A case is described of Tunga penetrans infection in the feet of a tourist returning from South America to Scotland The condition had initially mimicked verrucae vulgaris, but microscopic examination of the lesions together with the travel history allowed the diagnosis of tungiasis to be made. This is the first case reported in Scotland

“Practically every volume of the United States Statutes contains one or more acts or joint resolutions of Congress authorizing action by the President in respect of subjects affecting foreign relations, which either leave the exercise of the power to his unrestricted judgment, or provide a standard far more general than that which has always been considered requisite with regard to domestic affairs.” Thus did Mr. Justice Sutherland summarize the legal precedents for the President's actions in imposing an embargo on arms shipments to certain belligerent countries in South America, after Congress had delegated to him discretionary authority in such matters. This is more than a succinct historical summary. It is the first case in which the dicta of the Court has made clear that sharp line of distinction between the Chief Executive's discretionary powers in foreign affairs and those in internal matters.

Coccidioidomycosis (CM) is an endemic fungal infection that is found in the Southwestern United States and adjacent areas of Mexico as well as Central and South America. In the United States, 150 000 to 300 000 infections occur annually. The majority are asymptomatic. Of the symptomatic cases, the majority are primary pneumonic disease that varies from mild to very severe. A minority of persons develop disseminated disease (extrapulmonary disease). These typically manifest as meningitis, osteomyelitis, synovitis, and integumentary. CM has been described in virtually every part of the body, including the genitourinary system. Disseminated CM to the genitourinary tract is well known to occur but is rarely documented. It is believed this is the first case to report disseminated CM to the renal parenchyma. Diagnosis and treatment are described in a 56-year-old Hispanic male.

Malaria falciparum merupakan infeksi yang disebabkan oleh Plasmodium falciparum (Pf) yang sering menimbulkan komplikasi hingga timbul malaria berat dengan mortalitas yang tinggi. Manajemen malaria di daerah terpencil memiliki tantangan tersendiri sebagai akibat dari kurangnya kemampuan sumber daya manusia dan fasilitas diagnosis dan tata laksana yang memadai. Pada artikel ini akan dibahas mengenai kasus infeksi Malaria Falciparum dengan diagnosis yang ditegakkan secara pasti melalui pemeriksaan mikroskopis darah yang menyatakan adanya parasit Pf. Pasien sudah menerima Artemisinin-Based Combination Therapy (ACT) berupa dihidroartemisinin (DHA)-piperakuin (PPQ) dan primakuin (PQ). Pada evaluasi setelah pemberian obat ACT dan primakuin, tidak ada perbaikan klinis dan mikroskopis. Pengobatan malaria lini kedua berupa kina, doksisiklin dan PQ diberikan dan memberikan perbaikan klinis dan mikroskopis.Kata Kunci: Artemisinin Combination Therapy, doksisiklin, kina, malaria falsiparum  Challenges in Providing Treatment of Severe Malaria Case in a District Hospital of Remote Area in IndonesiaSevere Malaria is commonly found due to Plasmodium falciparum infection, which is causing high mortality in patient. A Severe case might be difficult to treat optimally in the remote settings area that could be due to lack in awareness of disease manifestation and inadequate diagnostic facilities. We report a case of Severe Malaria Falciparum infection, confirmed by blood smear eamination of Plasmodium falciparum (Pf) parasites. Patient had received Artemicinin Based Combination Therapy (ACT), which consisted of dihydroartemisinin (DHA)-piperaquine and primaquine (PQ) with no clear clinical and microscopy improvement. The second line anti malaria consisted of quinine, doxycycline, and PQ was then initiated, showing clinical and microscopy responses.
