Abstract Aim A case of an 85-year-old patient with concurrent use of warfarin and medical cannabis containing delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) is described. Warfarin continues to be a cornerstone of anticoagulation treatment despite the recent addition of FDA-approved anticoagulant agents. It is well known that warfarin has numerous drug interactions; however, much remains unknown about its interaction with THC and CBD. A literature review was conducted to identify documented cases of possible interactions between cannabis and warfarin. The case reports we identified noted that cannabis may potentially increase warfarin’s effect. Therefore, we aimed to determine why an effect was not seen on our patient’s warfarin dose despite daily use of medical cannabis. Case This case report describes an 85-year-old patient who despite starting an oromucosal medical cannabis regimen of THC and CBD (which provided 0.3 mg of THC and 5.3 mg CBD once daily and an additional 0.625 mg of THC and 0.625 mg CBD once daily as needed) had minimal INR fluctuations from October 2018 to September 2019. Conclusion Despite the introduction and use of medical cannabis therapy, with both THC and CBD components, an elderly patient with concurrent warfarin use did not see major INR fluctuations, in contrast to published literature. The potential for warfarin and THC/CBD interactions may be dependent on route of administration and dose of the cannabis product.

An elderly patient had COVID-19 infection in August 2020 and started the home remedies treatment for the first 18 hours, followed by Azithromycin 250 mg for 6 days. The patient revered well, and the infection was confirmed by antibodies in the patient's serum. Later on, the patient was on Matily Herbal Drink and Matily Herbal & Spices Mix to avoid complications connected with COVID-19 re-infection. After completing 12 months of post COVID-19 infection, the antibodies were assessed to find the status. It was found to be increased in qualitative assessment. The quantitative assessment of antibodies showed a much higher value compared with individuals who had both the dose of vaccines and tested the blood after two weeks since the second dose of vaccine. We suggest that the increased antibodies could be because of the bioavailability of polyphenols present in the Matily Herbal Drink and Matily Herbal & Spices Mix. Polyphenols inactivate COVID-19 virus and this may have helped the body to increase its antibody production. The bioavailability of polyphenols depends on various factors, including acidulants in the diet. Based on the present studies, we suggest India should have her own strategies to increase antibodies in the population instead of just following the norms laid by International Agencies for the booster dose of vaccine  Keywords: COVID-19, Matily Herbal Drink, Matily Herbal, Spices Mix

Background: In Japan, more than 1,000 participants died shortly after receiving the coronavirus disease 2019 (COVID-19) vaccine, but the causal relation between the injection and death remains uncertain. Methods: Applying long-term personal vital care data for 28 months for an elderly patient, I investigated and evidenced adverse reactions after the first dose of the COVID-19 Pfizer vaccination. Results: The precise, detailed, and continuous data statistically clarified the long-term fevers associated with no meals or drinks. Interrupted time series analysis showed significant and fluctuating increases of body temperatures, pressures, and pulses, although solely long-term plots showed an abrupt and timely increase in these vital data after the vaccine. Conclusions: Anorexia was fatal, and newly reported in the present care records since the patient received the first dose of the COVID-19 vaccine.

Superior mesenteric vein thrombosis (SMVT) is a rare complication of acute appendicitis. It has no specific clinic manifestation. Anticoagulation, antibiotics and surgery are the main treatment pillars. We report herein a case of an 86-years-old women treated for acute appendicitis complicated with SMVT and massive pulmonary embolism.
