BACKGROUND: The modern trend of dentistry is the principle of minimal invasiveness. Preservation of the vitality of the tooth is an actual principle of operation. In the treatment of deep caries or pulpitis by a biological method, the pulp chamber is often opened. Even with bleeding, there is a chance to leave the tooth vital. After hemostasis, antibacterial treatment of the carious cavity and padding were performed. Domestic manufacturers offer two analogues for these purposes "Trioxident" and "Kanal MTA. AIM: The purpose of the study is to compare the effectiveness of domestic materials "Trioxident" and "Kanal MTA" when used as a therapeutic and insulating coating of pulp. MATERIALS AND METHODS: 27 clinical cases were analyzed in patients aged 2335 with a diagnosis of "deep dentine caries" K02.2 (according to ICD-10) of molars and premolars of the upper and lower jaws. All clinical cases combined patients' complaints of intermittent aching pains, the depth of carious cavities and the opened tooth cavity at one point. The patients were divided into two groups, in each of which the following stages of diagnosis and treatment were carried out. Diagnostics was carried out before the start of work and during examination after 7 days, 6, 12, 18 months. The stages of caries treatment were the same, only the materials for closing the perforations differed: in group 1 "Trioxidant", in group 2 "Kanal MTA". RESULTS: It is noted that the use of materials "Trioxident" and "Kanal MTA" is an effective choice for closing perforations of the pulp chamber in the treatment of deep caries and chronic pulpitis. Pain was absent in 30.8% and 42.1% of patients in groups 1 and 2, respectively. After 6, 12, 18 months, patients did not complain about the treated teeth, the cold test was positive for a short time in 26 cases, the average current at electroodontodiagnostic (minimum value 4, maximum value 8) was 5.6 A, 25 restorations had an Alpha rating according to Ryge criteria, in one case in group 1, the tooth crown darkened pointwise from the vestibular surface. CONCLUSION: The materials "Trioxident" and "Kanal MTA" are recommended for use in the biological method of pulpitis treatment. They have therapeutic potential to cover the pulp and are competitive in relation to the foreign equivalent of "MTA Prorut", which gives them an economic advantage in providing dental care within the framework of compulsory medical insurance.

Relevance. On September 27, 2017, Russia announced the completion of disposal of chemical weapons with organophosphorus toxic substances. ahead of its international obligations. Besides men, military women also served in enterprises for disposal of these weapons.Intention. To assess the impact of occupational factors on the primary morbidity of female military personnel (n = 267) served in enterprises for disposal of chemical weapons with organophosphates in 2007-2016.Methodology. The object of the study was data on the primary morbidity of female military personnel employed in disposal of chemical weapons with organophosphates (group 1) and serving in support and service units (group 2). The results were compared with the indicators of primary morbidity of all female military personnel Armed Forces of Russia. Nosologies were correlated with the chapters of diseases by the International Statistical Classification of Diseases and Related Health Problems (ICD-10).Results and Discussion. In Group 1 of female military personnel, the primary morbidity for a number of disease categories was better than in Group 2 and all the female military personnel of the Russian Armed Forces, which can be explained by careful preliminary selection before recruting, good work organization, and timely prevention of initial health disorders and rehabilitation of the functional reserves of the body. In military women of Groups 1 and 2, compared to all the female military personnel of the Russian Armed Forces, a statistically significantly higher level of primary morbidity was revealed for mental disorders and behavioral disorders (Chapter V by ICD-10). This can be explained by the cumulative effect of mental tension during disposal of chemical weapons. Of note, the above disorders were not the leading category of primary morbidity in both female military personnel working in enterprises and in all the female military personnel of the Russian Armed Forces. In female women of Group 1 compared to all the female military personnel of the Russian Armed Forces, there was a statistically significantly higher level of diseases of the musculoskeletal system and connective tissue (Chapter XIII by ICD-10). It can be assumed that negative memories of the likelihood of emergencies when moving weapons that require disposal create additional physical efforts and overstrain of individual muscles. It is appropriate to point out that diseases of the musculoskeletal system and connective tissue increased in female military personnel in all groups and overall in 2007-2016. In Group 1 of female military personnel, there was an increase in proportion of diseases from Chapter XIII vs certain stability of their percentage in all the female military personnel f the Russian Armed Forces.Conclusion. The results obtained showed a sufficient effectiveness of preventive and protective measures during the disposal of organophosphate weapons by female military personnel.

ABSTRACT Objective: This study was designed to investigate radiographically the effects of age and external irritating stimuli on the morphology and thickness of the pulp chamber ceiling and floor in mandibular molars. Materials and Methods: A total of 234 panoramic radiographs were recruited and 494 mandibular molars were examined in this study. The sample was divided into six age groups: Group 1, <20 years; Group 2, 20-29 years; Group 3, 30-39 years; Group 4, 40-49 years; Group 5, 50-59 years and Group 6, >60 years. Each group was subdivided into two subgroups with regard to the tooth condition (intact or non-intact teeth). Four distances, two angles and two ratios were measured, in order to estimate the dimensions of the pulp chamber and the thickness of the pulp chamber ceiling and floor. Results: The thickness of the pulp chamber ceiling and floor increased significantly from Group 1 to 6. The pulp chamber floor angle presented progressive sharpness from Group 1 to 6 whereas the pulp chamber ceiling angle presented progressive bluntness from Group 1 to 5 and sharpness from Group 5 to 6. Significant differences were identified in the thickness of the pulp chamber ceiling, the thickness of the pulp chamber floor and the ceiling-floor distance between intact and non-intact teeth. Conclusion: Based on those results, there are differences in the location of secondary dentin formation between the two pulp chamber walls. Age is related to diminished pulp chamber size. The increase rate of the pulp chamber ceiling thickness is similar to that of the pulp chamber floor thickness. Furthermore, external irritating stimuli have an effect on the pulp chamber dimensional changes.

This retrospective study analyzed the surgical and refractive outcomes of a XEN Gel Implant (Allergan, Abbvie Company, Irvine, CA, USA) in naïve patients versus those with previous glaucoma surgery. We evaluated the efficacy of XEN implantation in 86 glaucoma patients during a long-term follow-up period. Patients were divided into two groups: naïve patients (Group 1) and patients with previous glaucoma surgery (Group 2). Eyes that received a XEN Gel Stent placement from December 2014 to October 2019 were included. Intraocular pressure (IOP) change, corrected distance visual acuity (CDVA), change in glaucoma medications, frequency of slit lamp revision procedures, and frequency of secondary glaucoma surgeries were the primary outcomes. In Group 1, the mean IOP before surgery was decreased significantly from 25.00 ± 7.52 mmHg to 16.83 ± 5.12 mmHg by the end of the study. In Group 2, the mean IOP decreased significantly from 25.35 ± 7.81 mmHg to 17.54 ± 5.34 mmHg. The mean IOP decrease from baseline was 29% in Group 1 and 27% in Group 2 (p = 0.567). There were no significant differences between the groups in the IOP baseline level, the final level, or the change between preoperative and final levels. The qualified success rate for Group 2 was 68.7% versus 76.5% for Group 1 for the initial procedure and 15.4% vs. 20.2%, respectively, for complete success rate (p > 0.05). However, at the end of the follow-up, more patients achieved an IOP < 18 mmHg in Group 1 than in Group 2. Despite the need for more anti-glaucoma medications, repeat XEN Gel implantation appears to show promising results in patients with previously failed anti-glaucoma procedures, owing to its minimal invasiveness.

This study evaluated the microtensile bond strength (μTBS) of 1-step self-etch adhesives (1-SEAs) and 2-step self-etch adhesives (2-SEAs) to pulp chamber dentin immediately after bleaching with 2 types of common bleaching techniques. Pulp chamber dentin of bovine teeth was bleached using 30% hydrogen peroxide (H2O2) solution with quartz-tungsten-halogen light-curing unit (Group 1) and 3.5% H2O2-containing titanium dioxide (TiO2) (Pyrenees®) activated with 405-nm violet diode laser for 15 min (Group 2). Unbleached specimens were placed in distilled water for 15 min and used as controls. After treatment, dentin was bonded with resin composite using 1-SEA or 2-SEA and stored in water at 37°C for 24 h. Each specimen was sectioned and trimmed to an hourglass-shape andμTBS was measured. Fractured specimens were examined under a scanning electron microscope to determine fracture modes. All specimens in Group 1 failed before proper bonding tests. In Group 2, theμTBS of 2-SEA was significantly greater (with no failed specimens) than 1-SEA (where 21 out of 36 failed). These results indicate that 2-SEA is a better adhesive system than 1-SEA on bleached dentin. Our results also demonstrated that application of H2O2significantly decreases bond strength of resin to dentin; however, in the case of nonvital tooth bleaching, Pyrenees® is a better alternative to the conventional 30% H2O2bleaching.

The purpose of this study was to evaluate in vitro the dentin permeability of the deciduous pulp chamber floor after employing 2-octyl cyanoacrylate and Er:YAG laser. Twenty four deciduous molars were used, divided into four groups. After chemical-surgical preparation each group received a different treatment: Group 1 - control, without treatment; Group 2 - the floor of the pulp chamber was covered with a fine layer of 2-octyl cyanoacrylate; Group 3 - the floor of the pulp chamber was irradiated with Er:YAG laser (250 mJ, 10 Hz for 30 seconds, 80 J of energy and 320 pulses), and covered with a fine layer of 2-octyl cyanoacrylate; and Group 4 - the floor of the pulp chamber was irradiated with Er:YAG laser set at the parameters already described. After that the specimens received application of 0.5% methylene blue, for 15 minutes. The teeth were cut, photographed, and the digitalized images were analyzed using the ImageLab program. The results obtained were submitted to statistical analysis. Group 4 (Er:YAG) presented the largest averages in percentage of dye penetration area (19.5%), followed by Group 1 (11.1%), Group 3 (1.4%) and Group 2 (0.2%). The experimental model allowed to conclude that the specimens conditioned with 2-octyl cyanoacrylate (Group 2) and Er:YAG laser associated to 2-octyl cyanoacrylate (Group 3) presented a decrease in permeability, and the specimens treated with Er:YAG laser (Group 4) presented an increase in permeability of the analyzed area.

OBJECTIVE: to investigate the influence of three different rotary systems for cervical flaring on establishment of the real working length. MATERIAL AND METHODS: Thirty mandibular first molars were submitted to conventional endodontic access and initial working length measurement, followed by irrigation/suction of the pulp chamber with 5% sodium hypochlorite. Teeth were randomly divided into 3 groups (n=10) and cervical flaring of the mesiobuccal canals were performed using one of the following instrument systems: Group 1 - Gates-Glidden burs; Group 2 - Orifice Openers; Group 3 - La Axxess system. Two subsequent numbers of instruments of each rotary system were used and the final working length was recorded. A digital calyper was used to record the working length, in millimeters, to investigate a possible discrepancy between initial and final measurements. RESULTS: Analysis of variance (Anova) na Tukey test revealed statistical difference between Group 1 and Groups 2 and 3 (p£ 0.05). CONCLUSIONS: all groups presented shorter working length after cervical flaring; groups prepared with instruments La Axxes and Orifice Opener presented the best results among the systems studied.

Eating disorders of bulimic type are among the most common comorbidities with depression. The objective is to evaluate cognitive function and mood dynamics in patients with depression and eating disorders in pharmacotherapy. In total, 52 outpatients, who met criteria for “major depressive episode” (ICD-10), participated. The level of depression was estimated with Hamilton Depression rating scale (HAM-D) and cognitive function–Montreal Cognitive Assessment (MoCa). Sample was divided into two groups. Patients of group 1 also met criteria for eating disorder of bulimic type and patients of group 2 did not have any eating disorder. Treatment included standard doses of SSRI. Assessments were performed after 2, 4 and 8 weeks (D14, D30, D60). The level of HAM-D was significantly greater (P < 0.05) in eating disorders group (16.75 ± 2.83 in group 1; 13.04 ± 1.93 in group 2 at screening) and significance was preserved till D60 (9.39 ± 2.54 in group 1; 6.32 ± 1.27 in group 2 at D60). Clinically significant antidepressive effect was revealed faster in group 2 (at D7) compared to group 1 (at D14). Overall score of MoCA was significantly lower (P < 0.05) in eating disorders group (20.33 ± 0.54 in group 1; 23.43 ± 2.32 in group 2 at screening) at all stages of treatment (23.39 ± 0.78 in group 1; 26.96 ± 3.27 in group 2 at D60) and it reached normal range (25 and more) only in group 2 at D60. Significant change from screening was revealed at D30 at group 2 and at D60 at group 1.ConclusionEating disorder have an impact on SSRI treatment efficacy including antidepressive and procognitive effects. It is necessary to reveal eating disorders as a co-morbidity in patients with depression.Disclosure of interestThe authors have not supplied their declaration of competing interest.

OBJECTIVE: To assess the frequency of comorbidities of mental and behavioral disorders (CMBD) in psychoactive substance (PAS)-dependent patients with different periods of abstinence cared for at Alcohol and Other Drug Psychosocial Care Centers (CAPS-ad). METHOD: All patients under treatment in the two CAPS-ad of the city of Uberlândia-MG, between April and September 2010, were consecutively assessed. The ICD-10 symptom checklist was used to diagnose CMBD; additional information was obtained from interviews and medical records. The patients were divided according to duration of abstinence: < 1 week (Group 1); 1-4 weeks (Group 2); and > 4 weeks (Group 3). RESULTS: Of all patients assessed, 62.8% were diagnosed with CMBD, which were more frequent (p < 0.05) in Group 1 (72%) than Group 3 (54.2%), and both groups were similar to Group 2 (61%). Depressive and anxiety disorders were more frequent among patients of Group 1. Mood disorders were more frequent (p < 0.05) in women [22/34 (65%) vs. 54/154 (35.1%)], whereas psychotic disorders were more frequent (p = 0.05) in men [16/154 (10.4%) vs. 0]. The presence of CMBD was associated with more severe clinical conditions. CONCLUSIONS: The higher frequency of diagnosis of CMBD in patients of Group 1 may have resulted from the difficulties in distinguishing mental disorders that are due to PAS intoxication or withdrawal from those that are not. However, to make the diagnosis of CMBD, even during detoxification, can increase the likelihood of better response to treatment.

Aim. This study aimed to estimate the results of congenital long bone deformities using the consecutive application of guided growth and external fixation. Materials and methods. We performed a retrospective analysis of the treatment results of 38 children with congenital deformities of long bones. Group 1 consisted of 17 children who underwent consecutive application of two methods: guided growth and external fixation. Group 2 (control group) consisted of 21 children who underwent isolated lengthening and deformity correction by external fixation. Results. There were 14 complications in group 1 and 25 complications in group 2. Moreover, only seven cases in group 1 had complications requiring surgical treatment, whereas 17 cases in group 2 required operative treatment for complications. There was a relatively low level of refractures: zero cases in group 1 and three cases in group 2. The most common complication was a recurrence of deformity associated with the continuous growth of children: seven cases in group 1 and eight cases in group 2. However, no recurrence of the torsion component of deformity was observed in any group 1 cases, and repeated guided growth could be performed in the six cases of growing children. Conclusion. The consecutive use of external fixation and guided growth to treat congenital deformities of the lower limbs is a promising direction for pediatric orthopedics because it reduces the incidence of complications. The repeated use of guided growth, because of its minimal invasiveness, is the most effective solution for the recurrence of deformity in a growing child.
