ABSTRACT Introduction: Brief introduction: Ankle tendon and ligament sports injuries are common in football players. Objective: To continue to improve special strength training related to the characteristics of football after rehabilitation of injured ankle tendons and ligaments. Methods: Two master football sportsmen were rehabilitated by multi-point equal-length, short-arc and long-arc equal-speed training combined with balance ability exercises. Results: There were two long muscle L be maintain muscle tone plantar flexors force four times of 96 n/m, n/m 121, 140 n/m, 145 n/m than back flexors force of 63 n/m, 52 n/m, 60 n/m, 74 n/m tall. Plantar flexor fatigue was 57%, 30%, 29%, 12%, 28%, 18%, 20%, 21%. Conclusions: With the passing of time, the relative peak moment value of the right ankle plantar flexor muscle group of the two patients kept rising, the dorsiflexor muscle was basically flat, and the work fatigue index decreased step by step, indicating that the right ankle muscle strength level was significantly improved, the anti-fatigue ability was improved, and the rehabilitation treatment had a good effect. We compared the effect of programmed (PFI) and thirst-driven (TDFI) fluid intake on prolonged cycling performance and exercise associated muscle cramps (EAMC). Eight male endurance athletes (26 ± 6 years) completed two trials consisting of 5 h of cycling at 61% V˙O2peak followed by a 20 km time-trial (TT) in a randomized crossover sequence at 30 °C, 35% relative humidity. EAMC was assessed after the TT with maximal voluntary isometric contractions of the shortened right plantar flexors. Water intake was either programmed to limit body mass loss to 1% (PFI) or consumed based on perceived thirst (TDFI). Body mass loss reached 1.5 ± 1.0% for PFI and 2.5 ± 0.9% for TDFI (p = 0.10). Power output during the 20 km TT was higher (p < 0.05) for PFI (278 ± 41 W) than TDFI (263 ± 39 W), but the total performance time, including the breaks to urinate, was similar (p = 0.48) between conditions. The prevalence of EAMC of the plantar flexors was similar between the drinking conditions. Cyclists competing in the heat for over 5 h may benefit from PFI aiming to limit body mass loss to <2% when a high intensity effort is required in the later phase of the race and when time lost for urination is not a consideration.

Our aim was to clarify the effect of aging on the coherence of electromyograms of plantar flexor pairs during bipedal stance and to clarify the relationship between coherence and center-of-mass acceleration (COMacc). The subjects were 16 adults and 18 older adults. Intra- and intermuscular coherence and phase analyses were used to analyze the muscle pairs of bilateral and unilateral plantar flexor muscle groups. The relationship between coherence value and anterior–posterior COMacc of the plantar flexor muscle pairs was also examined to determine whether the connectivity of the lower limb muscle pairs is functionally important. The older adults showed higher coherence in the frequency range of 0–4 Hz for muscle pairs than the younger adults. In phase analysis, the older adults showed a phase difference between bilateral heteronymous muscle pairs in the frequency range of 0–6 Hz, which was one of the characteristics not seen in the younger adults. Correlation analysis showed that all the muscle pairs were moderately correlated with COMacc in the older adults. Not only does aging affects the organization of the bilateral and unilateral postural muscle activity of the plantar flexors during bipedal stance, but such organization may also be related to the increased COMacc characteristics of older adults.

Background: Lower limb spasticity after stroke is common that can affect the balance, increase the risk of falling, and reduces the quality of life.Objective: First, evaluate the effects of spasticity severity of ankle plantar flexors on balance of patients after stroke. Second, to determine the relationship between the spasticity severity with ankle proprioception, passive ankle dorsiflexion range of motion (ROM), and balance confidence.Methods: Twenty-eight patients with stroke based on the Modified Modified Ashworth Scale (MMAS) were divided into two groups: High Spasticity Group (HSG) (MMAS &gt; 2) (n = 14) or a Low Spasticity Group (LSG) (MMAS ≤ 2) (n = 14). The MMAS scores, Activities-Specific Balance Confidence Questionnaire, postural sway of both affected and non-affected limbs under the eyes open and eyes closed conditions, timed up and go (TUG) test, passive ankle dorsiflexion ROM, and ankle joint proprioception were measured.Results: The ankle joint proprioception was significantly better in the LSG compared to the HSG (p = 0.01). No significant differences were found between the LSG and HSG on all other outcome measures. There were no significant relationships between the spasticity severity and passive ankle dorsiflexion ROM, and balance confidence.Conclusion: The severity of ankle plantar flexor spasticity had no effects on balance of patients with stroke. However, the ankle joint proprioception was better in patients with low spasticity. Our findings suggest that the balance is affected regardless of the severity of the ankle plantar flexor spasticity in this group of participants with stroke.

Vibratory (Tvib) and sustained (Tsust) torque responses to concurrent Achilles tendon vibration and neuromuscular electrical stimulation applied over the muscle belly (vib+stim) are used as indicators of motoneuron facilitation and, theoretically, persistent inward current strength. However, neuromuscular electrical stimulation (NMES) applied to the nerve trunk may potentiate motoneuronal excitability more than muscle belly NMES, yet it remains unclear whether NMES applied over the nerve evokes robust Tvib and Tsust responses when used during the vib+stim protocol. This study tested whether a nerve-targeted vib+stim protocol elicits Tvib and Tsust responses in the ankle plantar flexors with acceptable intra- and inter-session reliability. Fifteen men performed the vib+stim protocol with NMES applied over the tibial nerve three times across two sessions; twice in a single session (5-min apart) to test intrasession reliability and then again after 48 h to test intersession reliability. Intraclass correlation coefficients (ICC3,1), within-participant coefficients of variation (CV) and pairwise comparisons were used to verify relative and absolute reliability as well as systematic bias. Thirteen men presented Tvib and Tsust responses (response rate of 87%). Intrasession Tvib and Tsust ICCs were >0.73 but inter-session ICCs were <0.5. Although no systematic bias was detected (p>0.05), both intra- and inter-session CVs were large (>10%) for Tvib and Tsust. The Vib+stim protocol with NMES applied over the nerve evoked Tvib and Tsust in almost all participants, but presented a large intra- and inter-session variability. The method does not appear to be effective for assessing motoneuron facilitation in the plantar flexors.

Abstract Background Power bursts of hips and ankle plantar flexors are prerequisites to walking propulsion. However, these power bursts are reduced during gait for persons with cerebral palsy (CP) and mainly in the ankle plantar flexors. Hence, task specific training, such as ballistic strength training, is suggested to increase muscle power in walking but not investigated in adults with CP. Therefore, the aim was to investigate if adults with CP could perform and benefit from ballistic strength training to improve walking, evaluated through physical measures and self-reported measures and interviews. Methods In this mixed methods feasibility study, eight ambulatory adults (aged 24–56) with spastic CP conducted ballistic strength training on a glideboard targeting the ankle plantarflexors two times a week for eight weeks. The feasibility of the training was assessed through objectives described by Orsmond and Cohn. Before and after the intervention, physical measures (6-Minute Walk Test and the eight-item High-level Mobility Assessment Tool) and self-reported measures (Patient Global Impression of Change, Numeric Pain Rating Scale, Fatigue Impact and Severity Self-Assessment, and Walk-12) were collected. After the intervention, semi-structured interviews explored experiences of this training. Results The participants experienced training the ankle plantar flexor as relevant but reported it took about four weeks to coordinate the exercises successfully. Although we observed no changes in the physical performance measures, most participants reported improvements; some felt steadier when standing, walking, and hopping. Conclusion This study demonstrated that ballistic strength training was feasible and suitable in adults with CP. However, guidance and a long (4 weeks) familiarization time were reported necessary to master the exercises. Most participants reported self-experienced improvements, although no physical performance measures improved. Thus, prolonged intervention may be required for perceived physical improvements to emerge. Also, other outcome measures sensitive to power output remains to be investigated.

The different effects of intermittent and continuous stretching on the mechanical properties of the musculotendinous complex have been unclear. This study aimed to compare the effects of intermittent and continuous stretching for the same duration on the range of motion (ROM), passive resistive torque (PRT), and musculotendinous stiffness (MTS) of ankle plantar flexors. Eighteen healthy young men participated in the study. Intermittent (four sets × 30 s) and continuous stretching (one set × 120 s) were performed in random orders on two separate days. Both stretching protocols were conducted using a dynamometer with a constant torque applied. ROM and PRT were determined using a dynamometer, and MTS was calculated using the torque–angle relationship measured before and after stretching. Two-way repeated measures analysis of variance was performed for all parameters. Both intermittent and continuous stretching significantly increased ROM and decreased PRT and MTS (p < 0.05). Intermittent stretching led to greater changes in ROM and PRT than continuous stretching. However, the reduction in MTS did not differ between the two conditions. These results suggest that intermittent stretching is more effective in increasing ROM and changing the mechanical properties of the musculotendinous complex.

BACKGROUND: Assessment of the plantar flexion (PF) isokinetic performance has been greatly diverse and based on personal preferences rather than standardized guidelines. OBJECTIVE: To examine the performance of the plantar flexors under different settings including knee joint angle and subject position. METHODS: Thirteen women and 20 men took part in this study. The isokinetic protocol (60∘/s) was set to ankle movement between 10∘ dorsiflexion to 30∘ PF. Participants performed three repetitions of concentric PF in randomly-ordered knee angles; 15∘, 45∘ and 90∘, and in seated and supine positions. Surface electromyography (EMG) data were collected from the Soleus (SOL) and Gastrocnemius. RESULTS: Knee angle impacted the PF moment (P⩽ 0.001–0.026) and work (P⩽ 0.05) measures in both genders. The moment and work measures were significantly less in the 90∘ than those in the 45∘ and 15∘ positions. The 45∘ position had the highest values, particularly in sitting in the male participants. Only the GL EMG data was significantly impacted (P= 0.017) by the subject position. However, the difference was trivial (1.6%). The SOL muscle showed a consistent pattern of increased activity when the knee was in flexion. CONCLUSION: The 45∘ position seems to be optimal for obtaining the highest isokinetic PF scores.
