An atypical case of Guyon’s canal syndrome is reported. A 40-year-old woman with hypoaesthesia and claw-finger deformity of the little finger, hypothenar muscle atrophy but no first dorsal interosseous muscle atrophy, underwent release of a fibrous band to decompress the ulnar nerve at the deep branch just proximal to the branch to the abductor digiti minimi muscle. The claw-finger deformity recovered 1 week after surgery.

Guyon’s canal syndrome is a compression neuropathy of the ulnar nerve that occurs at the wrist. Compression can be caused by many conditions, such as benign tumors, repeated occupational trauma, vascular lesions, fractures of the carpal bones, rheumatoid arthritis and anomalous hypothenar muscle at Guyon's canal. Here, the authors report a case of compression neuropathy of the ulnar nerve by the accessory abductor digiti minimi muscle (AADM), one of the anomalous hypothenar muscles, at Guyon's canal. The case report is accompanied by a literature review.

AbstractAnomalous hypothenar muscles in the wrist are relatively common with 47% prevalence, and their possible consequences associated with Guyon's canal are well known. The accessory of the abductor digiti minimi muscle (ADM) is known to be one of the causes of ulnar tunnel syndrome or ulnar artery thrombosis. A unique ADM was found in the volar wrist region of an 87-year-old male cadaver passing through Guyon's canal.

Anomalous variations of abductor digiti minimi are commonly found at Guyon's canal but rarely cause ulnar nerve compression. We report such a case with particular emphasis on the effectiveness of ultrasound to detect and delineate anatomical structures in this region.

This cadaver study investigated the innervation patterns of the abductor digiti minimi in Guyon’s canal. There was only one branch to the abductor digiti minimi in 22 of the 30 specimens. Two branches were found in three hands, and three branches in two. Three other variations were documented.
