The article presents the issues of complex treatment of osteoarthritis of the knee with the use of preformed factors in postmenopausal women compared to standard drug therapy. The study included 60 women aged 50 to 75 years with clinical manifestations of osteoarthritis of the knee joints in the postmenopausal period. Complex treatment with preformed factors has been found to be an effective treatment for this category of patients, improving the clinical course of osteoarthritis by 53% and improving quality of life.

The purpose of the study is to improve rehabilitation treatment of patients with chronic pancreatitis (CP) at inpatient stage by strengthening pharmacological potential of drug therapy due to inclusion of therapeutic physical factors (TPF) in therapeutic programs. Materials and methods. 159 patients with acute CP were observed. By simple randomization, 4 groups were formed: the control group (MG, 39 people) received standard drug therapy; 1st comparison group (GC1; 38 people) additionally received TPF; GC2 (40 people) in addition to treatment in GC1 had drinking mineral water «Slavyanovskaya»; in main group (42 people) in addition to the treatment in GC2 got preformed peloidotherapy on the cervical-collar zone. All the patients underwent the evaluation of clinical score and quality of life before and after medical rehabilitation. Results. In MG, clinical symptomatology leveling was by 78,2% (p<0,01), in GC1 — by 71,5% (p<0,01), GC2 — by 62,3% (p<0,01), CG — by 57,2% (p<0,01) on average immediately after the treatment, which was in a clear correlation with indicators of quality of life. In the long term (in 6 and 12 months), the advantage of combination therapy was noted with the same validity, the preservation of the achieved positive result was mostly noted in the MG: after 6 months the improvement in physical health compared to the initial values was noted by 34,4% (p<0,01), after 12 months — by 24,0% (p<0,05); mental — by 32,3% (p<0,01) and 22,5% (p<0,05), respectively. In both comparison groups, positive dynamics was 10–12% lower, and in the control group, after 6 months, there was only a tendency to improve quality of life indicators. Conclusion. The inclusion of TPF in the programs of the inpatient stage of medical rehabilitation of patients with chronic pancreatitis by strengthening the pharmacological potential of drug therapy contributes to the leveling of clinical manifestations (abdominal pain, dyspepsia and diarrhea), the result of which is an improvement in the quality of life of this category of patients.

Obesity consistently associated with the development of a number of chronic diseases, leading to a decrease in quality of life, disability and death. The article examines the connection between obesity and disease of the musculoskeletal system, describes the mechanisms by means of which obesity leads to the development of osteoarthritis. It is evident that reduction of body mass can slow the progression of osteoporosis. The own experience of non-pharmacological and pharmacological treatment of obesity with the use of orlistat in 50 obese patients with osteoarthritis of the knee IIIII stage is presented. Treatment has resulted in a decrease in body weight, waist circumference, accompanied by a decrease in symptoms osteoarthritis among all the patients. Our results showed that the addition of orlistat to standard osteoarthritis scheme leads to significant reduction in weight and reduction of clinical manifestations of osteoarthritis. According to the above, the drugs that have impact on weight loss, should be included in the treatment regimen of patients with osteoarthritis and obesity. Pain syndrome, along with mechanical jaundice, is one of the most evident clinical manifestations of pancreatic cancer and is often a sign of tumor neglecting. Existing treatment options for chronic pain in cancer patients are diverse and include the various ways of impact on the different links of pathological pain: medicinal, endoscopic, surgical. With this pathology, drug therapy using analgesics, weak and strong opioids, is usually not effective enough. In such cases, preference is given to chemical neurolysis — denervation of the autonomic structures of the retroperitoneal space, carried out under the guidance of ultrasound or computed tomography. Conducting percutaneous neurolysis of the celiac plexus with the use of alcohol or phenol is prescribed upon insufficient effectiveness of pharmacotherapy. Surgical methods of anesthesia are used extremely rarely, as they considerably worsen the patient’s condition, without significantly affecting the quality of life or the prognosis. Thoracoscopic splanchnicectomy is considered a modern and effective method of endoscopic treatment of pain in unresectable pancreatic cancer, the conduction of which can significantly decrease the intensity of pain, reduce the number of narcotic analgesics and improve the quality of life of cancer patients. This minimally invasive intervention is the basis for the subsequent lifelong systemic drug therapy.

The article highlights the pathogenesis, clinical manifestations of lesions of the central and peripheral nervous system that have arisen or persist in patients in the postcoid period (Long-COVID-19). Their correct assessment, the use of effective methods of complex treatment, targeted neurorehabilitation contribute to the reversibility of functional disorders, prevention, reduction of disability, improvement of quality of life indicators, prevention of the progression of cognitive, emotional, behavioral disorders initiated by SARS-CoV-2.

Целью исследования было выявление возможных проблем диагностики, междисциплинарного взаимодействия и причины дефектов терапии пациентов с остеоартритом на амбулаторном приеме, а также оценка эффективности и безопасности применения в рутинной клинической практике у пациентов с остеоартритом коленных суставов препарата, основой которого является биоактивный концентрат мелких морских рыб. Проанализированы результаты амбулаторного приема ревматолога и истории болезни 2312 пациентов с направительным диагнозом «остеоартрит» различной локализации. Оказалось, что из всех обратившихся к ревматологу пациентов с направительным диагнозом «остеоартрит» 47% направлений можно расценить как безосновательные. Было обнаружено, что 45% пациентов даже при установленном достоверном диагнозе «остеоартрит» на амбулаторном приеме терапия назначалась не в полном объеме и не соответствовала рекомендациям по ведению данной категории больных. После двух курсов (в течение года) лечения препаратом на основе биоактивного концентрата мелких морских рыб эффектом от терапии были удовлетворены 67% пациентов с остеоартритом коленных суставов, отметившие снижение интенсивности боли, улучшение функции суставов и качества жизни. В группе наблюдения не было зарегистрировано нежелательных явлений. Несмотря на то, что в настоящее время проблема остеоартрита хорошо освещена, в реальной клинической практике на этапе первичного звена врачи нередко сталкиваются со сложностями решения как диагностических, так и терапевтических задач. В связи с этим возникает острая необходимость в пересмотре и адаптации к рутинной амбулаторной практике междисциплинарных алгоритмов ведения больных остеоартритом, с определением порога вмешательства врача первичного звена и специалиста. Существует также необходимость в разработке и внедрении в рутинную клиническую практику оптимизированных шкал/опросников для динамической оценки качества жизни пациентов с остеоартритом коленных суставов. Проведенное исследование демонстрирует достаточно высокую эффективность, безопасность и доступность препарата на основе биоактивного концентрата мелких морских рыб, а лечение данным препаратом может широко применяться в комплексной терапии остеоартрита коленных суставов. The purpose of the study was to identify the problems of diagnosis, interdisciplinary interaction and the causes of defects in the treatment of patients with osteoarthritis on an outpatient basis, as well as to assess the efficacy and safety of using the medicine from bioactive concentrate of small marine fish, in routine clinical practice in patients with knee osteoarthritis. The results of an out-patient reception hours with a rheumatologist and case histories of 2312 patients with a referral diagnosis of osteoarthritis of various localization were analyzed. It turned out that 47% of all referrals to a rheumatologist with a referral diagnosis of osteoarthritis can be regarded as unfounded. It was found that 45% of patients, even with an established reliable diagnosis of osteoarthritis, on an outpatient basis, therapy was not prescribed in full and did not comply with the recommendations for the management of this category of patients. After 2 courses (within a year) of treatment with a drug based on a bioactive concentrate of small marine fish, 67% of patients with osteoarthritis of the knee joints were satisfied with the effect of therapy, noting a decrease in pain intensity, an improvement in knee joint function and quality of life. No adverse events were reported in the observation group. Despite the fact that at present the problem of osteoarthritis is well covered, in real clinical practice at the stage of primary care physicians often face difficulties in solving both diagnostic and therapeutic problems. In this regard, there is an urgent need to revise and adapt to the routine outpatient practice of interdisciplinary algorithms for managing patients with osteoarthritis, with the determination of the intervention threshold of a primary care physician and a specialist physician. However, there is a need to develop and introduce into routine clinical practice optimized scales/questionnaires for dynamic assessment of the quality of life of patients with knee osteoarthritis. The study demonstrates a fairly high efficacy, safety and availability of a drug based on a bioactive concentrate of small marine fish, and treatment with this drug can be widely used in the complex therapy of osteoarthritis of the knee joints.

This review analyzes the clinical symptoms, differential diagnosis of COVID-19 and influenza to facilitate clinical diagnosis of both diseases. The development of the COVID-19 epidemic occurred with an exponential growth of the incidence due to the high contagiousness of the virus, asymptomatic carriage. The situation was aggravated by the contagiousness of the patient in the last 2–3 days of the incubation period (before clinical manifestations), which made it difficult to isolate patients and contacts in a timely. The spectrum of damage to the organs and systems is constantly expanding with a more detailed study of the clinical course of COVID-19. Sometimes, the manifestations of COVID-19 persist even during the period of convalescence, which cause a decrease in the performance and quality of life of patients. The article discusses neurological manifestations, gastrointestinal symptoms and features of pneumonia in COVID-19. The clinical features of influenza and the main differences in the clinical course from COVID-19 are also demonstrated.

: Sepsis-associated encephalopathy (SAE) is a common complication in the acute phase of sepsis, and patients who develop SAE have a higher mortality rate, longer hospital stay, and worse quality of life than other sepsis patients. Although the incidence of SAE is as high as 70% in sepsis patients, no effective treatment is available for this condition. To develop an effective treatment for SAE, it is vital to explore its pathogenesis. It is known that hyperammonemia is a possible factor in the pathogenesis of hepatic encephalopathy as ammonia is a potent neurotoxin. Furthermore, our previous studies indicates that non-hepatic hyperammonemia seems to occur more often in sepsis patients; it was also found that >50% of sepsis patients with non-hepatic hyperammonemia exhibited encephalopathy and delirium. Substatistical analyses indicate that non-hepatic hyperammonemia is an independent risk factor for SAE. This study updates the definition, clinical manifestations, and diagnosis of SAE; it also investigates the possible treatment options available for non-hepatic hyperammonemia in patients with sepsis, and the mechanisms by which non-hepatic hyperammonemia causes encephalopathy.

According to the results of the study in the main group of patients who received complex treatment (IPP and neuroleptic), there was a significant positive dynamics of the main clinical manifestations, compared with the control group. All patients reported a significant decrease in the intensity of heartburn and pain.The use of probulin with vegetative-corrective action in the complex treatment of patients with GERD with vegetative dystonia syndrome allowed to more effectively stop the painful manifestations and reduce the emotional experience of bodily discomfort, improve the emotional state and mental health of patients, as well as improve their quality of life.
