A 46- year-old woman presented a uterine adenosarcoma originating in the lower uterine segment. The diagnosis was made in an endometrial biopsy and confirmed in the pathological examination of the complete surgical specimen, both identifying heterologous malignant elements. In addition, complementary immunohistochemical studies were performed. We reviewed the literature, illustrating the clinical and morphological characteristics and the differential diagnoses to be evaluated.

Post-partum hemorrhage is still a headache to all obstetricians around the whole world. Every obstetrician exerts his own full effort to control bleeding which can occur post-partum by applying all maneuvers to preserve the fertility and the uterus for the patient. We demonstrate a new technique (Mostafa Maged) technique to control and prevent post-partum hemorrhage. It is so simple maneuver and easy to be applied within short period of time. Satisfactory hemostasis can be assessed after application. The aim of this technique is to see the Success in Controlling and prevention of the bleeding from placenta previa cases from lower uterine segment. The average duration of this new technique is (5-7) minutes. The results have shown that the hysterectomy done to one patient with new technique (1/13) (7.6%) cases because of the uncontrollable bleeding, blood was creeping down from the flipped sutured lower uterine segment. All of patients are introduced to the operating room as first-time cesarean section. One patient (7.6%) out of thirteen patients were tachycardiac post-operatively due to the more loss of blood as the new (Mostafa Maged) technique took a long time in these two patients (8 minutes). The tests of success were expected if hemostasis is done by the bimanual compression at first place.

NE is a 36yr old petty trader who has had four previous caesarean sections. She went for her routine abdominal ultrasonography at 36weeks of her fifth pregnancy and an incidental posterior bladder polypoid mass was found with a normal singleton fetus. Patient declined cystoscopy but accepted exploration of bladder at same time with a caesarean section.  At Surgery, a polyp was found arising from the lower uterine segment traversing the thin posterior bladder wall into the bladder. The polyp was carefully dissected out and separated from bladder. Bladder and uterus were repaired, and patient had uneventful recovery.

Abstract Introduction Postpartum hemorrhage is a serious complication of childbirth and is still the leading cause of maternal death worldwide. Lower uterine segment hemorrhage during cesarean section is an important cause of postpartum hemorrhage. Our objective is to expore the efficacy and safety of King’s combined uterine suture for hemostasis during cesarean section. Methods We examined 48 cases: 16 cases of pernicious placenta previa (including one case of twins), 11 cases of central placenta previa (including one case of twins), 18 cases of uterine scarring (including two cases of twins), as well as one case of twin pregnancy, two cases of breech presentation, and one case of pulmonary hypertension. The “King’s combined uterine suture” method for hemostasis was used in patients with lower uterine segment hemorrhage during cesarean section. Results The results showed that all patients had successful hemostasis during surgery, and there were no cases of hysterectomy. Conclusion We have concluded that King’s combined uterine suture is a fast and safe hemostasis method for cesarean section that can effectively reduce blood loss and restore the normal shape of the lower uterine segment. Furthermore, this suture method can reduce postpartum hemorrhage and hysterectomy rate, as well as improve maternal prognosis.
