AbstractLifestyle-related factors are major determinants/modifiers of prognosis in patients with cirrhosis. Accumulating evidence indicates that malnutrition, obesity, sedentary lifestyle, alcohol and smoking habits, and likely poor oral hygiene can increase the risk of progression of the disease, and some of them are linked to higher risk of hepatocellular carcinoma. Importantly, lifestyle-related factors can be largely corrected, and as such they represent an attractive approach to be added to etiological and pharmacological therapy in patients with cirrhosis. Nonetheless, lifestyle is often neglected in this population. In this concise review, the authors present evidence supporting lifestyle changes in patients with cirrhosis—including, but not limited to, nutrition and physical activity in malnourished and obese patients. They also discuss some elements of motivational interviews as a tool to support a better interaction between hepatologists and patients in this field.

Adherence to lifestyle changes - beginning to exercise, for example - is assumed to be mediated by self-referent thoughts. This paper describes a pilot study and three studies conducted to develop and validate a questionnaire for adults to determine their self-perceptions related to health-oriented exercise. The pilot study identified items pertinent to the domains considered important in this context, and began the process of selecting items. Study 2 examined the factor structure, reduced the number of items, determined the internal consistency of the factors, and explored the discriminative validity of the questionnaire as to physical activity level and gender. Four factors with a total of 24 items were accepted, measuring mastery of exercise, body perception, social comfort/discomfort in the exercise setting, and perception of fitness. All subscales had acceptable internal consistencies. Preliminary validity was demonstrated by confirming hypothesized differences in scores as to gender, age, and physical activity level. The third study examined and demonstrated convergent validity with similar existing subscales. The fourth study examined an English-language version of the questionnaire, confirming the existence of the factors and providing preliminary psychometric evidence of the viability of the questionnaire.

Khmer Girl’s in Action is a nonprofit that successfully utilizes community-based participatory research (CBPR) with university partners to create social change for youth in Long Beach, CA. Based on semi-structured interviews and content analysis of news articles, I explore the impact and sustainability of this research work and the research partnerships. Findings highlight impacts such as youth empowerment, heightened awareness around community needs, policy change, and CBPR curriculum improvements in the field as impacts. Sustainability requires integrating research into program funding, utilizing a tailored training curriculum, building on community members prior relationships, and selecting partners that share common goals, levels of commitment, and flexibility. As funders demand more data to justify community needs, understanding more examples of such work in the Asian American community will be useful for informing future partnerships.

Many are the factors involved in the pathogenesis of excess weight with interactions between genetic, environmental and biological factors and therefore, also the therapeutic approach must be multidisciplinary and multidimensional.
