Introduction: Stroke is a major health problem in the world, and associated with mortality, disability, and economic burden. Stroke is the second leading cause of death in the world after heart disease and cause of disability in adults. Several recent studies show that levels of Lp-PLA2 contribute to high serum lead to ischemic stroke, whereas some others do not get the association. Aims: studies an association between serum Lp-PLA2 with incident ischemic stroke. Method: This study is a descriptive analytic comparative with cross sectional design in the Neurology ward RS. DR. M. Djamil Padang between October 2013 to April 2014 with total of 88 subjects consisting of 44 subjects ischemic stroke patients and 44 healthy control group who participated in the study. Statistical analysis is used parametric test with unpaired t test, test for normality with the Kolmogorov-Smirnov test, and logistic regression analysis. Result: P value 0.05 was considered statistically significant. The mean age of subjects in the group of ischemic stroke patients was 57.30 ±8.86 years old. Female in patients with ischemic stroke group was 23 people (52,3%). Mean levels of Lp-PLA2 serum in patiens with ischemic stroke group was 103.49±48.00 ng/mL. A significan association between serum levels of Lp-PLA2 with incident ischemic stroke with p<0.05. Conclusion: There was a significant relation in elevation of serum Lp-PLA2 levels with incident of ischemic stroke.

Background: Cognitive impairment is a common condition that may affect up to 50% of stroke patients. Post stroke cognitive impairment is associated with reduced quality of life, which may increase the number of dependency. Recurrent stroke may happen in approximately 25% patients and they have higher rates of cognitive impairment. Objective: The aim of this study is to examine the comparison of cognitive function between first ischemic stroke and recurrent ischemic stroke patients in Hasan Sadikin Hospital, Bandung. Methods: This study is a retrospective, cross-sectional study using the data recruited between the year 2012 - 2016 at the Department of Neurology at the Hasan Sadikin Hospital Bandung. The data collected in this study were demographic data, including age, level of education, and residence, and the clinical data as well. Cognitive function was assessed using Mini-Mental State Examination (MMSE). The comparison between the cognitive function between both groups were analyzed using the Mann-Whitney U test. Results: There were 428 subjects eligible for this study, with 207 subjects categorized as first ischemic stroke group and 221 subjects categorized as recurrent ischemic stroke group. There was a statistically significant difference in MMSE scores between the first ischemic stroke patients (24.90 ± 4.64) and recurrent ischemic stroke patients (22.85 ± 4.64 ) with a p value of 0.002. Conclusion: Recurrent ischemic stroke patients had lower MMSE scores than the first ischemic stroke patients. Clinicians should be more aware in detecting early cognitive impairment in stroke patients and in preventing the incidence of recurrent stroke.

Biosignal can provide information about body conditions, including physiological conditions of ischemic stroke. The regulation of blood in the brain is regulated through the mechanism of Cerebral Autoregulation (CA). Some parameters that can be used to determine this mechanism are Blood Flow Velocity (BFV) and Blood Pressure (BP). Stroke is also related to nervous system activity, which is represented through the Heart Rate Variability (HRV). This study aims to determine the relationship between those biosignals and their effects on the physiology of ischemic stroke sufferers. The subjects were divided into two groups (20 strokes and 20 controls). BFV data is obtained in the Middle Cerebral Artery (MCA), BP is obtained through the arteries of the upper arms, and 3 leads electrocardiogram is placed in the chest. The results showed that there was a relationship between BP and BFV in the control group (p-value < 0.05; r = -0.574). This correlation was not found in the stroke group. The relationship between BP and HRV was only found in the stroke group, which was associated with high sympathetic activity and lower parasympathetic activity (p-values < 0.05 and r > 0.4). It was based on SDRR, RMSSD, CVRR, LF, and SD1 parameters. In the control group, there was no relationship between HRV and BP. The relationship between BFV and HRV in the control group was not found statistically. Still, in the stroke group, this relationship was found in the LF and LF/HF Ratio parameters (p-value < 0.05; r > 0.4). Based on this research, parameters on HRV that can be used to determine the characteristics of stroke patients in all positions are MeanRR, VLF, and LF

Background: Parathyroid hormone (PTH) is vasoactive, and the endothelium is one of the target tissues of this hormone. Hyperparathyroidism is frequently associated with hypertension.Objective: To determine if hyperparathyroidism, which develops particularly in elderly women as a compensatory mechanism to osteoporosis, may be a risk factor for ischemic stroke.Methods: Serum PTH levels and bone mineral density (BMD) in 107 elderly patients with ischemic stroke (≥65 years old) were assessed on the day of onset. The control group consisted of 107 healthy volunteers matched for age and sex.Results: BMD was significantly lower and serum PTH higher in female stroke patients than in control subjects; there was a negative correlation between these two measurements. One-third of the female stroke patients had a serum PTH level higher than the mean + 2 SD of the control subjects (high PTH group), and the interval between menopause and the stroke was significantly longer in the high PTH group than in the normal PTH group. Multiple logistic analyses revealed hypertension and ischemic heart disease were more prevalent in the high PTH group. BMD and PTH were normal in male stroke patients.Conclusion: High serum PTH level may be associated with high incidence of ischemic stroke in women, possibly through the increased incidence of hypertension.

Ischemic thalamus stroke has become a serious cardiovascular and cerebral disease in recent years. To date the existing researches mostly concentrated on the power spectral density (PSD) in several frequency bands. In this paper, we investigated the nonlinear features of EEG and brain functional connectivity in patients with acute thalamic ischemic stroke and healthy subjects. Electroencephalography (EEG) in resting condition with eyes closed was recorded for 12 stroke patients and 11 healthy subjects as control group. Lempel-Ziv complexity (LZC), Sample Entropy (SampEn), and brain network using partial directed coherence (PDC) were calculated for feature extraction. Results showed that patients had increased mean LZC and SampEn than the controls, which implied the stroke group has higher EEG complexity. For the brain network, the stroke group displayed a trend of weaker cortical connectivity, which suggests a functional impairment of information transmission in cortical connections in stroke patients. These findings suggest that nonlinear analysis and brain network could provide essential information for better understanding the brain dysfunction in the stroke and assisting monitoring or prognostication of stroke evolution.

Background: Diabetes and higher HbA1c level have increased the incidence of stroke. Hemoglobin levels both high and low are associated with poor outcomes. Leukocytes play an important role in the initiation of the atherosclerosis process.Methods: This was a cross-sectional study with a sample size of 62 people selected by non-random sampling method on a consecutive basis, patients with acute stroke were taken blood to measure blood glucose levels, HbA1c, Hb and Leucocytes when entering the hospital room. Outcomes were assessed using NIHSS and MRS. The calculation of NIHSS and MRS scores was performed on the first day of admission and the fourteenth day. To analyze the correlation of blood sugar levels during HbA1c, Hb and leukocytes in acute stroke, this study used Spearman's correlation test. The p value <0.05 was considered statistically significant.Results: The study subjects of 62 acute stroke patients (acute ischemic stroke 31 people and 31 people hemorrhagic stroke). Acute stroke patients were consisted of 38 men (61.3%) and 24 women (38.7%). Of 31 people with hemorrhagic stroke, there were 16 men (51.6%) and 15 women (48.4%), ischemic stroke patients were consisted of 22 people (71, 0%) and women were 9 people. Spearrman repair test showed no symptoms between blood glucose levels, HbA1c, Hemoglobin and leukocytes with outcomes in acute stroke.Conclusions: There was no significant association between blood glucose levels, HbA1c, hemoglobin and leukocytes with outcomes in acute stroke.

Cognitive decline is a significant complication that affects most stroke survivors. Early detection of cognitive decline in ischemic stroke patients and identification of risk factors improves their clinical outcomes. This study aimed to determine the characteristics of cognitive status in the sub-acute phase of ischemic stroke. A cross-sectional study was conducted on 89 sub-acute ischemic stroke patients in three hospitals in West Nusa Tenggara recruited consecutively from August 2019 to April 2020. The data collected were demographic and clinical characteristics, cognitive status, and functional outcome. The association between clinical and demographic characteristics and cognitive decline was analyzed using logistic regression. In addition, the relationship between cognitive status and functional outcomes of these patients was examined using the chi-square test. This study revealed that the prevalence of cognitive decline in these subjects was 71.9%. Multiple logistic regression showed that age was the only characteristic associated with cognitive decline in the subjects (OR = 5.12,95% CI = 1.08-24.28). Furthermore, the frequency of cognitive decline in these subjects was significantly associated with functional outcomes (p-value =0.014). Thus, there was a high prevalence of cognitive decline in sub-acute ischemic stroke patients associated with increasing age and poor functional outcomes.

Background: Hematological parameters are major indicator of ischemic stroke. Hematological parameters and stroke incidence have not been investigated in local population. Objective: The present study was designed to assess whether hematological parameters play an important role in ischemic stroke patients. Methodology: For this purpose a cross-sectional study was done at Department of Pathology,Darbhanga Medical College andHospital, Laheriasarai, Bihar from January 2020 to August 2020. A total of 150subjects were enrolled in Medicine Department and referred to Pathology Department for various hematological test.Patients divided into two groups, control (n=50) and ischemic stroke subjects (n=100). Data regarding risk factors was collected through questionnaire. Their hematological parameters such as WBCs count, RBCs count, haemoglobin concentration, HCT, MCV, MCH and PLT count were assessed by hematology analyzer. Statistical analysis was done by using SPSS (version 13). Result: Mean age of control and ischemic stroke group was 61.40y±1.33 and 63.8y±1.36 respectively with non-significant difference between the groups. Among the ischemic stroke subjects, 73% of the subjects were suffering from hypertension, 53% reported diabetes, 41% had heart disease, 39% had family history of stroke, 32% were smokers and 46% use high cholesterol food. The WBCs count, MCV, MCH and HCT were found significantly higher in ischemic stroke subjects when compared with control group while the significant decrease was observed in RBCs count and hemoglobin concentration. PLT count increased in ischemic stroke subjects as compared to control group. Conclusion: It was concluded that in ischemic stroke patients hematological parameters were greatly altered.

Objectives: Although there are many studies on stroke, few studies on electrolyte disturbance have been done in our country, even on the outside. Our aim in this study is to estimate the level of serum potassium and sodium in acute stroke patients with comparison to patients of the control group.Methods: Our study is a comparative cross-sectional study conducted on patients in the neurological center who suffer from stroke and others in medicine department admitted for any disease other than cerebrovascular accidents (CVA) they consider the control group. All were in AL-Diwaniyah Teaching Hospital from April to July 2018. The level of potassium and sodium from all patients is estimated. Patients with ischemic stroke, hemorrhagic stroke, and transient ischemic attack (TIA) were classified into having Glasgow coma scores (GCS) of 3–8, 9–12, and 13–15, respectively.Result: Significant difference was seen in the distribution of patients according to GCS levels (p=0.014). Mean serum sodium was significantly lowest in hemorrhagic stroke, then ischemic stroke, followed by TIA, and the highest sodium level was seen in the control group (p<0.001). Mean serum potassium was significantly lowest in hemorrhagic stroke, then ischemic stroke, followed by TIA, and the highest sodium level was seen in the control group (p<0.001). Mean serum-to-potassium ratio was significantly highest in hemorrhagic stroke, then TIA, followed by ischemic stroke, and finally, by control group (p<0.001).Conclusion: This study reveals that, in hemorrhagic stroke, the incidence of electrolytes imbalance was more than ischemic and which was mostly hyponatremia and hypokalemia.

Purpose: In all types of ischemic stroke, especially in the acute phase, excessive oxidative stress causes structural and functional damage to the brain. This may play a major role in the pathophysiology of the brain damage. Higher serum levels of bilirubin have therapeutic effects in oxidative stress-induced stroke. Nevertheless, role of increased serum levels of bilirubin in the acute phase of ischemic stroke is controversial. Methods: This study was a cross-sectional prospective descriptive study conducted in the Emergency Department (ED) of Imam Reza hospital, Tabriz University of Medical Sciences, Tabriz, Iran, throughout six months. 275 ischemic stroke patients were evaluated based on their brain CT scan infarct size, NIHSS, MRS, and serum levels of bilirubin. Later, data were analyzed using SPSS software. Results: Total, direct and indirect bilirubin levels were significantly higher in expired patients (p< 0.0001). Total (p< 0.0001), direct (p< 0.0001) and indirect (p< 0.0001) bilirubin levels, NIHSS score (p< 0.0001), and ischemic area (p< 0.0001) significantly predicted the outcome in these patients. Conclusion: Total, direct and indirect bilirubin levels was significantly associated with mortality in the acute phase of ischemic stroke patients.

Background. Organochlorine pesticides (OCP) are persistent organic pollutants that have been implicated in causing several deleterious effects in humans. These are known neurotoxins in high doses, but the role of environmentally acquired OCPs in the body to induce seizures in children has not been investigated yet. Objectives. To assess the serum levels of OCPs in children aged 2–12 with idiopathic seizure and to find out any association between the two are our objectives. Methods. It was a cross-sectional pilot study. Twenty developmentally normal children aged 2–12, presenting with idiopathic generalized seizures, were recruited. Twenty age-matched controls without any history of seizures were also taken. Their serum levels of α, β, and γ hexachlorocyclohexane (HCH); and aldrin; dieldrin; p,p-dichlorodiphenyltrichloroethane (DDT), o,p-DDT, and p,p dichlorodiphenyldichloroethylene (DDE); and α and β endosulfan were analysed using gas chromatography (GC). Mann-Whitney U test was used to compare OCP levels between the groups. Spearman correlation was used to find the correlation between individual pesticide levels with age and seizure duration. Results. Levels of β, γ, and total HCH were significantly higher among cases as compared to the control group (P≤0.05). Conclusion. There exists a possible association between idiopathic seizures and high serum levels of OCPs, especially HCH.
