Sexual Disorders in the Course of Clomipramine Treatment: A Report of Three Cases

Clomipramine, a tricyclic antidepressant, has been used in the treatment of obsessive neurosis successfully. In the course of treatment with this medication, sexual disorders in the form of decreased libido and impotence were noticed in three patients whose cases are reported. These disturbances occurred in the first week and lasted during the course of treatment. Upon withdrawal of the medication, the side effects disappeared. These side effects seem to be more common than is usually thought. The physician should be alerted to them as they can be a source of noncompliance.

SummaryPlasma levels of antidepressant drugs were measured in 17 depressed patients at roughly monthly intervals over follow-up periods of three to 26 months. Good results in seven were associated with small fluctuations in level from visit to visit, while big fluctuations were associated with poor outcome. Such fluctuations were probably from poor compliance or drug interaction from self-medication. Severe side effects and cardiotoxicity were associated with high levels.

Tricyclic antidepressants (TCAs) are still prescribed worldwide for the treatment of major depression and other disorders. Unfortunately, the safety margin of these agents is rather narrow and TCA-related intoxication is associated with a number of potentially life-threatening side effects. The symptoms being rather unspecific, a high index of suspicion is warranted in order to identify promptly the intoxicated patients and to provide the appropriate care, even before the detection and dosage of TCAs and related metabolites in the blood and/or urine. In the absence of a specific antidote, the treatment basically consists in the support of ventilation in comatose patients, the correction of arterial hypotension, the treatment of arrhythmias, and the enhancement of renal excretion of TCAs.

Dibenzepin hydrochloride is a new tricyclic antidepressant drug with pharmacological properties midway between those of imipramine and amitriptyline. In a double-blind comparative trial, J. M. Fielding (Med. J. Australia, 1969, 1, 614) found no significant difference in the speed of the effect of the two drugs in depressed patients. He reported that side-effects rated subjectively by patients were maximal before starting on the drugs and tended to decrease with time. The following trial was accordingly staged.

IntroductionFor a long time, antidepressants sexual side effects have been neglected. Currently, no reliable scientific data is available regarding the nature and frequency of sexual dysfunction induced by antidepressants. The aim of our study was to evaluate the prevalence and type of sexual dysfunction induced by antidepressants, and to identify factors associated with the occurrence of these disorders.MethodologyA descriptive and analytical cross-sectional study extending over a period of two week. For the purpose of this research, a socio-demographic card, the Arizona Sexual Experiences Scale (ASEX) and the Psychotropic-Related Sexual Dysfunction Questionnaire (SALSEX) were used.ResultsFifty-five patients were recruited. The diagnosis of major depressive episodes was dominant (49.1%). Moreover, fluoxetine and tricyclic were in top of the list of antidepressants with respective proportions of 41.8% and 38.2% and respective dose 20.86 mg/24 h and 72.38 mg/24 h. The score using the ASEX scale was 14.63 ± 5.23. Using the SALSEX scale, 47.3% of patients claimed to have had sexual disorders secondary to antidepressants with a moderate score of 9.19 ± 2.56. Furthermore, sexual disorders were more common in the elderly aged of 45 (66.66%) as well as in patients started on paroxetine (66.66%) and on sertraline (66.66%) (P ≤ 0.05).ConclusionThe sexual side effects of antidepressants have a major impact on the quality of life and adherence to treatment. They also represent an important risk factor for relapse and recurrence in major depression, in this context, the prescription of an antidepressant.Disclosure of interestThe authors have not supplied their declaration of competing interest.

From the ancient times human have always tried to correct the sex related problems from various herbal medicinal plants and their extracts with various dosage forms. The history of sex therapy is very old and herbal drugs have been the great reliever as the sex therapy from the evolution of human. Instead of the current scenario with the synthetic compounds the herbal drugs are far more safe and comparatively effective without any serious side effects. Antihypertensives such as β-blockers can cause the side effects which are responsible for the diminished quality of life. The chronic use of such drugs can be more fatal in some cases than their actual gain. Several sexual problems can occur such as erectile dysfunction , loss of libido, ejaculation disorders etc. The phytochemicals found in the plants which are having aphrodisiac properties shows sildenafil like effect on human body without the side effects of synthetic compound. The pharmaceutically active compounds found in natural aphrodisiacs such as flavonoids , saponins, free amino acids and vitamins are known to be libido enhancer and hence useful in treating sexual disorders. The natural aphrodisiacs can be derived from any source including medicinal plants , vegetables, flowers ,roots or fruits and their pharmacological potential varies as per the part of the plant used. The target of this review article is to deliver information about medicinal plants for the protective action against fall-off libido due to chronic use of β-blockers and to investigate for further secondary phytochemicals for aphrodisiac potential.

Introduction: Despite advancements in the treatment of depression and availability of newer compounds, TCAs likeamitriptyline remain to be the cornerstone of antidepressive therapy for more than three decades, however significantly more patients receivinga tricyclic withdraw from treatment mainly because of side effects. Higher response, lower incidence of side effects and improved compliancecan be enhanced by the optimal use of Therapeutic Drug Monitoring (TDM). No research data is currently available on the therapeutic drugmonitoring of TCAs in Pakistan. Objectives: To compare the relative efficacy of Tricyclic antidepressant in the treatment of depression, with andwithout Therapeutic Drug Monitoring Main Outcome Measures: Changes in HAMD scores in patients on TCAs. Study Design: A Quasiexperimental study design was used. Setting: The study was conducted at Department of Psychiatry, Military Hospital Rawalpindi. Subjects:34 patients completed the study in the monitored group (with TDM) and 33 in control group (without TDM). Methods: Serum TCA levels andHAMD scores at baseline and subsequently at sixth, eighth and tenth week of treatment were collected. In all the subjects, all the blood sampleswere drawn as a fasting sample in early morning. Results: The mean age of the monitored group was 31.97 years (SD=10.432) while that of thecontrol group was 33.52 years (SD=9.385). in the monitored group 20 (58.82 %) of the patients were males while 14 (41.17%) were females, inthe control group 22 (66.66%) were males and 11(33.33%) were female patients. A significant reduction in HAMD scores was noted at 8 weeksof treatment. The groups did not differ in terms of efficacy of TCAs, however the monitored group had fewer dropouts than the control group.Conclusions: Lower incidence of side effects and improved compliance can be enhanced by the optimal use of Therapeutic Drug Monitoring(TDM) of TCAs.

This paper calls attention to a relatively unnoticed problem, ie, morbidity in neonates caused by treatment of pregnant women with the tricyclic antidepressant clomipramine (Anafranil). Recently we have seen identical clinical pictures in two neonates born to mothers treated throughout pregnancy with clomipramine because of an endogenous depression (200 mg daily in case 1 and 100 mg daily in case 2). Both babies were delivered without complications at term after otherwise uneventful pregnancies. CASE REPORTS Case 1 Physical examination of the neonate at birth revealed the following: birth weight, 3,550 gm; height, 52 cm; and head circumference, 34.6 cm. Apgar scores were 9 at one minute and 10 at five minutes.
