Objective: To explore personality and readiness to change among substance use disorders (SUD) patients with and without ADHD. Method: SUD + ADHD versus SUD − ADHD patients consecutively entering treatment between 2010 and 2012 were compared concerning personality (Temperament and Character Inventory) and readiness to change (Stages of Change Readiness and Treatment Eagerness Scale). Results: Among 103 SUD patients (76 men, age M = 43.3, SD = 11.1), 16 (15.5%) were diagnosed with ADHD. SUD + ADHD patients reported significantly elevated eagerness to effort ( p = .008) compared with SUD − ADHD patients, who reported significantly elevated fear of uncertainty ( p < .000). SUD + ADHD patients reported higher ambition ( p = .025), self-forgetfulness ( p = .029), and lower recognition ( p = .022). They were younger ( p = .019) and showed more often amphetamine addiction ( p = .022) compared with SUD − ADHD patients. Conclusion: The distinct characteristics found in SUD + ADHD and SUD − ADHD patients underline the need for differentiated treatment interventions.

<B>Fragestellung:</B> Die deutsche Version eines Instruments zur Erfassung der Änderungsmotivation (Stages of Change Readiness and Treatment Eagerness Scale; SOCRATES) wurde auf ihre Faktorenstruktur und die Übereinstimmungsvalidität hin untersucht. </P><P> <B>Methodik:</B> Bei 207 Patienten in stationärer Alkoholentzugsbehandlung wurde die Änderungsbereitschaft in Form einer Selbstbeurteilung durch die SOCRATES und die Fremdbeurteilung der Änderungsmotivation durch die Therapeuten mittels der Denial Rating Scale (DRS) erhoben. Von 50 Patienten lagen beide Beurteilungen zu Beginn und am Ende der Behandlung vor. </P><P> <B>Ergebnisse:</B> Konfirmatorische Faktorenanalysen konnten das 3-Faktoren-Modell der SOCRATES nicht bestätigen und die Übereinstimmungsvalidität war gering. </P><P><B>Schlussfolgerungen:</B> Die SOCRATES ist für die individualisierte Therapieplanung in der stationären qualifizierten Entzugsbehandlung Alkoholabhängiger nicht geeignet.

Research has identified different stages that clients may go through as they change a substance use problem. These stages include precontemplation, contemplation, preparation, action, and maintenance (relapse prevention). The goals of this chapter are to learn these different stages, to identify the immediate and long-term consequences of either quitting or continuing to use substances, and to learn strategies to increase the client’s motivation for change.

This article provides a critical analysis of motivational interviewing stages of change model. Although rarely included in textbooks on human behavior and the social environment, this model has much to teach us about that aspect of human behavior most germane to social work practice—personal motivation for change of behaviors that are dysfunctional. The basic concepts that underlie motivational interviewing are derived from empirically-based principles from the science of social psychology. This article provides a historical and comparative approach to theory development, argues for the utility of this perspective as a guide to practice, and provides suggestions for further theory development.
