Introduction: Hemiplegia due to cerebral stroke is associated with various dermatological co-morbidities, arising out of motor, sensory and autonomic dysfunctions. Aim: To find out the different dermatological conditions prevalent in patients of hemiplegia secondary to cerebral stroke. Materials and Methods: An observational cross-sectional study was conducted in the Department of Neurology in collaboration with the Department of Dermatology at Hi-Tech Medical College and Hospital, Bhubaneswar, Odisha from July 2017 to June 2020. Patients with hemiplegia of more than one month duration were included. The skin findings present before the neurological insult and patients with systemic conditions like atopic dermatitis, diabetes mellitus and thyroid disorders causing skin changes were excluded. Relevant tests for diagnostic confirmation of the skin lesions were done wherever needed. The software Statistical Package for the Social Sciences (SPSS) version 16.0 was used for the statistical analysis. Results: Study included 411 patients of cerebral stroke. A total of 382 (92.9%) patients were found to have dermatological manifestation. The male:female ratio in the study population was 2.6:1. A total of 549 and 128 skin conditions were diagnosed in the paralysed and non paralysed limbs, respectively. The common dermatological conditions diagnosed in the affected limbs were, xerosis {371 (97.12%)}, tinea corporis/cruris {61 (15.97%)}, onychomycosis {31 (8.12%)}, tinea pedis {28 (7.33%)} and nail dystrophy abnormalities {27 (7.07%)} patients. Conclusion: Knowledge of various dermatological conditions associated with hemiplegia helps in early intervention and better management during rehabilitation phase of patients with hemiplegia.

Background: Acute undifferentiated fever illness (AUFI) is a common cause of morbidity and mortality in developing countries, owing to its non-specific features. The aim of the study was to delineate the causes and clinical parameters associated with AUFI.Methods: A cross-sectional study was done among 156 patients of AUFI, admitted in the Department of Medicine, Indira Gandhi Medical College, Shimla, Himachal Pradesh, India, from November 2018 to October 2019.Results: The mean age of the study participants was 37.37±14.2 years. The study participants admitted for acute undifferentiated febrile illness had fever with mean duration of 8.38±3.7 days before hospitalization. The most common aetiology of AUFI was found to be enteric fever (44%), followed by scrub typhus (35%). The majority of the cases presented from the month of September to December.Conclusions: The aetiology and clinical spectrum of AUFI is wide and variable. To outline a proper algorithm to contain it, meticulous analysis of the hospital data at each level is necessary.

BACKGROUND Cutaneous lesions of external ear are quite common in clinical practice. Patients often present with various cutaneous lesions of the external ear to various disciplines. They are often underdiagnosed as many lesions, though commonly diagnosed by our dermatologist colleague are missed by other clinicians. These lesions can be classified in various ways, according to the aetiology, and according to the anatomic sites. In the present study, we tried to assess the prevalence of different skin lesions affecting external ear presenting to an ENT and skin outpatient department of a tertiary care medical college in Eastern part of India. METHODS This is a descriptive observational study, cross sectional in design. 240 patients attended the outpatient department with cutaneous lesions affecting external ear from January 2019 to February 2020. The patients presenting in both ENT and skin outpatient department were chosen randomly. Cases were diagnosed based on history and clinical examination and appropriate investigation as required. All cases were categorised into 5 types (infective, inflammatory, autoimmune, neoplasm & naevi, and reactive & reparative) according to the aetiology. In each category, prevalence of various skin lesions was recorded. RESULTS A total number of 240 cases were detected during the study period. Among them, infective 43.33 %, inflammatory 30.83 %, autoimmune 9.17 %, neoplasm & naevi 4.17 % and reactive & reparative were 12.82 %. Taenia was the most common lesions encountered (20.51 %) followed by keloid (12.5 %). Impetigo and seborrheic dermatitis shared same number of cases (9.4 %). Neoplastic lesions were the least common. CONCLUSIONS With some awareness and basic knowledge, many of the cutaneous lesions of external ear can be diagnosed and treated whenever patients report to a clinician, thus saving valuable time, effort and money of the patient. Background: This study was carried out to assess the knowledge, attitude, and practice of contraceptive methods among women attending a tertiary care hospital in Western Rajasthan.Methods: This cross-sectional study was conducted in the department of obstetrics and gynecology, PBM and the associated group of hospitals, attached to Sardar Patel Medical College, Bikaner, Rajasthan, India. Total 500 married women between age group 15-49 yrs were included. These all women interviewed using a structured questionnaire after approval of the hospital’s ethical committee.Results: Total 500 women interviewed for their knowledge, attitude, and practices about contraception. Out of which, 402 (80.4%) women had some knowledge of at least one contraception method at the time of the study.  The most common sources of information about contraception were husband and family member. Amongst the various contraceptives most commonly known was condom (78.4%). Only 265 (53%) women were practicing contraception at present. Most commonly used contraceptive was condom (40.8%). The most common reason for the non-practice of contraception was need of more child or male child in 34.8% families followed by opposition from in-laws (12.4%).Conclusions: Literacy was positively associated with family planning related knowledge and practices. Preconception knowledge and practices about contraception are poor in our study population. Many of the women were getting MTP pills over the counter without consulting to health care persons and then later presents with complications.

Introduction: Ischaemic Heart Diseases (IHD) are increasing at an alarming rate globally. Pharmacotherapy, the mainstay of management of IHD, has Dual Antiplatelet Therapy (DAPT) at its crux. DAPT, comprising of clopidogrel and aspirin, is used often for 12 months in medically managed Acute Coronary Syndrome (ACS). Clopidogrel being a prodrug, needs to be activated after ingestion, mainly by Cytochrome family of enzymes, for the desired pharmacological effects. Abnormal enzymes that convert clopidogrel to its active form can reduce the effectiveness of the drug. Therefore, Single Nucleotide Polymorphisms (SNPs) of the enzymes involved, would predispose the patient to therapy failure and recurrence of the illness. This study aims at identifying the presence of a genetic variant associated with clopidogrel responsiveness in patients with ACS. Aim: To estimate the frequency of CYP2C19*2 allele in patients with ACS presenting to Government Medical College, Thrissur, Kerala, India. Materials and Methods: Present study was a cross-sectional study done among 60 patients who presented to Government Medical College, Thrissur with ACS, between June 2018 to June 2019. Having obtained informed consent from the participants blood samples were collected in Ethylene diamine tetra-acetic Acid (EDTA) tubes. Samples were used to look for presence of CYP2C19*2 allele by Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP). Results: Out of 60 patients 38 were males and 22 were females. Most common age group was 61-65 years. The frequency of CYP2C19*2 allele was 41% in the study population and the variant genotype (AA/AG) was present in 62%. Conclusion: The variant genotype was found to be present in a high frequency in the study population implying the high risk for therapy failure in the study population. This asserts the need to probe the matter further, as the gene is also implicated in the metabolism of several other drugs. The introduction of pharmacogenomic testing prior to initiation of therapy may be therefore recommended for better treatment outcome. However, to validate the same, further studies with larger sample size may be necessary.

Abstract: Transfusion by blood and blood products forms an important part of treatment. But this is not free from transfusion reactions which may be in the form of fever, rashes, shivering, anaphylaxis or even death. Hemovigilance is a programme of reporting such events.Materials and Methods: The present study was conducted in a tertiary care hospital of Jammu, (J& K). Permission was taken from Institutional Ethics Committee, Govt. Medical College, Jammu. A pretested questionnaire was prepared and circulated among practitioners. They were given time of 15 minutes to fill that questionnaire. After 15 minutes, the filled questionnaire were collected and were analysed. The data was presented in tabulated form.Results: Total 50 practitioners were included in the study. All the practitioners had knowledge about transfusion reactions. Only 10% of the practitioners had knowledge that transfusion reactions can be prevented, 40% of the practitioners had knowledge about hemovigilance programme and had an idea that transfusion reactions can be reported. But only 10% of the practitioners knew where to report and who can report, 6% knew how to report. Only 48% of the practitioners thought that transfusion reactions should be reported, 70% of the practitioners thought that transfusion reactions can be dangerous. 40% of practitioners told that seminars/CMEs should be planned. 80% of practitioners had encountered transfusion reactions, but only 2% had documented the same. 10% of the practitioners had attended seminars, CMEs. Many reasons were quoted for not reporting transfusion reactions.Conclusion:  Overall, most of the practitioners have a positive attitude towards transfusion reaction reporting but knowledge regarding the haemovigilance concept is poor and the majority of them never reported such reaction. Hence, our study demands increased awareness and continued training to strengthen the haemovigilance system.

Introduction: The exact position of the Parotid papilla and commissure of the lip plays a crucial role in determining the maxillary occlusal plane which directly facilitates rehabilitating edentulous patients with a removable complete denture. Since the beginning, many authors have conducted different studies regarding its position but multiple variations in its location have been noted. Also, this type of study has not been conducted in the Nepalese population which drags the urge to determine the position of these soft-tissue landmarks in context to the Nepalese population. Objective: To determine the position of parotid papilla and commissure of lip in relation to the maxillary occlusal plane. Materials and Methods: A descriptive cross-sectional study was conducted in the 45 dentate subjects visiting Nobel medical college from November 2019 to April 2020. A vestibular impression was made, stone index fabricated and vernier caliper was used to locate position of parotid papilla & commissure of lip. All the data were collected and statistically analyzed. Results: The mean measurement of distances between maxillary occlusal plane and parotid papilla was 3.69 ± 1.19 mm and between maxillary occlusal plane and commissure of lip was 0.41 ± 0.83 mm respectively. Conclusion: This study showed that the position of the parotid papilla was superior to the position of the maxillary occlusal plane with a mean distance of 3.69mm. The commissure of lip was found to be at the level of maxillary occlusion plane in highest number of cases followed by a superior position with least at the inferior position with respect to the maxillary occlusal plane.

Introduction: COVID-19 is a beta coronavirus that is transmitted by physical interaction or close contact. This Coronavirus Pandemic has also created stress and anxiety among pregnant women all over the world. The disease was first identified in Wuhan city, China, in late December 2019 and was declared pandemic by the World Health Organization on 11th March 2020. Concern and stress in pregnancy are associated with pre-eclampsia, intrauterine growth restriction, preterm labour, depression etc. Pregnancy is an immune-compromised state and poses a high risk to this risk. This study aims to identify anxiety about the coronavirus infection among pregnant women visiting a tertiary care centre in Kathmandu, Nepal, during this COVID–19 pandemic. Methods: This is a descriptive cross-sectional study conducted at Kathmandu Medical College and Teaching Hospital from 15th July 2020 to 30th July 2020 after taking the ethical clearance from the Institutional Review Committee of Kathmandu Medical College. Convenient sampling method was used. All the data were entered in Statistical Package for the Social Sciences data 20.0 and analyzed. Data was presented in frequencies, charts and percentage. Results: Among the total 273 cases, only 2 (0.73%) case had a score between 25-30 corresponding to moderate to severe anxiety, 21 (7.69%) had a score between 18-24, which corresponds to mild to moderate anxiety and 250 (91.57%) had score 0-17 which corresponds mild status. Conclusions: Most of the participants in the study reported a mild status of anxiety. Very few participants reported moderate to severe anxiety.

Background: Adolescence is a developmental period of rapid physical, psychological, sociocultural, and cognitive changes characterized by efforts to confront and surmount challenges and to establish a sense of identity and autonomy. Objective was to study the sexual behavior practiced by the adolescent and its source of inspiration.Methods: An observational cross-sectional study was carried out in Department of Community Medicine, Santosh Medical College, Ghaziabad and Urban area of district Ghaziabad. 750 Adolescents aged 10 to 19 years studying in the selected schools of urban Ghaziabad were included for the study. The period of study was from November 2011 to October 2012.Results: 26.8% of the students were sexually active. Boys were sexually more active than girls (34.7% vs. 18.9%). Condom use was present in 51.2% of the sexually active students. Nearly half (52.3%) of sexually active males had sex under the influence of pornography, almost one-third (32.3%) had sex merely because of peer pressure and 15.4% did it for curiosity and fun. Among girls who experienced sex 70.4% succumbed to peer pressure and 23.9% under the influence of pornography. In the present study more boys (77.9%) than girls (54.1%) had knowledge about reproductive health issues and this difference between two sexes were found to be statistically significant.Conclusions: The present health status of the study population has the potential possibility of leading to increased health problems for this vulnerable population, in future.

Background: Inadequate understanding by patients of medical prescription given to them adversely affects treatment compliance; which can result in treatment failure and increase the incidence of adverse effects. The study site is a tertiary care hospital in a metropolitan city and the patients attending here are particularly vulnerable in this regard both due to the heavy patient load in the outpatient department and their low socioeconomic status.Methods: This was a cross sectional study conducted in the pharmacy of K.E.M. Hospital, Mumbai over a period of 1 month. Study population included general outpatient department patients of the hospital. Patients were interviewed using a pre validated questionnaire and their knowledge of the prescription analysed under five key aspects including dose, timing, duration, side effects and purpose of each drug. Data was entered using Microsoft Excel and analyzed using SPSS 22 software.Results: The mean age of the population was 37.6, with a standard deviation of 12.4. A good overall knowledge of the prescription was present only in 12.4% patients. In 52% of patients, the level of knowledge ranged from ‘no knowledge to little knowledge’. Age and literacy had statistically significant associations with overall knowledge of prescription.Conclusions: There is a significant gap in prescription knowledge in the study population. Age and literacy of the patient has an effect on prescription knowledge.
