The socket-shield technique and early implant placement for tooth rehabilitation: A case report

For a long time, different approaches to minimize the dimensional ridge alteration following tooth extraction have been described even though none of them was capable to avoid alveolar bone loss. The Socket-Shield Technique (SST) seems to be an alternative for alveolar bone preservation in dental extraction planning. This technique conventionally uses a flapless approach and the buccal root portion retention to prevent bone loss. This case report describes the SST with an early implant placement in a patient who presented the nonrestorable tooth # 15 due to the extensive cavity. After six months of the implant placement a porcelain-fused-to-metal crown was fabricated and screwed into the implant with a satisfactory esthetic and functional result. The SST is a non-invasive and effective approach to tooth extraction and rehabilitation, especially in the aesthetic zone. Approximately two-year follow-up shows a satisfactory aesthetic maintenance.

Immediate implants are placed in the site of surgical extraction of the tooth to be replaced. The percentage success of such procedures varies among authors from 94-100%. Immediate implant placement is most commonly indicated when tooth extraction is done with pathologies not amenable to treatment. The advantages include reduced post-extraction alveolar bone resorption, shortened treatment time, and the avoidance of a second surgical intervention with regard to delayed implantation. This report describes a case of immediate implant placed in a maxillary central incisor followed by evaluation of soft and hard tissue changes occurring during post-operative period with a follow-up at five years.

The aim of this retrospective study was to clinically evaluate the five-year outcomes of implants placed following a combined approach to the sinus, consisting of sequential drills and osteotomes. Medical records of patients with implants placed in combination with crestal sinus lift using sequential drills and osteotomes, with a residual alveolar bone crest between 4 to 8 mm, and a follow-up of at least five years after final loading, were evaluated. Outcomes were implant and prosthetic survival and success rates, any complication, and marginal bone loss. Data from 96 patients (53 women and 43 men; mean age 54.7 years; range 23–79 years) were collected. A total of 105 single implants were analyzed. After five years of function, two implants were lost and two prostheses failed. No major biological or prosthetic complications occurred. At the five-year examination, the marginal bone loss was 1.24 ± 0.28 mm. Within the limitations of this retrospective study it can be concluded that implants placed following a combined approach to the sinus consisting of sequential drills and osteotomes seem to be a viable option for the treatment of posterior atrophic edentulous maxilla.

Purposes: After tooth extraction alveolar socket bone undergoes a remodelling process. The horizontal and the vertical bone losses complicate dental rehabilitation with implants. Socket-Shield technique maintain the periodontal attachment including cementum, periodontal ligament and bundle bone. In this report this technique was indicated to replace the lost teeth. Case report: A socket shield technique was used for immediate implant placement on a non-restorable teeth by failure of crowns in teeth 12, 11 and 21. Through a osteotomy and manipulation of soft tissues with provisionals it was possible to correct the gingival contours and zeniths. Conclusions: Atraumatic extraction and socket preservation technique decreases the alveolar bone resorption by maintaining the hard and the soft tissue volume. Provisionalization allows management of the soft tissues around the implants. This treatment offers a predictable aesthetic and functional result .

Objectives: The purpose of this study was to evaluate vertical alveolar bone loss 3 months after tooth extraction when a technique of ridge preservation was applied using a particulate demineralized freeze-dried bone allograft 300 - 500 µm associated with platelet concentrates (platelet-rich-fibrin) in the form of gel and membranes. Material and Methods: A retrospective radiological clinical study was conducted on 56 patients for whom 95 extractions had been performed immediately followed by alveolar filling. Among the patients, 17 were smokers and 16 were provided with an immediate removable temporary prosthesis after extractions. Vertical bone loss was measured radiologically by panoramic X-ray before extractions and by a computed tomography scan 3 months after, at the level of mid-buccal bone wall, by two independent observers. For statistical analysis, Student’s t-test was performed to compare the mean bone loss between mono- and pluri-radicular teeth and to compare the mean bone loss between tobacco users versus non users and finally to compare the mean bone loss between individuals that had provisional removable prosthesis and those that had not. Results: Three months after tooth extraction, the mean of vertical loss of the mid-buccal bone wall was 0.72 (SD 0.71) mm (5.53% SD 5.19). No significant difference between bone loss at mono-radicular and pluri-radicular teeth (P = 0.982) was observed. There was no significant correlation between tobacco habits and bone loss (P = 0.2), nor between provisional removable prosthesis and bone loss (P = 0.786). Conclusion: These results indicate a good potential for the technique using Demineralized Freeze-Dried Bone Allograft 300 - 500 µm and platelet concentrates in alveolar bone preservation.

Single tooth implant restorations in the aesthetic area are a demanding challenge. If a complete osseointegration is mandatory, the final result has to result in a higher standard of biomimetic and soft tissue health among natural teeth. This outcome is traditionally pursued by cementing crowns over individualized abutments. However, in recent years, the need for controlling peri-implant health and the preference towards a retrievable solution has led to an increase in screw-retained crowns, which is not always applicable when the implant axis is not ideal. In the aesthetic area, the use of a novel technical solution represented by the angled screw channel (ASC) of the abutment has been proposed in order to match the advantages of the screwed solution with the aesthetic demands. The aim of this study was to compare ASC crowns to cemented crowns (CC) in single implant restorations using the white esthetic score (WES) and pink esthetic score (PES) at the crown delivery and at a follow-up of a minimum of 2 years. Peri-implant health and marginal bone loss (MBL) were also evaluated. The mean follow-up was 44.3 months, with a mean MBL of 0.22 mm in the ASC group and 0.29 mm in the CC group. The total WES/PES score was 16.6 for ASC, compared with 17.3 for CC at baseline, and 16.2 and 17.1, respectively, at follow-up. Both of the groups reached a high WES/PES, and this was maintained over time, without signs of peri-implant diseases or bone loss, regardless of the choice of connection. In conclusion, ASC can be adopted in cases where the implant axis is not ideal, with aesthetic and functional results that are comparable to implants restored by cemented crowns.

Traumatic oral injuries present their own unique restorative challenges to the clinician due to their variable clinical presentation. In such cases, achieving optimum aesthetics and function in the maxillary aesthetic zone is challenging. This case report describes the rehabilitation of a patient with immediate implant placement after suffering trauma to the oral cavity resulting in a complete loss (avulsion) of a permanent central incisor tooth. In order to preserve the existing soft and hard tissue and to achieve predictable and aesthetically pleasing results, we decided to place an immediate implant as opposed to replantation of the avulsed tooth. Flapless implant surgery was planned and a dental implant (Bio horizons Implant Systems, Inc.) was placed following non-submerged protocol. Introduction. Myxoma is the most common primary benign heart tumor. The most frequent location is the left atrium, the chamber of the heart that receives oxygen- rich blood from the lungs. Myxomas usually develop in women, typically between the ages of 40 and 60. Symptoms may occur at any time, but most often they are asymptomatic or oligosymptomatic for a long period of time. Symptoms usually go along with body position, and are related to compression of the heart cavities, embolization and the appearance of general symptoms. The diagnosis of benign tumors of the heart is based on anamnesis, clinical features and findings of the tumor masses by use of non-invasive and invasive imaging methods. Extensive surgical resection of the myxoma is curative with minimal mortality. Long term clinical and echocardiographic follow-up is mandatory. Case report. We reported a case of a 62-year-old male, presented with 15 days of intermittent shortness of breath, dizziness and feeling of heart palpitations and subsequently diagnosed with right atrial myxoma based on transthoracic echocardiography . The patient was emergently operated in our hospital. Long-term followup did not reveal recurrence. Conclusion. Our case was an atypical localisation of right atrial myxoma. Whether the intracardiac mass is benign or malignant, early surgery is obligatory in order to prevent complications.
