Background: Smartphone self-monitoring through ecological momentary assessment (EMA) provides insights into the daily lives of people in psychiatric treatment and has the potential to improve their care. Currently, no clinical tools are available that help clients and clinicians with creating personalized EMA diaries and interpreting the gathered data. Integration of EMA in treatment is therefore difficult. Objective: To develop a web-based application for personalized EMA in routine psychiatric care, in close collaboration with all stakeholders (i.e., clients, clinicians, researchers, and software developers). Methods: We engaged 52 clients with mood, anxiety, and/or psychotic disorders and 45 clinicians (psychiatrists, psychologists, and psychiatric nurses) in interviews, focus groups, and usability sessions. We used human-centered design principles to determine important requirements for the web-app and designed high-fidelity prototypes that were continuously reevaluated and adapted. Results: The iterative development process resulted in PETRA (PErsonalized Treatment by Real-time Assessment), which is a scientifically grounded web-app for the integration of personalized EMA in clinical care. PETRA includes a decision aid to support clients and clinicians with constructing personalized EMA diaries, an EMA diary item repository, a text-message-based diary delivery system, and a feedback module for visualizing the gathered EMA data. PETRA is integrated in electronic health record (EHR) systems to ensure ease-of-use and sustainability, and adheres to privacy regulations. Conclusions: PETRA was built to fulfill the needs of clients and clinicians for a user-friendly and personalized EMA tool embedded in routine psychiatric care. PETRA is unique in this co-development process, its extensive yet user-friendly personalization options, its integration in EHR systems, its transdiagnostic focus, and its strong scientific foundation in the design of EMA diaries and feedback. The clinical effectiveness of integrating personalized diaries via PETRA into care awaits further research. As such, PETRA paves the way for a systematic investigation into the utility of personalized EMA for routine mental health care.

The transition from adolescence to adulthood is a critical period for the development of healthy behaviors. Yet, it is often characterized by unhealthy food choices. Considering the current pandemic scenario, it is also essential to assess the effects of coronavirus disease-19 (COVID-19) on lifestyles and diet, especially among young people. However, the assessment of dietary habits and their determinants is a complex issue that requires innovative approaches and tools, such as those based on the ecological momentary assessment (EMA). Here, we describe the first phases of the “HEALTHY-UNICT” project, which aimed to develop and validate a web-app for the EMA of dietary data among students from the University of Catania, Italy. The pilot study included 138 students (mean age 24 years, SD = 4.2; 75.4% women), who used the web-app for a week before filling out a food frequency questionnaire with validation purposes. Dietary data obtained through the two tools showed moderate correlations, with the lowest value for butter and margarine and the highest for pizza (Spearman’s correlation coefficients of 0.202 and 0.699, respectively). According to the cross-classification analysis, the percentage of students classified into the same quartile ranged from 36.9% for vegetable oil to 58.1% for pizza. In line with these findings, the weighted-kappa values ranged from 0.15 for vegetable oil to 0.67 for pizza, and most food categories showed values above 0.4. This web-app showed good usability among students, assessed through a 19-item usability scale. Moreover, the web-app also had the potential to evaluate the effect of the COVID-19 pandemic on students’ behaviors and emotions, showing a moderate impact on sedentary activities, level of stress, and depression. These findings, although interesting, might be confirmed by the next phases of the HEALTHY-UNICT project, which aims to characterize lifestyles, dietary habits, and their relationship with anthropometric measures and emotions in a larger sample of students.

BACKGROUND Quarantines and nationwide lockdowns dictated for containing the spread of the COVID-19 pandemic may lead to distress and increase the frequency of anxiety and depression symptoms among the general population. During the national lockdown of the first wave of the COVID-19 outbreak in Spain, we developed and launched a Web App to promote emotional self-care in the general population and facilitate contact with healthcare professionals. OBJECTIVE To describe the Web App and analyse its utilization pattern throughout two successive waves of the COVID-19 outbreak in Spain. METHODS The Web App targeted all individuals aged 18 years or more and was designed by adapting the contents of a mobile App for adjuvant treatment of post-traumatic stress disorder (i.e., the PTSD Coach App) to the general population and the pandemic/lockdown scenario. We retrospectively assessed the utilization pattern of the Web App using data systematically retrieved from Google Analytics. Data were grouped into three time periods, defined using a join point analysis of COVID-19 incidence in our area: first wave, between-wave period, and second wave. RESULTS The resulting Web App, named gesioemocional.cat, maintains the navigation structure of the PTSD Coach App, with three main modules: tools for emotional self-care, a self-assessment test, and professional resources for on-demand contact. The self-assessment test combines the Patient Health Questionnaire-2 (PHQ-2) and the 7-item Generalized Anxiety Disorder Scale (GAD-7) and offers professional contact in the advent of a high level of depression and anxiety; contact is prioritized according to a screening questionnaire administered at the time of obtaining individual consent to be contacted. The tools for emotional self-care can be accessed either on-demand or symptom-driven. The utilization analysis showed a high number of weekly accesses during the first wave. In this period, press releases regarding critical events of the pandemic progression and government decisions on containment measures were followed by a utilization peak, irrespective of the sense (i.e., positive or negative) of the information. Positive information pieces (e.g., relaxation of containment measures due to a reduction of COVID-19 cases) resulted in a sharp increase in utilization immediately after information release, followed by a successive decline in utilization. The second wave was characterized by a lower and less responsive utilization of the Web App. CONCLUSIONS mHealth tools may help the general population coping with stressful conditions associated with the pandemic scenario. Future studies shall investigate the effectiveness of these tools among the general population―including individuals without diagnosed mental illnesses―and strategies to reach as many people as possible. CLINICALTRIAL Not applicable

Large reference datasets of protein-coding variation in human populations have allowed us to determine which genes and genic sub-regions are intolerant to germline genetic variation. There is also a growing number of genes implicated in severe Mendelian diseases that overlap with genes implicated in cancer. Here, we hypothesized that mitotically mutable genic sub-regions that are intolerant to germline variation are enriched for cancer-driving mutations. We introduce a new metric, OncMTR, which uses 125,748 exomes in the gnomAD database to identify genic sub-regions intolerant to germline variation but enriched for hematologic somatic variants. We demonstrate that OncMTR can significantly predict driver mutations implicated in hematologic malignancies. Divergent OncMTR regions were enriched for cancer-relevant protein domains, and overlaying OncMTR scores on protein structures identified functionally important protein residues. Finally, we performed a rare variant, gene-based collapsing analysis on an independent set of 394,694 exomes from the UK Biobank and find that OncMTR dramatically improves genetic signals for hematologic malignancies. Our web app enables easy visualization of OncMTR scores for each protein-coding gene (https://astrazeneca-cgr-publications.github.io/OncMTR-Viewer/).

Abstract: During the last few years, video conferencing has become very popular and very reliable as a tool to bridge the gap where travel is not an option. And the COVID-19 epidemic has also led to lockdown orders that have led to dramatic changes in the way people work. The number of people working in the home (WFH) had increased significantly during the pandemic. The need for distance learning has also increased and has become a compulsory education system in the midst of this current situation. The Companies are also adopting an innovative recruitment process at such time. So to address this issue, our project aims to build a conference app that helps to provide communication between people through audio conferencing, video conferencing, screen sharing and messaging in real time. In this, we have created group video chat with the help of WebRTC technology and socket programming. Also we have added real-time chat feature and screen share feature. We had created the web app using Jquery for front end and node.js express.js for signaling server and real time database of Firebase for storing chats and user information. WebRTC helped us to create peer to peer connection and with the help of sockets we have done transfer of sdp packets and ice candidates. We have discussed extensively about them in our paper. Keywords: Video conferencing app, sockets, webrtc, peer-to-peer, realtime data transfer

Simulation studies allow us to explore the properties of statistical methods. They provide a powerful tool with a multiplicity of aims; among others: evaluating and comparing new or existing statistical methods, assessing violations of modelling assumptions, helping with the understanding of statistical concepts, and supporting the design of clinical trials. The increased availability of powerful computational tools and usable software has contributed to the rise of simulation studies in the current literature. However, simulation studies involve increasingly complex designs, making it difficult to provide all relevant results clearly.Dissemination of results plays a focal role in simulation studies: it can drive applied analysts to use methods that have been shown to perform well in their settings, guide researchers to develop new methods in a promising direction, and provide insights into less established methods. It is crucial that we can digest relevant results of simulation studies. Therefore, we developed INTEREST: an INteractive Tool for Exploring REsults from Simulation sTudies. The tool has been developed using the Shiny framework in R and is available as a web app or as a standalone package. It requires uploading a tidy format dataset with the results of a simulation study in R, Stata, SAS, SPSS, or comma-separated format.A variety of performance measures are estimated automatically along with Monte Carlo standard errors; results and performance summaries are displayed both in tabular and graphical fashion, with a wide variety of available plots.Consequently, the reader can focus on simulation parameters and estimands of most interest. In conclusion, INTEREST can facilitate the investigation of results from simulation studies and supplement the reporting of results, allowing researchers to share detailed results from their simulations and readers to explore them freely.
