Objective: to describe the experience of a playful activity to reflect on the hand hygiene technique among nursing professionals regarding the care of patients with Coronavirus. Method: this is a study of a professional experience report about an educational activity carried out with 25 professionals who are on the front line of COVID-19. Result: the participants realized the flaws during the hand hygiene procedure and were clarified about the main doubts about the correct technique, friction time, amount of soap to be used, causing their practices to be put to the test. Conclusion: the permanent education is essential to encourage and carry out appropriate techniques, mainly those related to the barriers of exposure to the virus. The increasing in quality of hand hygiene requires investment in continuing education for health professionals and changes in the aspects related to the beliefs and culture of these professionals.

Relatar a experiência da educação permanente em saúde na prática da higienização das mãos. Trata-se de um relato de experiência sobre a educação permanente em saúde na temática de higienização das mãos com profissionais de saúde de um hospital infantil. As atividades foram desenvolvidas em conjunto com a Comissão de Controle de Infecção Hospitalar, divididas em quatro momentos.  A experiência possibilitou a aquisição de novos conhecimentos e percepção das dificuldades que os profissionais possuem para a adesão da técnica correta de higienização das mãos, além de evidenciar a importância da educação permanente com os profissionais para a mudança no processo de trabalho. A realização das atividades permitiu a vivência de experiências significativas para o grupo, envolvendo aspectos pessoais, acadêmicos e profissionais contribuindo para o conhecimento da situação e incentivo a continuação de novos projetos que estimulem a educação permanente no âmbito hospitalar.Descritores: Educação Continuada, Higiene das Mãos, Criança Hospitalizada. Permanent health education: safe care for hospitalized childrenAbstract: To report the experience of continuing health education in the practice of hand hygiene. In view of the negative impact of pain on the quality of life of cancer patients, identifying and stimulating the use of effective strategies to minimize these painful feelings is highly relevant for care. The aim of this study was to identify painful experiences of adolescents with cancer and to get to know their strategies for pain relief. This is an exploratory research, using qualitative data analysis. Semistructured interviews were held with nine adolescents with cancer, who reported on acute, recurring and chronic, physical and emotional painful experiences. To relieve these pains, they described pharmacological and non-pharmacological strategies, including: distraction, presence of relatives, bed positioning, cooperation to accomplish procedures and keeping up positive thinking. Therefore, it is essential for health professionals to know available evidence for pain relief and to develop skills to articulate this knowledge with their professional experience and with the patients' own strategies.

Objective: To evaluate the adhesion rate to the hand hygiene practice and the application of the technique among the professionals at a Health Center. Method: This is an exploratory study of a quantitative character made from a questionnaire that evaluates technique, time, solutions used, and professionals’ adhesion to the hand hygiene practice. Results: 44 health professionals were evaluated in 90 hand hygiene opportunities, with P=0.026, showing low adherence of professionals. The nurses were the professionals who most adhered to the practice without considering the correct technique (64%). The average time for hand hygiene was 20-30 seconds, and in 13 times the hands were sanitized using only water. Conclusion: Professionals did not adhere to hand hygiene in a satisfactory way, thus, health education, based on the results found and considering the difficulties of the place, can become a solution strategy.

Objetivo: Relatar as estratégias de enfrentamento à COVID-19 de um Centro de Saúde da Atenção Primária à Saúde de um município do sul da Bahia. Método: Relato de experiência Resultados: O resgate da Educação em Saúde e o foco na Educação Permanente facilitaram a adesão ao distanciamento social pela comunidade e capacitou a equipe para lidar com a situação atual. Além disso, a pandemia estimulou nos profissionais de saúde uma reinvenção das formas de atuação, bem como a ressignificação dos processos de autocuidado. Nesse contexto, a utilização das Práticas Integrativas e Complementares e da criatividade no cuidado de si e do outro permitiram o desenvolvimento da empatia, o fortalecimento do vínculo, a harmonia e o controle emocional, mesmo em meio ao pânico criado pela pandemia. Considerações finais: Em todas as ações, percebeu-se que, apesar de inserido em uma equipe multiprofissional, o profissional de Enfermagem é o protagonista da Atenção Primária à Saúde, destacando-se desde o planejamento às execuções e avaliação das ações implementadas.Descritores: COVID-19; Pandemia; Enfermagem; Atenção Primária à Saúde; Saúde Coletiva. PRIMARY HEALTH CARE IN FRONT OF COVID-19: EXPERIENCE REPORT FROM A HEALTH CENTERObjective: Report coping strategies to COVID-19 from a Primary Health Center in a municipality in the south of Bahia, Brazil. Results: The Health Education and the focus on Permanent Education facilitated adherence to social distance by the community and enabled the team to deal with the current situation. In addition, the pandemic has encouraged health professionals to reinvent their ways of acting, as well as to redefine self-care processes. Además, la pandemia ha alentado a los profesionales de la salud a reinventar sus formas de actuar, así como a redefinir los procesos de autocuidado. En ese contexto, el uso de prácticas integradoras y complementarias y la creatividad en el cuidado de uno mismo y del otro, permitió el desarrollo de la empatía, el fortalecimiento del vínculo, la armonía y el control emocional, incluso en medio del pánico creado por la pandemia. Despite some general interest in the development and evaluation of continuing education for health professionals, the voluminous research literature on the subject is unfortunately equivocal as to exactly what approaches work and in which situations. In 1977, Bertram and Brookes-Bertram reviewed 113 studies of continuing medical education (CME): they found that three out of the eight studies which they judged as acceptably designed showed persistent positive effects. More recently, of six studies which examined the effect of CME upon the quality of care, half reported positive effects and half reported no effect, although all the studies demonstrated cognitive improvements.

Objetivo: caracterizar a adesão da prática de higienização das mãos pelos profissionais de saúde.  Método: estudo transversal, com a equipe multidisciplinar da unidade de terapia intensiva de um hospital do Rio de Janeiro, entre janeiro e fevereiro de 2017. A coleta de dados foi através da observação direta com um formulário adaptado. Foram realizadas 165 observações analisadas pelo software R. O projeto foi aprovado por Comitê de Ética em Pesquisa. Resultados: em 13% das observações foi alcançado o padrão ouro que corresponde a higiene das mãos antes e após o contato com o paciente com a técnica correta. Todas as categorias profissionais realizaram mais a higienização das mãos após o contato com o paciente. Conclusão: verificou-se baixa taxa de adesão à higienização das mãos pelos profissionais de saúde. Contudo, a categoria enfermeiro é a que mais higieniza as mãos antes e após o contato com o paciente, conforme a técnica correta.ABSTRACTObjective: to characterize health professionals’ adherence to hand hygiene procedure. Results: the gold standard (hand hygiene before and after contact with the patient using the correct technique) was achieved in 13% of the observations. All categories of health personnel performed hand hygiene more after contact with the patient. Conclusion: the rate of adherence to hand hygiene by health professionals was low. Todas las categorías profesionales realizaron más la higienización de las manos después del contacto con el paciente. Conclusión: se verificó baja tasa de adhesión a la higienización de las manos por los profesionales de salud. Sin embargo, la categoría enfermera es la que más higieniza las manos antes y después del contacto con el paciente según la técnica correcta.

Background: Exploring the concept of ethical competence in the context of healthcare is essential as it pertains to better quality of care. The concept still lacks a comprehensive definition covering the aspects of ethical expertise, ethical knowledge and action of a health professional. Objective: This article aims to report an analysis of the concept of ethical competence. Method: A modified strategy suggested by Walker and Avant was used to analyse the concept. Results: As a result, the concept of ethical competence can be defined in terms of character strength, ethical awareness, moral judgement skills and willingness to do good. Virtuous professional, experience of a professional, human communication, ethical knowledge and supporting surroundings in the organisation can be seen as prerequisites for ethical competence. Ethical competence results in the best possible solutions for the patient, reduced moral distress at work and development and democratisation of society. Conclusion: The results of the analysis establish a basis for an instrument to evaluate health professionals’ ethical competence. It will guide educators, as well as managers in healthcare, to support the development of ethical conduct in healthcare.

AbstractObjective: because they are health professionals, nursing and medical students' hands during internships can function as a transmission vehicle for hospital-acquired infections.Method: a descriptive study with nursing and medical degree students on the quality of the hand hygiene technique, which was assessed via a visual test using a hydroalcoholic solution marked with fluorescence and an ultraviolet lamp.Results: 546 students were assessed, 73.8% from medicine and 26.2% from nursing. The area of the hand with a proper antiseptic distribution was the palm (92.9%); areas not properly scrubbed were the thumbs (55.1%). 24.7% was very good in both hands, 29.8% was good, 25.1% was fair, and 20.3% was poor. The worst assessed were the male, nursing and first year students. There were no significant differences in the age groups.Conclusions: hand hygiene technique is not applied efficiently. Education plays a key role in setting a good practice base in hand hygiene, theoretical knowledge, and in skill development, as well as good practice reinforcement.

ABSTRACT Objective: To identify in the literature the tools used to measure self-efficacy of health professionals in hand hygiene. Methods: Integrative literature review carried out by consulting the databases PubMed, Scopus, Web of Science, Cumulative Index to Nursing and Allied Health Literature, Europe PubMed Central, and Science Direct using the descriptors Self Efficacy, Hand Hygiene, and Health Personnel. Results: Six articles, all of which with observational design, were selected. It was possible to infer that four studies used validated instruments to measure self-efficacy of health professionals in the conformity with the recommendations for hand hygiene. The other studies used questionnaires that were not validated. Final considerations: Despite the extensive literature on hand hygiene, there is a lack of scientific evidence regarding the use of validated instruments to measure self-efficacy of health professionals in the procedure. The use of properly validated psychometric instruments is useful to guarantee the quality of results in studies.

Background: Human hands are home to thousands of microorganisms, which may be transmitted to surfaces that the hands come into contact with. When in contact with people who are ailing or have weakened immune systems, some of these microorganisms can cause infections and disease. Correct hand hygiene goes a long way in eradicating these potentially infective microorganisms and forms the cornerstone of infection prevention and control (IPC) within healthcare facilities and beyond. The healthcare industry is constantly challenged by healthcare-associated infections (HAIs) and their negative effects on patient safety and clinical outcomes. Hospitals in Pretoria are facing similar challenges posed by HAIs and there is no report available on compliance of healthcare professionals (HCPs) to the World Health Organization’s (WHO) ‘five moments of hand hygiene’. Healthcare professional’s compliance to all of the five moments of hand hygiene, particularly within the patient zone, is crucial in mitigating and reducing the spread of contact-based infections in the healthcare setting.Methods: A quantitative longitudinal design was used in a covert direct observation of HCP compliance to the WHO’s five moments of hand hygiene. The observations were conducted over 4 weeks in three hospitals, covering 25 wards, inclusive of four adult critical care units using the WHO’s ‘five moments of hand hygiene’ observation form.Results: A total of 1906 hand hygiene opportunities were directly observed in three hospitals. Hand hygiene compliance was 17.26% (n = 329). Allied health professionals had higher compliance (23.02%) than medical (19.26%) and nursing professionals (15.76%). The moment before patient contact had the lowest compliance (8.21%) as compared with all other moments.Conclusions: In general, HCPs had low compliance to the five moments of hand hygiene within the patient zone. Allied health professionals had higher compliance than medical and nursing professionals. Compliance in public hospitals was lower than in private hospitals. Critical care units had higher compliance compared to general wards. Healthcare professionals better complied to the moments meant for their safety as compared to those indicated for patient safety.
