The aim of this review is to summarize the effects of carbohydrate consumption on endurance sports. Due to the promotion of physical activity by health authorities, participation in endurance sports is increasing. Regardless of the level, athletes look for ways to optimize their performance through training and nutrition. In consequence, research in the field of sports nutrition has focused on those strategies that can improve performance. Fatigue during prolonged exercise is associated with depletion of muscle glycogen and reduced blood glucose concentrations. Therefore, carbohydrate feeding during exercise has shown that it can improve capacity and performance, because it helps maintain blood glucose levels, as well as, increase carbohydrate oxidation rates and preserve glycogen, finding better results when are consumed multiple-tranportable carbohydrates for absortion (e.g., fructose and glucose), since in this way is possible to increase absorption rates, gastric emptying and fluid absorption, in addition to decrease gastrointestinal distress.

There is a current trend in endurance sports to move athletes towards a low-carbohydrate diet or use periods of low carbohydrate consumption to increase both health and performance. As a result, a market is developing for sports supplements to provide nutritional support during training and racing for athletes who follow a low-carbohydrate lifestyle. PHAT FIBRE (PF) is a powdered sports supplement that includes medium-chain triglycerides suspended in a digestion-resistant carbohydrate and is tailored to the needs of low-carb athletes. Eleven healthy participants were administered 25 g of PF after an overnight fast. After 30 minutes, median blood glucose increased by 6 mg/dl from 94 mg/dl to 100 mg/dl (p = 0.002). At the same time points, median blood beta-hydroxybutyrate (BHB) increased from 0.3 mmol/L to 0.5 mmol/L. The increase in BHB was significant (p = 0.02) after excluding one outlier who had elevated levels of fasting BHB. Insulin levels did not change significantly at any point during the study. In a single participant, a revised formulation of PF (PFv2) produced a 0.6 mmol/L increase in BHB with no effect on blood glucose. These data suggest that PF can provide a source of energy for the low-carb athlete by supporting ketone production without negatively impacting insulin or blood glucose levels.

There is no consensus regarding optimal exercise timing for reducing postprandial glucose (PPG). The purpose of the present study was to determine the most effective exercise timing. Eleven participants completed four different exercise patterns 1) no exercise; 2) preprandial exercise (jogging); 3) postprandial exercise; and 4) brief periodic exercise intervention (three sets of 1-min jogging + 30 s of rest, every 30 min, 20 times total) in a random order separated by a minimum of 5 days. Preprandial and postprandial exercise consisted of 20 sets of intermittent exercise (1 min of jogging + 30 s rest per set) repeated 3 times per day. Total daily exercise volume was identical for all three exercise patterns. Exercise intensities were 62.4 ± 12.9% V̇o2peak. Blood glucose concentrations were measured continuously throughout each trial for 24 h. After breakfast, peak blood glucose concentrations were lower with brief periodic exercise (99 ± 6 mg/dl) than those with preprandial and postprandial exercise (109 ± 10 and 115 ± 14 mg/dl, respectively, P < 0.05, effect size = 0.517). After lunch, peak glucose concentrations were lower with brief periodic exercise than those with postprandial exercise (97 ± 5 and 108 ± 8 mg/dl, P < 0.05, effect size = 0.484). After dinner, peak glucose concentrations did not significantly differ among exercise patterns. Areas under the curve over 24 h and 2 h postprandially did not differ among exercise patterns. These findings suggest that brief periodic exercise may be more effective than preprandial and postprandial exercise at attenuating PPG in young active individuals. NEW & NOTEWORTHY This was the first study to investigate the effect of different exercise timing (brief periodic vs. preprandial vs. postprandial exercise) on postprandial glucose (PPG) attenuation in active healthy men. We demonstrated that brief periodic exercise attenuated peak PPG levels more than preprandial and postprandial exercise, particularly in the morning. Additionally, PPG rebounded soon after discontinuing postprandial exercise. Thus, brief periodic exercise may be better than preprandial and postprandial exercise at attenuating PPG levels.

Ingesting quality carbohydrates has been shown to be essential for dancers. Given that most dance classes take place in the morning, it has been recommended that dancers eat a well-balanced breakfast containing carbohydrates, fats, and protein as a means of fueling this activity. The aim of this study was to determine the effect of a moderate glycemic index energy (MGI) bar or a fasting condition on dancers’ blood glucose levels and perceived pleasure-displeasure response during the first dance class of the day. In a randomized counterbalanced design, 10 female preprofessional dance students took their regular scheduled contemporary dance class, on four separate occasions. On each occasion, they consumed either a commercially prepared carbohydrate (CHO)-dense energy bar (47.3 g CHO) or water (FAST). Plasma glucose responses and pleasure-displeasure affect were measured before and at two time points during the class. Dancers who consumed the MGI bar had significantly greater peak blood glucose levels at all time points than those who fasted (p<0.05). Regarding affective state measures, participants who had breakfast had significantly greater pleasure scores than those who only ingested water (p<0.05). In conclusion, results suggest that CHO with an MGI value positively impacts blood glucose concentrations during a dance class. Further, we conclude that skipping breakfast can have an unfavorable effect on the pleasure-displeasure state of dancers. These findings highlight the impact of breakfast on how one feels, as well as the physiological and metabolic benefits of CHO as an exogenous energy source in dancers.

The aim: To compared blood glucose concentrations after intravenous injection of dexamethasone in the pregnant and non-pregnant women under general anesthesia. Materials and methods: Eighty patients aged 18-50 years took part in the study (ASA class 1 and 2). Forty of patients were undergoing elective cesarean section under G/A and the other forty undergoing elective laparoscopic Cholecystectomy under G/A. Anesthesia was induced using IV anesthetic drugs (0.5mg/kg ketamine, sleeping dose of propofol up to 2mg/kg, muscle relaxant was 0.6 mg/kg rocuronium and maintained with isoflurane). All of patients have been injected with 0,1mg/kg dexamethasone intravenously, at induction of anesthesia, Blood glucose concentrations were measured at induction and then in 60min, 180min and in 360 min after injection of dexamethasone and results were compared between the groups; IV fluid added was normal saline (0.9%) during the study. Results: Regarding to blood glucose levels, we noticed that its level significantly increased over time and peaked in 180min after dexamethasone injection in both groups. The difference percentage between the lower reading (pre injection) and the upper reading (in 180min after) was 33.5% in pregnant woman and 46.2%for non-pregnant women, this difference was statistically significant relative to the pre injection, as this difference was lower in the pregnant women. In 360min after blood glucose level began to drop in both groups. After giving 0.1 mg/kg of dexamethasone, blood glucose level increased in both groups, but it was lower in pregnant women.

The purpose of this study was to examine the effect of physiological plasma nonesterified fatty acid (NEFA) levels on insulin-stimulated forearm and whole body glucose uptake and substrate oxidation during euglycemia and hyperglycemia. Seven healthy men received Intralipid and heparin for 210 min in two studies, with saline as control in two further studies. Insulin (0.05 U.kg-1.h-1) was infused from 60 min, and euglycemia was maintained during lipid (EL) and control (EC) studies, and hyperglycemia was maintained in the other studies (HL and HC). Forearm NEFA uptake was comparable in the lipid studies (+61 +/- 10 and +52 +/- 8 nmol.100 ml forearm-1.min-1, EL and HL) and was suppressed in the controls. With Intralipid, forearm glucose uptake decreased during euglycemia but not during hyperglycemia (+3.85 +/- 0.34 vs. +3.34 +/- 0.25 mumol.100 ml forearm-1.min-1, EC vs. EL, P less than 0.02), with comparable changes in whole body glucose uptake. Glucose oxidation and forearm alanine release decreased with Intralipid at both blood glucose levels, with no significant change in the rates of nonoxidative glucose disposal. These observations support the operation of the glucose-fatty acid cycle at physiological plasma NEFA levels at both blood glucose concentrations, but this was associated with a decrease in peripheral insulin sensitivity only during euglycemia.

Numerous studies have shown that ingesting carbohydrate in the form of a drink can improve exercise performance by maintaining blood glucose levels and sparing endogenous glycogen stores. The effectiveness of carbohydrate gels or jellybeans in improving endurance performance has not been examined. On 4 separate days and 1–2 hr after a standardized meal, 16 male (8; 35.8 ± 2.5 yr) and female (8; 32.4 ± 2.4 yr) athletes cycled at 75% VO2peak for 80 min followed by a 10-km time trial. Participants consumed isocaloric (0.6 g of carbohydrate per kg per hour) amounts of randomly assigned sports beans, sports drink, gel, or water only, before, during, and after exercise. Blood glucose concentrations were similar at rest between treatments and decreased significantly during exercise with the water trial only. Blood glucose concentrations for all carbohydrate supplements were significantly, p < .05, higher than water during the 80-min exercise bout and during the time trial (5.7 ± 0.2 mmol/L for sports beans, 5.6 ± 0.2 mmol/L for sports drink, 5.7 ± 0.3 mmol/L for gel, and 4.6 ± 0.3 mmol/L for water). There were no significant differences in blood glucose between carbohydrate treatments. The 10-km time trials using all 3 carbohydrate treatments were significantly faster (17.2 ± 0.6 min for sports beans, 17.3 ± 0.6 min for sports drink, and 17.3 ± 0.6 min for gel) than water (17.8 ± 0.7 min). All carbohydrate-supplement types were equally effective in maintaining blood glucose levels during exercise and improving exercise performance compared with water only.

Objectives The objective of this study was to determine if hypoglycemia is an effect of overnight fasting and gonadectomy in kittens, as well as to determine predictors of baseline and postoperative blood glucose. Methods This was a prospective observational study. Seventy-five kittens between the age of 8 and 16 weeks undergoing routine castration or ovariohysterectomy at an animal shelter were included. Two blood glucose measurements were analyzed per kitten after an overnight fast: a baseline reading prior to preoperative examination, and a reading immediately postoperatively. Predictors of the baseline and postoperative blood glucose levels were determined using multi-level mixed-effects linear regression. Results Kittens, when fasted overnight, were not hypoglycemic (<60 mg/dl). No kittens exhibited clinical signs consistent with hypoglycemia. No kittens had a blood glucose <70 mg/dl postoperatively. Postoperative hyperglycemia (>150 mg/dl) was observed in 44% of kittens. The only predictor of fasted blood glucose levels was body condition score. The only predictor of postoperative blood glucose levels was the fasting blood glucose value. Conclusions and relevance Overnight fasting prior to elective sterilization in 8- to 16-week-old kittens did not result in hypoglycemia. Concern regarding hypoglycemia after a prolonged fast in kittens may be unwarranted for short procedures in healthy animals.

AbstractAim: the purpose of the present study was to determine the effects of amisulpride at different doses on body weight, glucidic and lipidic metabolism.Material and method: Thirty-six white Wistar rats were treated daily for 9 weeks with amisulpride 1mg/kg and 10mg/kg body weight. Another group received distilled water and served as control group. At the end of the treatment period blood samples were collected and the follow biochemical parameters were determined: serum cholesterol, triglycerides, blood glucose, GOT, GPT. Body weight gain was also assessed weekly.Results: After treatment with amisulpride in doses of 1mg/kg and 10 mg/kg for a period of nine weeks, weight gains were recorded for both groups compared with the initial weight and the control group. Blood glucose concentrations in the group treated with 1 mg amisulpride/kg body weight were significantly increased (p<0.05 vs control group), but in the group treated with 10 mg/kg body weight glucose levels were not statistically significant increased compared to controls. Other biochemical parameters (cholesterol, triglycerides, GOT, GPT) showed no statistically significant differences compared to control group.Conclusions: amisulpride administered over a period of 9 weeks, in doses of 1mg/kg and 10mg/kg showed a slight increase of body weight regardless of gender, increased blood glucose only when was administered in the low dose, and does not affect lipid metabolism, even though decreased cholesterol and triglycerides levels. This results highlight a real benefit of treatment with amisulpride, comparatively with other athypical antipsychotics.

Aim This study investigated whether kisspeptin-10 (KP-10) prevents diabetic rhesus monkeys from insulin-induced hypoglycemic shock. Materials and methods Thirty-six adult male rhesus monkeys were used, six in each group. Diabetes was induced with streptozotocin (45 mg/kg b.w.; single dose i.v.). Groups were as: saline control, insulin alone, pre-insulin (treated with KP-10, 30 min before insulin), post-insulin (treated with KP-10, 30 min after insulin), treated with premix dose of KP-10 (50 μg) and insulin, and the group treated with the kisspeptin antagonist P234 (50 μg). Following an overnight fast, each animal was subjected to respective treatment, and blood glucose concentrations were recorded every 30-min interval for 3 h. Results Intergroup comparisons demonstrated that treatment with KP-10 prior to insulin administration and kisspeptin–insulin premix treatment allowed blood glucose levels to rise to significantly higher levels (p < 0.001) by 180 min in diabetic and healthy animals compared to treatment with insulin alone. However, intragroup comparisons revealed a significant decrease in blood glucose level in diabetic animals only. Treatment with P234 antagonist followed by insulin administration abolished the preventive action of kisspeptin, whereby blood glucose decreased significantly (p < 0.001) in both diabetic and healthy animals. KP-10 post-insulin treatment, however, remained ineffective and led, instead, to significantly decreased glucose concentrations by 180 min in both diabetic and healthy animals when compared to animals treated with insulin alone. Conclusions KP-10 bears therapeutic potential to prevent hypoglycemic shock that may sometimes occur during intensive insulin therapy. Several pharmacological aspects of its interaction with insulin and other drugs, however, remain to be investigated.

Glucose cycling (GC) is increased in pancreatic islets from hyperglycemic 6-mo-old ob/ob mice. We determined whether normalization of glycemia alters islet GC and insulin release in response to glucose as well as oxidation and utilization of the glucose. Mice were treated with phlorizin in dimethyl sulfoxide (DMSO) for 10 days, which resulted in normalization of blood glucose concentrations. Controls received DMSO. The mice were fasted overnight and killed, and their islets were isolated for measurements of insulin release at 5.5 and 16.7 mM glucose and at 16.7 mM glucose plus 10 mM arginine. GC was measured by the incorporation of 3H from 3H2O into carbon 2 of glucose, glucose oxidation by the yield of 14CO2 from [U-14C]glucose, and glucose utilization by the yield of 3H2O from [5-3H]glucose. Phlorizin treatment did not alter the response of insulin to glucose and to glucose plus arginine. GC was 30% in control and phlorizin-treated animals. Glucose oxidation and utilization were also the same in both groups. In fed 10- to 12-mo-old mice exhibiting a broad range of blood glucose levels, there was no correlation between GC and either insulin release or glucose concentrations. Thus the islets of ob/ob mice exhibit an increased rate of GC regardless of glycemia. This indicates that the increased rate of GC is an important characteristic of the diabetic syndrome in these animals and not simply secondary to hyperglycemia.
