Introduction: Measles is a major cause of mortality globally. In many countries, management of measles is based on clinical suspicion, but the predictive value of clinical diagnosis depends on knowledge and population prevalence of measles. In the pre-vaccine era with high measles incidence, Koplik's spots (KS) were said to be "pathognomonic". This study prospectively evaluated test properties and diagnostic odds ratios (OR) of KS. Methodology: Data including KS status were prospectively collected for a six-month period on all suspected measles cases reported to the North-West London Health Protection Unit. Saliva test kits were sent to all cases and KS test properties were analysed against measles confirmation by PCR or IgM testing (gold standard). Results: The positive predictive value (PPV) of clinically suspecting measles was 50%. Using KS as diagnostic tool improved the PPV to 80% and the presence of KS was associated with confirmed measles in the multi-variable analysis (OR 7.2, 95% Confidence Interval 2.1-24.9, p=0.001). Conclusion: We found that Koplik's spots were highly predictive of confirmed measles and could be a good clinical tool to enable prompt measles management and control measures, as action often needs to be taken in the absence of laboratory confirmation. We suggest that current clinical case definitions might benefit from the inclusion of KS.

A combination of historical, geographical, and epidemiological studies has given sufficient insight into the ecology of sleeping sickness to enable the main factors influencing the development and spread of an epidemic to be traced.The evidence shows that in West Africa sleeping sickness is not primarily a disease of the forest, where tsetse flies are most abundant, but belongs essentially to the dry country in the north of the savanna woodland zone, where the earliest occurrences and severest outbreaks have been located.The first mention of sleeping sickness comes from the upper Niger and dates back to the 14th century. By the beginning of the present century intense though localised epidemics were devastating parts of the Mossi, Grounsi and Lobi country of the upper Volta rivers. At this time the disease was unknown on the coast and of sporadic occurrence only in the forest. A severe trans-Volta epidemic covering 60,000 square miles, developed between 1924 and 1940, but was confined to the north of the inland savanna zone with nothing comparable in the forest.The epidemic spread in three principal ways : (1) Outwards from original foci of infection because of the dynamic nature of the disease. This produced a concentration of infection around headwaters, a feature characteristic of advanced epidemics. (2) Through the agency of travellers, originally from north to south but subsequently in both directions : a rapid method of spread producing linear distribution of infection along trade routes. The tempo was greatly increased on the pacification and development of West Africa after 1900. (3) A gradual southward shift in the main epidemic zone appears to be resulting from a long-term change in the African climate which is combining with man's activities to produce a southerly extension of xerophytic vegetation types and a regression of forest.The most important spread was that caused by the trading caravans, more especially the cola traders, who have been coming down to the cola-nut areas in the Ashanti forest from the big markets on the Niger and Upper Volta since the 11th century. The caravans were formerly very large, up to one or two thousand strong, and were frequently made up of Mossi and Grounsi from the territory that was so heavily infected by the beginning of the present century. It is certain that a continuous introduction of infection would have been taking place into the forest ever since trypanosomiasis was prevalent in the north, that is for 100 years at least. And infection has been known in the forest for about that period, yet always to a mild degree, never reaching epidemic form. It has been sought for, because conditions in the forest, with the vector Glossina palpalis in contact with every village and path, appeared to be ideal for the transmission of infection and this drew the particular attention of the early workers from 1908 onwards. But the most that could be found was a threatened epidemic in north-west Ashanti, very significantly centering on the big cola markets which formed the termini for the northern traders.This historical evidence and the reasoning from epidemiology lead to the conclusion that conditions in the forest are not conducive to the development of epidemic sleeping sickness and that the low state of endemicity found there is maintained by the constant introduction of infection from the true epidemic areas in northern savanna.From this conclusion arises a practical point of the greatest importance. If the sources from which infection is introduced into the forest could be eliminated the disease there should eventually die out and the tsetse, from the human point of view, would be harmless. Tsetse control in the forest may prove difficult and expensive, and if it is attempted by clearing this might end in the literal destruction of the forest. Such measures would be hard to justify, so many other factors of possibly greater importance than trypanosomiasis are involved, both the intrinsic value of a forest for its products and the wider value through its influence on climate, soil and water.In formulating a plan for the control of sleeping sickness, the habits of both vectors, human as well as insect, should be considered. The tsetse plays a major role in the development of the high infection rates characterising the epidemic outbreaks in northern savanna ; the human vector distributes infection from these sources along trade routes and into the forest. The elimination of the disease at its source, in true epidemic centres, which can be most effectively accomplished by eradication of the tsetse, will check the distribution of infections to the secondary areas of lighter infection which could then be cleared up by quite minor control measures or might even disappear spontaneously.This plan is now in operation in the Gold Coast. The validity of the arguments on which it was based is being shown by the results that are already apparent : the high rates of reduction in the epidemic areas and the pronounced lowering of infection in neighbouring, uncontrolled areas, more particularly in the forest region of north-west Ashanti where it is entered by a trade route coming from the previously heavily infected country.

The occurrence of schistosomiasis is directly linked to the presence of its snail intermediate host <em>Biomphalaria</em> spp. Knowledge of geographical distribution, habitats and behaviour of these snails in relation to the climate is essential for guiding measures for disease prevention and control. This study aims to model the distribution of <em>B. glabrata</em> and <em>B. straminea</em> in schistosomiasis non-endemic areas of the metropolitan region of Recife (MRR) based on environmental data and estimates of snail distributions in endemic and neighbouring areas. We applied Kriging with the aim of determining the spatial distribution of these two snail species and MaxEnt for modelling their ecological behaviour. Kriging showed that the North and the Centre of the MRR were generally either snail-free or contained only <em>B. straminea</em>, while both snail species could be found in the South. MaxEnt supported our observation that the northern and southern coastal regions were favoured by <em>B. glabrata</em> and diurnal mean temperature variation; July rainfall and November rainfall were the three variables favouring <em>Biomphalaria</em> breeding sites that contributed the most in the predictive model we developed. The study showed the location of areas suitable to <em>Biomphalaria</em> spp. and therefore at potential risk, first for invasion of these snails and later for the development of new schistosomiasis- endemic areas. This information should be useful, not only to estimate expansion possibilities of this disease in the MRR, but also to point out the climatic variables that would contribute to this expansion, thereby allowing timely application of prevention and control measures.

Aim: To evaluate the epidemic situation on measles and rubella and to carry out the laboratory diagnostics of these infections in the North-West of Russia in the period of elimination.Materials and Methods. In 2016-2019 a total of 534 blood serum samples from patients with measles diagnosis, 210 – from patients with rubella diagnosis and 1251 – from patients with exanthema were studied by ELISA using “VectoMeasles-IgM” and “EIA-Rubella-IgM” test-systems.Results. Measles incidence in NWR varied from 0 to 0.96 per 100,000 in 2016 and 2019, respectively. Among measles patients the proportion of children and adults changed in 2018-2019, adults constituted 65.9%. Patients not vaccinated against measles and those with the unknown vaccination status were mainly involved in epidemic process (85%). High level of patients with revaccination status was revealed in 2018 (18.4%). These patients demonstrated high levels of IgG (>3.0 IU/ml) as well as high percentage of IgG avidity (>94.3%) on 4-7th day of rash onset thus evidencing secondary immune response. Molecular studies of the samples from patients revealed the circulation of D8 and B3 genotypes of measles virus. Strains of D8 and B3 genotypes were represented by 3 and 4 variants, respectively. Rubella incidence did not exceed 0.13 per 100,000. Strains of rubella virus of genotype 1E were detected.Conclusion. The data of annual measles incidence and molecular studies of circulating wild type measles viruses prove the on-going measles elimination process in the NorthWest of Russia. For rubella, the persistent phase of elimination was demonstrated in the region.

Abstract Fluoroquinolones are important antimicrobial agents for the treatment of Pseudomonas infections. A total of 11 isolates of P. aeruginosa were collected from different clinical samples from different medical centers in the North West Bank-Palestine during 2017. In this study, resistance to fluoroquinolones and secretions of β-lactamases were detected by phenotypic methods, while presence of β-lactamase gene sequences and other virulence factors were detected by PCR technique. PCR product for gyrA, parC and parE genes were sequenced for further analyses. The phylogenetic analyses, population diversity indices and haplotypes determination were conducted using computer programs MEGA version 6, DnaSP 5.1001 and median-joining algorithm in the program Network 5, respectively. Results of this study showed that the MIC for ciprofloxacin and norfloxacin had a range of 32-256 µg/ml. In addition, all isolates carried either exoT or exoT and exoY genes, different β-lactamase genes and 82% of these isolates harbored class 1 integrons. Analyses of the gyrA, parC and parE sequences were found to be polymorphic, had high haplotype diversity (0.945-0.982), low nucleotide diversity (0.01225-0.02001) and number of haplotypes were 9 for each gyrA and parE genes and 10 haplotypes for parC gene. The founder haplotypes being Hap-1 (18%), Hap-2 (27.3%) and Hap-6 (9.1%) for gyrA, parC and parE genes, respectively. Two of ParE haplotypes were detected as indel haplotypes. The Median-joining- (MJ) networks constructed from haplotypes of these genes showed a star-like expansion. The neutrality tests (Tajima’s D test and Fu’s Fs test) for these genes showed negative values. Palestinian fluoroquinolone resistant P. aeruginosa strains showed high MIC level for fluoroquinolones, β-lactamase producers, carried type III secretion exotoxin-encoding genes, most of them had integrase I gene and had high level of mutations in QRDR regions in gyrA, parC and parE genes. All these factors may play an important role in the invasiveness of these strains and make them difficult to treat. Isolation of these strains from different medical centers, indicate the need for a strict application of infection control measures in Medical centers in the North West Bank-Palestine that aim to reduce expense and damage caused by P. aeruginosa infections. Molecular analyses showed that Palestinian fluoroquinolone resistant P. aeruginosa haplotypes are not genetically differentiated; however, more mutations may exist in these strains.

INTRODUCTION: Shigella spp. are Gram-negative, nonsporulating, rod-shaped bacteria that belong to the family Enterobacteriaceae and are responsible for shigellosis or bacillary dysentery, an important cause of worldwide morbidity and mortality. METHODS: We studied the antibiotic resistance profiles of 122 Shigella spp. strains (81 S. flexneri, 41 S. sonnei, 1 S. boydii) isolated from patients (female and male from 0 to 80 years of age) presenting diarrhea in different districts of the State of Pará, in the North of Brazil. The antibiotic resistance of the strains, isolated from human fecal samples, was determined by the diffusion disk method and by using the VITEK-2 system. RESULTS: The highest resistance rate found was the resistance rate to tetracycline (93.8%), followed by the resistance rate to chloramphenicol (63.9%) and to trimethoprim/sulfamethoxazole (63.1%). Resistance to at least three drugs was more common among S. flexneri than S. sonnei (39.5% vs. 10%). Six (4.9%) strains were susceptible to all the antibiotics tested. All strains were susceptible to cefotaxime, ceftazidime, ciprofloxacin, nalidixic acid and nitrofurantoin. CONCLUSIONS: High rates of multidrug resistance in Shigella spp. are a serious public health concern in Brazil. It is extremely important to continuously monitor the antimicrobial resistances of Shigella spp. for effective therapy and control measures against shigellosis.

The pursuit of quality service delivery in the public sector management in Cameroon necessitated the decentralisation of public sector management by transferring more power and resources to the local councils. This is in a bid to make them more self-governing. These councils are expected to meet the aspirations of their municipalities by carrying out approved projects efficiently and effectively. In this regard, this paper sets out to investigate the relationship between budgetary control and performance of local councils in Mezam and Momo Divisions of the North West Region of Cameroon, and the challenges these councils encounter in the process of budgetary control. Data was collected through a survey and analysed using the Ordinary Least Square (OLS) estimation technique to regress the relationship between the budgetary control variables and council performance. The empirical results showed that the key budgetary control variables (planning, participation, monitoring and control, motivation, communication, and responsibility) have a positively and statistically significant effect on performance of the councils. Effective performance of local councils in Mezam and Momo Divisions of the North West Region of Cameroon could then be attributed to effective presence of budgetary control requiring the availability of financial resources rationally allocated, qualified and experienced personnel, participation of all responsibility center managers in the planning and control processes, and regular communication and motivation of the council staff.

The programme of oral vaccination of wildlife started in 1988 in Slovenia and is based on our and the experiences of other countries. Red foxes are the main reservoir of rabies in Slovenia. When the oral vaccination programme started in whole territory of Slovenia in the year 1995, 1089 (28.75%) of tested animals were detected positive among wild and domestic animals. Four years later only 6 (0.5%) positive cases were detected among 1195 tested animals. The number of positive cases been increased again in 2001 to 135 cases. Between 2002 and 2008 the vaccination was done only in the protection zone, a 30 to 50 km wide belt along the southern border with Croatia because no new rabies cases were found in the north-west region of the country. When rabies was reintroduced in Italy in 2008 the vaccination was carried out again in the whole territory of Slovenia. In order to improve the vaccination campaign the stability of two vaccines was measured over 8 weeks. In both vaccines the drop of the virus titre was the highest when baits were placed in the sunlight, but, in the shadow, the virus was detected until day 53 of observation. The aim of this study is to summarise the current status of rabies and to look for the best solutions in the next vaccination campaign.

In September 2016, a cluster of seven kayakers with clinical symptoms of leptospirosis with onset since July 2016 was reported to French health authorities. Human and animal investigations were undertaken to describe the outbreak, identify the likely place and source of infection and implement necessary control measures. We identified 103 patients with clinical symptoms of leptospirosis between 1 June and 31 October 2016 who lived in the Ille-et-Vilaine district in Brittany. Of these, 14 (including the original seven) reported contacts with the river Vilaine during the incubation period and were defined as outbreak cases: eight were confirmed by serology tests or PCR and six were probable without a laboratory confirmation for leptospirosis. All 14 cases were kayakers. Three distinct contamination sites were identified on a 30 km stretch of the river Vilaine. Nine cases reported having skin wounds while kayaking. None were vaccinated against leptospirosis. The outbreak was attributed to Leptospira kirschneri serogroup Grippotyphosa. Animal investigations did not allow identifying the possible reservoir. Leptospirosis outbreaks associated with freshwater sports are rare in temperate climates. The prevention of such outbreaks requires control of potential animal reservoirs in zones such as the Vilaine valley and that kayakers adopt the recommended individual prevention measures.

SUMMARYSaturation diving is an important and widely used technique in the Offshore Oil Industry. During 1974–5 two saturation dives in the North Sea were terminated because of outbreaks of incapacitating otitis externa, and others were disrupted. Pseudomonas aeruginosa was consistently isolated from the ears of affected divers. Because complex work schedules were threatened seven subsequent dives were subjected to microbiological monitoring and control. Colonization of the ear canal with P. aeruginosa or with other gram-negative bacilli occurred in 39 (67%) of the 58 divers studied, usually within 7 days of starting the dive. Data obtained by serotyping the isolations of P. aeruginosa suggested that a single infected diver may be the source of organisms which rapidly spread to his colleagues and throughout the living chambers, that the living chambers may constitute a reservoir of infection during and between dives, and that certain serotypes of P. aeruginosa are more likely than others to colonize the ear canal in the conditions of a saturation dive. The control measures used during the dives were only partially effective, but none of the divers suffered severe pain and all the dives were an operational success.
