Bite turbos are a relatively new but widespread fabrication used in a variety of orthodontic cases. Bite turbos placed on lingual surfaces of upper incisors help disclude the posterior teeth and open the bite. This report describes a simple, effective, and economical technique to fabricate resin composite bite turbos. This method does not require laboratory procedures and can be manufactured using materials that are usually available in a regular dental practice.

Objective. The aim of this study was to evaluate the effect of different disinfectant agents on bond strength of two types of resin composite materials.Methods. A total of 80 sound posterior teeth were used. They were divided into four groups(n=20)according to the dentin surface pretreatment (no treatment, chlorhexidine gluconate 2%, sodium hypochlorite 4%, and EDTA 19%). Each group was divided into two subgroups according to the type of adhesive (prime and bond 2.1 and Adper easy one). Each subgroup was further divided into two subgroups according to the type of resin composite (TPH spectrum and Tetric EvoCeram). Shear bond strength between dentin and resin composite was measured using Universal Testing Machine. Data collected were statistically analyzed byt-test and one-way ANOVA followed by Tukey’spost hoctest.Results. It was found that dentin treated with EDTA recorded the highest shear bond strength values followed by sodium hypochlorite and then chlorhexidine groups while the control group showed the lowest shear bond strength.Conclusions. The surface treatment of dentin before bonding application has a great effect on shear bond strength between resin composite and dentin surface.

Purpose: To compare the restorative material used in the treatment of posterior teeth taught and performed in two Dental Schools (UFRGS and ULBRA) and in 8 basic health units (BHU) from the Public Health System in Porto Alegre, Brazil. Materials and methods: Data referring the teaching of restorative procedures using amalgam (AM) or resin (R) were obtained through the analysis of patient’s files and questionnaires applied to final year dental students. Information regarding restorative procedures at BHU was obtained through patient’s records and a questionnaire applied to the dentists. The type of restorative material used in both BHU and Dental Schools were compared by chi-square test. Results: At UFRGS, 327 restorations were performed, 78.28% R and 21.72% AM, and at ULBRA 366 restorations, 92.63% R and 7.37% AM. At BHU, 1664 restorations were performed (35.93% R and 64.07% AM). A major proportion of AM restorations was performed in the Public Health Service in comparison to both Dental Schools, in which resin restorations prevailed (p=0.000). Conclusion: The change from AM to R in the dental material choice for posterior teeth at Dental Schools was not followed by the Public Health System, where the AM is still widely used in posterior teeth.

Background: The newly introduced self-adhering flowable resin-composites decrease the required time for application by incorporation of an acidic adhesive monomer, thus reducing the number of the steps, but its bonding is still uncertain. The aim of this study was to evaluate the interfacial microscopic examination and chemical analysis at the resin-dentin interface of a self-adhering flowable resin composite (Vertise-Flow) versus a total-etch (Te-Econom Plus) resin composite, using an etching agent (Eco-Etch gel) and  bonding agent (Single Bond Universal). Methods: Sixteen freshly extracted sound human posterior teeth were used. The teeth were randomly divided into two groups: 8 specimens per type of composite. Standard-shaped class V cavities were prepared on the buccal surface. One group was restored by Te-Econom Plus resin composite by total-etch technique using Eco-Etch gel, which was applied to dentine for 15 seconds, followed by rinsing, drying and bonding agent application (Single Bond Universal). The other group restored directly with self-adhering resin composite (Vertise-Flow) without application of etch or bond. Curing was done for 20 seconds using a light emitting diode light curing unit. Evaluation of the resin-dentin interface was done microscopically by examination of marginal gap distance in μm using scanning electron microscope (SEM), and chemical analysis of silver particles was observed using SEM with energy-dispersive X-ray spectrometry after 24 hours of specimen storage in ammoniacal silver nitrate. Results: Regarding marginal gap distance (µm) and silver atomic % mean values, teeth restored with self-adhering resin composite (Vertise-Flow) showed significantly higher mean values than the multi-step etch and rinse resin composite group (5.2 vs 0; 12.2 vs 8.2, respectively). Conclusions: Resin-dentin bonding using total-etch resin composite technique was more effective than self-adhering flowable resin composite (Vertise-Flow) regarding marginal gap formation and penetration of silver particles. Further studies for bond strength could be performed.

Silver diamine fluoride (SDF) is applied topically to arrest caries and has an increasing evidence base to support its efficacy, both in the primary dentition and to arrest root caries in older patients. It can be used as a non-aerosol generating procedure and is a simple technique. It has a side-effect of discolouring caries black, a factor which requires discussion with patients and their carers prior to application. Due to its efficacy and simplicity, it is a useful intervention for the management of caries. CPD/Clinical Relevance: Caries is a common condition in the UK, and silver diamine fluoride offers a simple alternative management technique when case selection is appropriate.

SUMMARY Objective: To analyze the shrinkage stress, bonding interaction, and failure modes between different low-viscosity bulk fill resin composites and conventional resin composites produced by the same manufacturer or a high-viscosity bulk fill resin composite used to restore the occlusal layer in posterior teeth. Methods & Materials: Three low-viscosity bulk fill resin composites were associated with the conventional resin composites made by the same manufacturers or with a high-viscosity bulk fill resin composite, resulting in six groups (n=10). The bonding interaction between resin composites was tested by assessing the microshear bond strength (μSBS). The samples were thermocycled and were tested with 1-mm/min crosshead speed, and the failure mode was evaluated. The post-gel shrinkage (Shr) of all the resin composites was measured using a strain gauge (n=10). The modulus of elasticity (E) and the hardness (KHN) were measured using the Knoop hardness test. Two-dimensional finite element models were created for analyzing the stress caused by shrinkage and contact loading. The μSBS, Shr, E, and KHN data were analyzed using the Student t-test and one-way analysis of variance. The failure mode data were subjected to chi-square analysis (α=0.05). The stress distribution was analyzed qualitatively. Results: No significant difference was verified for μSBS between low-viscosity bulk fill resin composites and conventional or high-viscosity bulk fill composites in terms of restoring the occlusal layer (p=0.349). Cohesive failure of the low-viscosity bulk fill resin composites was the most frequent failure mode. The Shr, E, and KHN varied between low-viscosity and high-viscosity resin composites. The use of high-viscosity bulk fill resin composites on the occlusal layer reduced the stress at the enamel interface on the occlusal surface. Conclusions: The use of high-viscosity bulk fill resin composites as an occlusal layer for low-viscosity bulk fill resin composites to restore the posterior teeth can be a viable alternative, as it shows a similar bonding interaction to conventional resin composites as well as lower shrinkage stress at the enamel margin.

The use of resin composite for routine restoration of cavities in posterior teeth is now commonplace, and will increase further following the Minamata Agreement and patient requests for tooth-coloured restorations in their posterior teeth. It is therefore relevant to evaluate the published survival rates of such restorations. A Medline search identified 144 possible studies, this being reduced to 24 when inclusion criteria were introduced. Of these, ten directly compared amalgam and composite, eight were cohort studies, and six were systematic reviews. It was concluded that posterior composites may provide restorations of satisfactory longevity and with survival rates generally similar to those published on amalgam restorations. However, the ability of the operator in placing the restoration may have a profound effect. CPD/Clinical Relevance: With the increasing use of composite for restorations in posterior teeth, it is relevant to note that these may provide good rates for survival.

Combined periodontal and prosthodontic treatment demands of patients require a structured coordination of pretreatments and an adequate choice of restorations. This is especially true if multiple teeth are missing and insufficient crown-root ratios are obvious. A 40-year-old patient with a severe periodontitis (Stage IV, Grade C) was treated with active, nonsurgical periodontal therapy. Afterwards, a supportive periodontal therapy was provided with a recall interval of three to four months. Due to a high tooth mobility of the anterior teeth in the upper jaw and a missing left canine, a combination of a resin composite (Signum composite, Kulzer, Hanau, Germany) and a unidirectional glass prepreg fiber (Tender Fiber Quattro, MICERIUM, Avegno, Italy) was utilized to fabricate a splint in a labside approach to stabilize the remaining teeth. Moreover, an artificial denture tooth was adhesively luted to the splint. A temporary polymer-based material (Vita CAD-Temp, VITA Zahnfabrik, Bad Säckingen, Germany) was selected to supply the posterior teeth of the patient with a 3-unit fixed dental prosthesis (FDP), and both restorations were adhesively cemented. 19 months after insertion, a fracture of the fiber-reinforced resin composite splint occurred that was intraorally repaired. In spite of the fracture of the splint, all materials were functionally and esthetically stable over the follow-up period of 22 months.
