Collaborative governance as a policy strategy in healthcare

Purpose The purpose of this paper is to examine the enactment of collaborative governance as a policy strategy in healthcare – in particular its effects in coordinating multiple collaborative initiatives dedicated to improve the performance of health organizations. It studies overarching governance mechanisms that serve as platforms at a meta-level between policy and frontline practice. Design/methodology/approach Four collaborative governance arrangements dedicated to improve health outcomes in the Netherlands are analyzed in a comparative case-study design, based on extensive document analysis (n=121) and interviews (n=56) with key stakeholders in the field, including the Dutch Ministry of Health, health organizations and other actors. Findings The studied policy-based governance mechanisms for the coordination of multiple micro-level collaborative initiatives function partly as platforms in bringing actors and resources together successfully. They do so, by fostering evolvability (the capacity to generate diversity in emergent ways) in relation to goal-setting and intermediation between actors. Yet, they marginalize open access to participants through high selectivity and deliberate exclusion strategies for certain actors, contrary to a platform logic of action. Research limitations/implications While the collaborative governance literature focuses on these dimensions as independent elements, findings reveal both trade-offs and interdependencies between studied dimensions of coordination associated with platforms, that need to be negotiated and managed. Practical implications Selectivity and exclusion in collaborative arrangements may negatively affect relational bonds and ties between actors, which challenges the application of collaborative governance as a policy strategy in pursuit of health objectives. Originality/value Responding to recent calls in the literature, this study applies ideas from public administration to the field of health organization and management to avert failures in the translation of policy ambitions into health practice.

Purpose – The purpose of this paper is to address the question of coordination by comparing two recent reforms schemes in Norway: internal security and the welfare administration. Both concern typically transboundary “wicked” policy problems where horizontal and vertical coordination is difficult. What kind of coordination problems did the reforms address, what kind of coordination solutions were provided, and what can explain the observed pattern? Design/methodology/approach – The paper draws on organizational theory, distinguishing between a structural-instrumental and a cultural-institutional perspective. A comparative case study design is applied. The analysis combines insights from four large research projects. Findings – Both cases represent broad government efforts to tackle “wicked” coordination problems when there is a mismatch between the problem structure and the organizational structure. In both cases, reorganization and structural changes resulted in hybrid and complex organizational arrangements. The welfare administration reform tried to solve a tension between ministerial responsibility and local self-government by introducing One-stop-shops. Within the area of internal security, coordination problems related to lacking ministerial capacity was tackled by introducing a formal principle of collaboration, a lead agency approach and network arrangements. Practical implications – Effective coordination might ease wicked problems by enhancing the understanding of the problem and its underlying causes, increasing the probability of finding agreed-upon solutions and help implementation. Enhanced communication and strengthened mutual trust and commitment among actors might be a positive outcome. However, coordination implies dilemmas and trade-offs, and reformers often have to balance different interests. Originality/value – The paper shows that different instruments of coordination are central for handling “wicked problems”.

Purpose The purpose of this paper is to argue that gossip is a neglected aspect of organizational communication and knowledge, and an under-used management resource. Design/methodology/approach The paper challenges mainstream managerial assumptions that gossip is trivial or tainted talk which should be discouraged in the workplace. Instead, gossip is re-framed at an organizational level of analysis, which provides the opportunity for relational knowledge about systemic failure and poor practice in healthcare to surface. Findings Rather than simply viewing gossip as an individual behaviour and interpersonal process, it is claimed that organizational gossip is also a valuable early warning indicator of risk and failure in healthcare systems. There is potentially significant value in re-framing gossip as an aspect of organizational communication and knowledge. If attended to (rather than neglected or silenced) gossip can provide fresh insights into professional practice, decision making and relational leadership. Originality/value This paper offers a provocative challenge to mainstream health organization and management thinking about gossip in the workplace. It offers new ways of thinking to promote patient safety, and prevent the scandals that have plagued healthcare organizations in recent years.

Purpose – This paper examines how social marketing communication messages influence physical activity attitudes when a health organization is revealed as the message source. Design/methodology/approach – This paper examines how social marketing communication messages influence physical activity attitudes when a health organization is revealed as the message source. Findings – Results from three studies using experimentally manipulated messages (Studies 1 and 3) and real TV commercials (Study 2) suggest that work-framed social marketing communication messages may be more effective than fun-framed messages when the sponsoring health organization is disclosed, versus not disclosed in the ad. Research limitations/implications – This research extends the literature on source-effects on message effectiveness by suggesting that the type of message sponsor (i.e. a health organization) may influence attitudes toward the physical activity promoted in the message content. Practical implications – The results suggest that health organizations may be able to maximize communication effectiveness by employing work – rather than fun-framed messages, when it is evident that the message source is a health organization. When individuals are unaware that a health organization is the message source or when a non-health organization is the message source, fun-framed messages may be as effective for encouraging physical activity. Social implications – This research may assist health organizations to make the best use of their limited resources by providing guidance for the development of social marketing communication messages that encourage people to be physically active. Originality/value – Although source effects on marketing message effectiveness have been well established in the marketing literature, this study is the first to suggest that a health-organization message source interacts with work- versus fun-framed message content to impact the persuasiveness of messages designed to encourage physical activity.

PurposeAs a discipline, health organisation and management is focused on health-specific, collective behaviours and activities, whose empirical and theoretical scholarship remains under-utilised in the field of implementation science. This under-engagement between fields potentially constrains the understanding of mechanisms influencing the implementation of evidence-based innovations in health care. The aim of this viewpoint article is to examine how a selection of theories, models and frameworks (theoretical approaches) have been applied to better understand phenomena at the micro, meso and macro systems levels for the implementation of health care innovations. The purpose of which is to illustrate the potential applicability and complementarity of embedding health organisation and management scholarship within the study of implementation science.Design/methodology/approachThe authors begin by introducing the two fields, before exploring how exemplary theories, models and frameworks have been applied to study the implementation of innovations in the health organisation and management literature. In this viewpoint article, the authors briefly reviewed a targeted collection of articles published in the Journal of Health Organization and Management (as a proxy for the broader literature) and identified the theories, models and frameworks they applied in implementation studies. The authors then present a more detailed exploration of three interdisciplinary theories and how they were applied across three different levels of health systems: normalization process theory (NPT) at the micro individual and interpersonal level; institutional logics at the meso organisational level; and complexity theory at the macro policy level. These examples are used to illustrate practical considerations when implementing change in health care organisations that can and have been used across various levels of the health system beyond these presented examples.FindingsWithin the Journal of Health Organization and Management, the authors identified 31 implementation articles, utilising 34 theories, models or frameworks published in the last five years. As an example of how theories, models and frameworks can be applied at the micro individual and interpersonal levels, behavioural theories originating from psychology and sociology (e.g. NPT) were used to guide the selection of appropriate implementation strategies or explain implementation outcomes based on identified barriers and enablers to implementing innovations of interest. Projects aiming to implement change at the meso organisational level can learn from the application of theories such as institutional logics, which help elucidate how relationships at the macro and micro-level have a powerful influence on successful or unsuccessful organisational action. At the macro policy level, complexity theory represented a promising direction for implementation science by considering health care organisations as complex adaptive systems.Originality/valueThis paper illustrates the utility of a range of theories, models and frameworks for implementation science, from a health organisation and management standpoint. The authors’ viewpoint article suggests that increased crossovers could contribute to strengthening both disciplines and our understanding of how to support the implementation of evidence-based innovations in health care.

Purpose The use of data for instructional improvement is prevalent in today’s educational landscape, yet policies calling for data use may result in significant variation at the school level. The purpose of this paper is to focus on tools and routines as mechanisms of principal influence on data-use professional learning communities (PLCs). Design/methodology/approach Data were collected through a comparative case study of two low-income, low-performing schools in one district. The data set included interview and focus group transcripts, observation field notes and documents, and was iteratively coded. Findings The two principals in the study employed tools and routines differently to influence ways that teachers interacted with data in their PLCs. Teachers who were given leeway to co-construct data-use tools found them to be more beneficial to their work. Findings also suggest that teachers’ data use may benefit from more flexibility in their day-to-day PLC routines. Research limitations/implications Closer examination of how tools are designed and time is spent in data-use PLCs may help the authors further understand the influence of the principal’s role. Originality/value Previous research has demonstrated that data use can improve teacher instruction, yet the varied implementation of data-use PLCs in this district illustrates that not all students have an equal opportunity to learn from teachers who meaningfully engage with data.

Purpose – In order to achieve reductions in greenhouse gas (GHG) emissions, it is essential that all industry sectors have the appropriate knowledge and tools to contribute. This includes agriculture, which is considered to contribute about a third of emissions globally. This paper reports on one such tool: IMPACCT: Integrated Management oPtions for Agricultural Climate Change miTigation. The paper aims to discuss these issues. Design/methodology/approach – IMPACCT focuses on GHGs, carbon sequestration and associated mitigation options. However, it also attempts to include information on economic and other environmental impacts in order to provide a more holistic perspective. The model identifies mitigation options, likely economic impacts and any synergies and trade-offs with other environmental objectives. The model has been applied on 22 case study farms in seven Member States. Findings – The tool presents some useful concepts for developing carbon calculators in the future. It has highlighted that calculators need to evolve from simply calculating emissions to identifying cost-effective and integrated emissions reduction options. Practical implications – IMPACCT has potential to become an effective means of provided targeted guidance, as part of a broader knowledge transfer programme based on an integrated suite of guidance, tools and advice delivered via different media. Originality/value – IMPACCT is a new model that demonstrates how to take a more integrated approach to mitigating GHGs on farms across Europe. It is a holistic carbon calculator that presents mitigation options in the context other environmental and economic objectives in the search for more sustainable methods of food production.

Purpose – The purpose of this research is to increase understanding of post-project business relationships in service-intensive projects, a topic unexplored to date. This research contributes to the project marketing research focusing on post-project interaction, by building a conceptual research framework capable of illustrating the path from the initiation of a relationship through the project’s afterlife. Design/methodology/approach – A comparative case study is used across four different service-intensive project contexts to highlight the conceptual research framework, derived from the IMP-related interaction research, in practice. Findings – According to the research findings, there are at least four potential post-project business relationships associated with service-intensive projects. Furthermore, the findings indicate that these relationships embody certain antecedent and process characteristics, enabling us to compile four distinct development paths. Research limitations/implications – The four cases of the empirical research were chosen on theoretical grounds to highlight the conceptual research framework in practice, and thus the purpose was mainly descriptive. The findings should be generalized only with caution, as more empirical research is needed in this emerging project context. Practical implications – For managers, the findings provide practical guidance to deal with different post-project relationships. They will help managers to initiate, maintain and develop post-project relationships and to avoid a mismatch between relationship antecedent, processes and outcomes. Originality/value – Post-project buyer – seller interaction has been studied by the project marketing research stream, but mainly from the perspective of social exchange and sleeping relationships. With the advent of service-intensive projects, however, a whole new breed of post-project business relationships is unfolding and demanding research attention. This research is a step toward understanding the different post-project business relationships associated with service-intensive projects.

Purpose – Asset recovery proceedings increasingly target corrupt foreign officials who acquire lavish assets as a result of capital gained through criminal acts. One extremely difficult issue arising in asset recovery proceedings is whether the capital used to acquire the assets can be traced to a criminal act. The purpose of this paper is to critique US tracing procedure through comparative analysis. Design/methodology/approach – A prominent series of cases brought by the USA and France against assets owned by Teodoro “Teodorín” Nguema Obiang, second Vice President of the Republic of Equatorial Guinea, produced mixed results on the tracing element. This paper utilizes a qualitative comparative case analysis to examine the US and French cases. Findings – The US results reflect serious weaknesses in the US law as compared to more effective French asset recovery procedure. Originality/value – Though this paper is certainly a comparative case study analysis, nearly identical facts and two different jurisdictions reaching separate conclusions bring us in the legal community as close as we can realistically come to quasi-experimental research. Comparative research in this area is severely lacking and sorely needed. The mechanisms identified in the French system clearly show flaws that are present in the US system.

Purpose The purpose of this study is to investigate the effect of product market competition and corporate governance on firm’s management performance in the Tehran Stock Exchange market. According to the research literature, the governance mechanisms used in this study consist of ownership structure, structure of the board of directors and capital structure. In addition, Herfindahl–Hirschman Index and market size were used to measure the product market competition. Design/methodology/approach This study used one selected sample among the firms in the capital market of Iran from 2004 to 2012. Findings The results of this study indicated that there is a significant relation among the major governance mechanisms (including ownership concentration, independence of the board of directors and debt ratio) and product market competition and management performance. The findings of this study also showed that product market competition is effective on the relation between corporate governance and the performance, and this is what has been ignored in most of the conducted studies. Originality/value In general, the results of this study supported the idea that product market competition is effective on implementation and efficiency of governance mechanisms.
