presented, followed by a discussion of the acute, potentially life-threatening manifestations of the cholesterol embolism syndromes. Every major organ system except the lungs may be directly affected by cholesterol emboli; devastating consequences encompass cerebral, myocardial, spinal cord, intestinal, renal, and other visceral organ infarction, as well as peripheral and perineal gangrene. Additional complications include severe hypertension, gastrointestinal bleeding, and hemodynamic instability. Anticoagulants and thrombolytic therapy may exacerbate atheromatous embolism and are relatively contraindicated. Aggressive supportive therapy may improve chances of survival, but long-term prognosis is poor. Prevention remains the most important aspect in this devastating disorder.

E-governance uses Internet and communication technologies to automate governance in innovative ways, so that it becomes more efficient, more cost-effective, and empowers the human race even more. E-governance exercises are being attempted for more than a decade now, but have so far achieved only mixed success. The long-term prognosis for e-governance, however, remains extremely positive. The emergence of Web-services technologies, the continually proliferating computer networks, and the irreversible migration towards digital information strongly confirm the view that e-governance is here to stay. The eventual success of any e-governance project is intimately linked to the methodology used, and to that complex mesh between men, machines, and mindsets. We explain the “what,” “why,” and “how” of e-governance. We also talk of e-governance concerns, and discuss a few illustrative case studies.

Accidental tooth avulsion is a grievous injury and common among the children. Management of avulsed tooth within alveolar socket by reimplantation becomes a challenge for the clinician due to extraoral time and media of transportation. Although the long-term prognosis of reimplantation is poor, the time during which the tooth remain within the arch will guide the development of alveolar bone completely. Moreover, reimplantation will maintain anatomical, functional and esthetic rehabilitation of the patient. In this case report, we present a case of accidental avulsion where teeth were gently rinsed of any debris and placed in normal saline during the examination and preparation of the reimplantation sockets. The teeth were then reimplanted, functionally splinted followed by endodontical treatment. After 12 months follow-up the periodontal space was healed perfectly without any resorption or ankylosis.J Enam Med Col 2017; 7(3): 165-169
