The unsolved mystery on the disappearance of Malaysia Airlines (MAS) Flight MH370 has been one of the most highly discussed air crashed incidents in recent times. The doomed flight was en route from Kuala Lumpur to Beijing with a total of 239 people on board. Even after years of investigation and searching efforts that ended recently, investigators have not been able to identify the reason that led to the deviation from its original route shortly after taking off. Accordingly, this case study evaluates the implementation of Malaysia Airlines’ Emergency Response Plan (ERP) in handling the incident, particularly in the early crucial weeks. The data were collected from the ERP practices retrieved from several articles, reports, and journals; and they were analyzed by using a qualitative case study methodology. This study hypothesizes that there were a number of loopholes that led to an ineffective implementation of Malaysia Airlines (MAS)’ ERP steered by the Emergency Response Team (ERT) and Malaysian authorities; that was further worsened by both internal and external crisis elements ranging from unverified information to the shortcomings in the Search and Rescue (SAR) operation conducted.

In the current industrial scenario there is a serious need for formulating strategies to handle hazardous substances in the safest way. Manufacture, storage, and use of hazardous substances pose a serious risk to industry, people, and the environment. Accidental release of toxic chemicals can lead to emergencies. An emergency response plan (ERP) is inevitable to minimize the adverse effects of such releases. The on-site emergency plan is an integral component of any process safety and risk management system. This paper deals with an on-site emergency response plan for a chlorine manufacturing industry. It was developed on the basis of a previous study on chlorine release and a full scale mock drill has been conducted for testing the plan. Results indicated that properly trained personnel can effectively handle each level of incidents occurring in the process plant. As an extensive guideline to the district level government authorities for off-site emergency planning, risk zone has also been estimated with reference to a chlorine exposure threshold of 3 ppm.

Mining design is usually evaluated with different multiple-criteria decision-making (MCDM) methods when it comes to large open pit or underground ore mines, but it is not used on quarry sites. Since Croatia is mostly mining stone, the implementation of such methods in decision making of the quarry mine design is imperative but left out. In this paper, the PROMETHEE II and AHP decision-making methods are implemented on the quarry site to find out the best final quarry design contour. By implementing the MCDM methods, the best quarry model was chosen based on 22 different criteria parameters out of three final quarry designs. The chosen model is not only financially sound but also has the least environmental impact.

At present, in the Russian Federation there are no legislatively approved recommendations on the procedure, structure and requirements for the design of an emergency response plan for coal open-pit mines. Therefore, the development of this document at the mining enterprises is carried out independently, coordinating it with emergency response services serving the hazardous production facility. The absence of regulatory documents forces the enterprise management to make decisions on the structure of the document, forms and its content. This can lead to errors in conducting mine rescue operations when attracting additional forces and means from other units of rescue units. The relevance of creating a single document for everyone to draw up a plan for the elimination of accidents at sections has been repeatedly noted at production meetings with the participation of scientists, specialists from surveillance services and representatives of rescue units. In order to increase the emergency preparedness of enterprises, on the basis of studies of the current regulatory documentation of Rostechnadzor, the EMERCOM of the Russian Federation, Mines Rescue Service and scientific literature, an algorithm was developed for compiling submarines for coal open-pit mines. The analysis of accidents at the coal open-pit mines of the country made it possible to create a list of types of accidents encountered. Based on this, instructions have been developed for the actions of all workers in the mine for the initial period in the event of an accident. Taking into account the existing standard forms for coal mines, the developed document forms for open pits were developed, this will make it possible to unify emergency rescue operations regarding the preparation of documents. Scientifically sound solutions can be used to develop a plan for the elimination of accidents in the future at all mining enterprises engaged in open mining. The results of the work expand knowledge in the field of safety of coal open pit mining.
