Ageing Does Not Affect Ulnar Variance: An Investigation in Taiwan

This paper presents a longitudinal study of the effect of ageing on ulnar variance. Between 1976 and 1985, ulnar variance in 1000 normal adult subjects was measured using the Palmer method. In 1995 to 2002, 17 to 22 years later, the ulnar variance was measured again in 864 of them. They were stratified into three age groups: Group I consisted of 351 subjects of young age (20–39 year-old), Group II of 318 subjects of middle age (40–59 year-old) and group III of 195 subjects of old age (60 year-old or older). The means of the initial and the final ulnar variance of the whole series and the three groups were compared by the paired Student’s t-test. There were no significant differences between the initial and final ulnar variances in all three age groups and in the whole series. In normal subjects without diseases or injuries affecting the wrist, ageing does not affect the ulnar variance.

The aim of this study was to investigate the rate of occupational leg swelling depending on the time period of the working day. Volumetric variations of the legs of 70 hospital employees, enrolled in three groups, were evaluated. Group I: 35 morning shift workers; Group II: 35 afternoon shift workers; and Group III: 15 individuals randomly selected from Groups I and II, who were evaluated on the day they worked 12 hours consecutively. Volumetry was performed before and after each shift for both legs of the participants in Groups I and II. For Group III volumetry was performed early in the morning, at noon and in the evening. For statistical analysis, the Student's t-test and Mann-Whitney test were used with an alpha error of 5% being considered acceptable ( P value < 0.05). Significant increases in volume were recorded for the limbs in all three groups ( P value < 0.001). On comparing Groups I and II, the accumulation of fluids was significantly higher in the morning than in the afternoon ( P value < 0.003). Asymptomatic workers may present with oedema of the legs during their work with the rate of oedema being different for morning and afternoon shifts. The possibility of wearing compression stockings should be considered for this type of work.

SummaryPlasma thrombomodulin (TM) has attracted considerable attention as a marker of endothelial cell membrane injury. We examined fluctuations in plasma TM levels in patients receiving therapy for the disseminated intravascular coagulation syndrome (DIC) using an enzyme immunoassay. Sixty healthy controls and 18 patients with DIC were studied.The mean ± SD of the TM values initially measured immediately after the onset of DIC was 42.00 ± 20.85 ng/ml, which was markedly increased as compared with the control value of 15.36 ±4.85 ng/ml (p <0.001).Fluctuations in the TM levels over time were studied after dividing the patients according to the presence or absence of improvement in the underlying disease and improvement or lack thereof in the coagulation findings. Group I showed improvement in both categories, Group II showed improvement only in the latter, and Group III showed no improvement in either category.In Group I, the mean ± SD of initial measured TM levels was 37.02 ±10.12 ng/ml and the mean of final values decreased to 58.9% of the initial value. This decrease was significant by paired Student’s t-test (p <0.01). The initial value in Group II was 45.86 ± 18.86 ng/ml and the final values increased to 117.0% of the initial values, this difference was not significant. The initial value in Group III was 44.48 ±21.53 ng/ml and the final values increased to 143.4% of the former. This increase was significant by paired Student’s t-test (p <0.05). The difference in % fluctuations between Group I and Group III was significant by Wilcoxon’s test (p <0.01).These results suggest that the measurement of plasma TM can be useful in the management of DIC.

AbstractThis study was aimed to evaluate different management modalities for peripheral vascular trauma in children, with the aid of the Mangled Extremity Severity Score (MESS). A single-center retrospective analysis took place between 2010 and 2017 at University Hospitals, having emergencies and critical care centers. Different types of vascular repair were adopted by skillful vascular experts and highly trained pediatric surgeons. Patients were divided into three different age groups. Group I included those children between 5 and 10 years; group II involved pediatrics between 11 and 15 years; while children between 16 and 21 years participated in group III. We recruited 183 children with peripheral vascular injuries. They were 87% males and 13% females, with the mean age of 14.72 ± 04. Arteriorrhaphy was performed in 32%; end-to-end anastomosis and natural vein graft were adopted in 40.5 and 49%, respectively. On the other hand, 10.5% underwent bypass surgery. The age groups I and II are highly susceptible to penetrating trauma (p = 0.001), while patients with an extreme age (i.e., group III) are more susceptible to blunt injury (p = 0.001). The MESS has a significant correlation to both age groups I and II (p = 0.001). Vein patch angioplasty and end-to-end primary repair should be adopted as the main treatment options for the repair of extremity vascular injuries in children. Moreover, other treatment modalities, such as repair with autologous vein graft/bypass surgery, may be adopted whenever possible. They are cost-effective, reliable, and simple techniques with fewer postoperative complication, especially in poor/limited resources.

ABSTRACT Objective: The purposes of this investigation were to determine the horizontal distances between the mandibular posterior teeth and the WALA ridge in a sample of Peruvians with normal occlusion and to compare them by tooth type, sex, arch side, and age groups. Methods: 65 dental casts of subjects with normal occlusion were collected. Posterior teeth, except for third molars, were evaluated. The horizontal distances between the occluso-gingival midpoints of the buccal surfaces (FA points) of each tooth and the WALA ridge were measured using a modified digital caliper. The values between each different tooth type within the sample were compared using the ANOVA and Scheffe tests, while comparisons by sex, arch side and age groups, using the Student’s t-test. Results: The mean distances in the sample was 0.96 mm for first premolars, 1.45 mm for second premolars, 2.12 mm for first molars and 2.55 mm for second molars. Statistically significant differences between each of the four tooth types were found. There were no significant differences found between sex, arch side and age groups. Conclusion: The horizontal distances between the mandibular posterior teeth and the WALA ridge increased progressively from the first premolars to the second molars in Peruvians with normal occlusion. The WALA ridge was a good landmark to evaluate the positions of posterior teeth in Peruvians with normal occlusion.

Aim: Study was conducted to examine the different parameters of human thymus glands of young and old patients and compare the findings between both age groups. Study Design: Comparative/observational study Place and Study: Study was conducted at Anatomy department of Nishtar Medical University Hospital, Multan for duration of six months from 15th January 2020 to 15th July 2020. Methods: Total 54 specimens of human thymus of 54 patients were enrolled in this study. All specimens were divided in to two groups I and II, Group I contains 27 patients with ages <30 years and group II with 27 patients having ages 45 to 60 years. All specimens were fixed in 10% formalin solution and then processed for paraffin embedding. Compare the different parameters such as thickness of interlobular connective tissue and thymic capsule, length and number of Hassal’s corpuscles between both groups. Data was analyzed by SPSS 24.0. Results: In group I 12 (44.44%) patients were ages <15 years and 15 (55.56%) patients were ages >15 years. In group 13 (48.15%) and 14 (51.85%) patients were ages <50 years and >50 years. There was a significant difference observed between both groups regarding thickness of interlobular connective tissue and thymic capsule, quantity and length of Hassal’s corpuscles with p-value <0.05. Conclusion: It is to be concluded that patients with young age had significantly less thickness of thymic capsule and interlobular connective tissue with more in number and decreasing size of Hassal’s corpuscles as compared to old age patients. ABSTRACT: The aim of this research was to measure the intraocular pressure (IOP) of normal chinchilla eyes using the rebound tonometer. A further aim was to assess whether there were differences in the values of intraocular pressure in relation to animals age, gender and time of day. Thirty-six chinchillas were divided into three groups of 12 chinchillas each, by age: Group I (2-6-month-old), Group II (20 and 34 months) and Group III (37 and 135 months). Ophthalmic examination was performed previously by Schirmer tear test, slit lamp biomicroscopy, indirect ophthalmoscopy and fluorescein test in all chinchillas. Three measurements of intraocular pressure were assessed on the same day (7, 12 and 19h). Tonometry was performed on both eyes using the rebound tonometer after calibration in "p" mode. Statistical analysis was performed with SigmaPlot for Windows. The mean IOP for groups I, II and III were 2.47±0.581mmHg, 2.47±0.581mmHg and 2.51±0.531mmHg, respectively. No significant differences were reported between age and IOP and no significant differences were reported between the time of day and IOP. The IOP in chinchillas did not differ significantly between genders or ages of the animals, and did not change with time of day.

Objective. To investigate the mortality rate, comorbidity prevalence, and etiology of community-acquired pneumonia (CAP) in elderly patient population. Materials and Methods. Hospitalized elderly patients with CAP were distributed into the following age groups: 65–74 years (group I), 75–84 years (group II) and 85–94 years (group III). The patients’ medical records were used for determining comorbidities and mortality rate. In order to determine etiology of CAP, sputum or BAL samples were collected. A total of 171 isolates were identified using MALDI-TOF MS. Results. The mortality rates were 27.1%, 31.5% and 45.7% in age groups I, II, III, respectively. The most common concomitant diseases in all age groups were arterial hypertension (47.4%, 54.6%, and 62.8% for groups I, II, and III, respectively), chronic heart failure (45.7%, 50.9%, and 60.0%, respectively), and coronary heart disease (15.2%, 25.9%, and 24.3%, respectively). The most frequently isolated bacteria by age group were the following: group I – non-fermenting Gram-negative bacteria (NFGNB) (7.4%), Enterobacterales (6.6%), S. aureus (6.6%); group II – Enterobacterales (13.9%), S. aureus (5.6%), Enterococcus spp. (5.6%), NFGNB (2.8%); group III – NFGNB (15.4%), S. aureus (7.7%), Enterococcus spp. (7.7%), Enerobacterales (7.7%). Conclusions. The mortality rates in elderly patients with CAP were high and varied from 27.1% in 65–74 years old patients to 45.7% in 85–94 years old patients. The most common comorbidities in all age groups were arterial hypertension (up to 62.8%), chronic heart failure (up to 60%), and coronary heart disease (up to 25.9%). The main pathogens causing CAP in elderly patient population were Enterobacterales and non-fermenting Gram-negative bacteria.

Background: Hemoglobin E disease are the most common structural haemoglobinopathies, reaching its highest frequency in South East Asia and also prevalent in Bangladesh. There are very few data for effective screening of HB E trait. The aim of the study was to analyze hematological profile and RBC indices as a marker of screening for Hb E trait.Methodology: A cross sectional observational study was conducted among 150 subjects at Dhaka Shishu (Children) Hospital from December 2008 to November 2009. On the basis of Hb electrophoresis, all subjects were divided into three groups. Group I comprised of 50 subjects with Hb E trait, group II comprised 51 subjects with ß thalassemia trait and Group III comprised of 49 normal subjects. Result of RBC indices and blood count were compared between Hb E trait and normal study population.Results: In group I the mean age was 29.53 years and the age range was 2-47 years. In group II the mean age was 30.31 years and the range was 1-55 years. In group III the mean age was 21.81 years and the range was 1-52 years. The present study revealed the mean±SD haemoglobin level (g/dl) in group I (E trait) and group III (normal subjects) were 11.76±1.91 and 10.77±1.4 respectively. Statistically no significant difference was found between these 2 groups (P>.05). The mean corpuscular volume (MCV) in mean±SD in group I and group III were 72.89±12.24 fl and 77.35±10.27 fl respectively. No statistical significance was found as P>0.05. The mean corpuscular haemoglobin (MCH) and mean corpuscular haemoglobin concentration (MCHC) in group I were 24.84±2.71pg & 32.97±2.27 g/dl and in group III were 28.28±2.71 pg & 32.97±2.38 g/dl respectively. There were no significant difference between group I and III regarding MCH and MCHC.The mean RBC count in E trait and normal subject group were 5.01±71 million/cmm and 4.61±0.65 million/cmm respectively. There was no significant difference was found P>0.05. Mean red cell distribution width (RDW) in group I and III were 14.32 ±2.34 and 14.83±1.28 without any statistical difference (P>0.05)Conclusion: In our study there were no diagnostic significance of Blood count and RBC Indices for detection of Hb E trait. ABSTRACT Objective: The objective of the following study is to investigate the mandibular vertical asymmetry in a group of patients with early unilateral mandibular first molar extractions. Materials and Methods: Mandibular asymmetry index measurements (condylar, ramal and condylar-plus-ramal) were performed on the panoramic radiographs of a study group including 51 patients (mean age: 18.60 ± 1.11 years) and a control group of 51 patients (mean age: 18.53 ± 1.29 years). Group I included patients with a unilateral mandibular first molar extracted before the age of 12 years. Group II included patients with no extractions and had excellent Class I relationships, no missing teeth and slight or moderate anterior crowding. A paired t-test was used to determine possible statistically significant differences between the sides for the measurements. Student's t-test was used for the comparison of asymmetry index values between the groups and genders. Results: No group showed statistically significant sex-or side-specific differences for posterior vertical height measurements. Condylar asymmetry index and ramal asymmetry index measurements were not statistically different between the groups, while condylar-plus-ramal asymmetry index (CRAI) measurements were statistically different between the groups (P = 0.019). Conclusions: A slight difference for CRAI value was found in patients with early unilateral mandibular first molar extractions.

Background/Aim. There are many specificities of merital infertility and sometimes surprising connections between some thinks with no connections at first sight. Examinations of these patients imply diagnostic actions such as the blood basal hormone sample, doing hysterosalpingography, ultrahysterosonography, ultrasound examinations, and sometimes laparoscopy and hysteroscopy if there are necessary. The aim of the study was to determine the characteristics of the connection between policystic ovary (PCO) syndrome (Sy) and congenital M?llerian ducts abnormalities. Methods. This study included 356 patients treated in the period from January 1, to December 31, 2009, in the Department of Infertility of the Clinic for Obstetrics and Gynecology in Nis, Serbia. Exclusion criteria were no myoma, ovary cysts, tubal and male factors of infertility. Results. A total of 180 patients were divided into 3 groups: the group I with PCO sy, the group II with uterine congenital malformation and the group III with a combination of these disorders. The middle age of patients was 29.6 ? 4.8, body mass index (BMI) was 26.1 ? 4,8 kg/m2 the middle thicknes of endometrium was 5.2 + 2.7 mm, and there were no significant differences between the examined groups. There were no significant among in a number of miscarriages in the examined groups. We found that PCO Sy and congenital abnormalities of M?llerian ducts were conjoint in 30% of examined patients. Conclusion. Conjoined PCO Sy and congenital abnormalities of M?llerian ducts do not result in a higher number of misscarriages than only either PCO Sy or abnormalities of M?llerian ducts. It is important to check BMI, basal level of follicle stimulating hormone and number of antral follicles because the induction protocol and concentracion of inductors depends on these characteristics, thus, the successful cycles and pregnancy.
