Effectiveness of isolated septal surgeries in nasal septal deviations: A prospective study with reference to the nasal obstruction symptom evaluation scale

<p class="abstract"><strong>Background:</strong> Inferior turbinate hypertrophy is one of the major causes of nasal airway obstruction. Medical treatment often produces insufficient improvement. In these circumstances, surgical reduction of inferior turbinate can be proposed. Many different techniques are currently available. The aim of our study is to prospectively evaluate the effectiveness of three techniques i.e., electrocautery, cryotherapy and radio frequency by comparing the response of patients to these surgical modalities of turbinate reduction.</p><p class="abstract"><strong>Methods:</strong> A total of 90 patients presenting with nasal obstruction with or without allergic symptoms, fulfilling the inclusion and exclusion criteria, attending ENT OPD are included in the study. A prospective study was conducted on three groups of thirty patients with symptoms and signs of nasal obstruction associated with inferior turbinate hypertrophy of more than 3 months duration. Then the percentage of subjective improvement in nasal airway is used to assess treatment outcomes 1 week, 1, 3, 6, 9 and 12 months after surgery.  </p><p class="abstract"><strong>Results:</strong> At the end of 12 months, 56.6% of patients treated with electrocautery and 56.6% of patients treated with cryotherapy showed 75% improvement in nasal obstruction whereas 59.99% of patients treated with radio frequency showed 100% improvement. No statistical difference in the amount of improvement in nasal obstruction was noted between the three treatment groups at the end of 12 months because of relatively small sample size.</p><strong>Conclusions:</strong> On this basis it is evident that clinically radio frequency showed better subjective improvement in nasal obstruction when compared to cryotherapy and electrocautery.

Twenty-five patients with bilateral nasal obstruction due to hypertrophy of the inferior turbinates were entered into a prospective study to compare the efficacy and the associated complications of total inferior turbinectomy and concho-antropexy. We found no significant statistical difference in the efficacy of these procedures in relieving nasal obstruction and discharge (>0.5). Total inferior turbinectomy was associated with more postoperative pain (p<0.05) and with long-term dryness and crusting (p<0.05) which were statistically significant. This is the first trial where concho-antropexy and total inferior turbinectomy are compared.

Introduction Since the last century surgical correction of nasal septum deviation has been improved. The Universal Eclectic Technique was recently reported and there are still few studies dedicated to address this surgical approach. Objective The objective of this study is to compare the results of septal deviation correction achieved using the Universal Eclectic Technique (UET) with those obtained through Cottle's Technique. Methods This is a prospective study with two consecutive case series totaling 90 patients (40 women and 50 men), aged between 18 and 55 years. We divided patients into two groups according to the surgical approach. Fifty-three patients underwent septoplasty through Universal Eclectic Technique (UET) and thirty-seven patients were submitted to classical Cottle's septoplasty technique. All patients have answered the Nasal Obstruction Symptom Evaluation Scale (NOSE) questionnaire to assess pre and postoperative nasal obstruction. Results Statistical analysis showed a significantly shorter operating time for the UET group. Nasal edema assessment performed seven days after the surgery showed a prevalence of mild edema in UET group and moderate edema in Cottle's technique group. In regard to complication rates, UET presented a single case of septal hematoma while in Cottle's technique group we observed: 02 cases of severe edemas, 01 case of incapacitating headache, and 01 complaint of nasal pain. Conclusion The Universal Eclectic Technique (UET) has proven to be a safe and effective surgical technique with faster symptomatic improvement, low complication rates, and reduced surgical time when compared with classical Cottle's technique.

<p class="abstract"><strong>Background:</strong> The deviated nasal septum (DNS) is a commonly occurring clinical condition that often causes nasal obstruction. This study, included two groups of subjects 100 each. The group A consists of subjects having DNS with nasal symptoms. The group B consists of subjects having DNS without nasal symptoms but with ear and throat symptoms. The present study makes an effort to review the types and its associated pathology.</p><p class="abstract"><strong>Methods:</strong> A prospective study was carried out in the Department of ENT, Government Medical College and Hospital, Nagpur from September 2016 to October 2018. Data was collected and analysed.  </p><p class="abstract"><strong>Results:</strong> The mean age 31.17 years in group A and 30.07 years in group B, with male to female ratio of 1.17:1 in group A and 1.04:1 in group B. Left-sided septal deviation was more common than right-sided deviation. The most common symptoms in group A was nasal obstruction (94%) and in group B was otorrhoea (80%)."C" shaped DNS was the most common type in both the groups. Out of total 200 subjects comprising both the groups, 70 (35%) subjects had significant sinonasal disease, 138 (69%) subjects had ear pathology, 69 (34.5%) had Eustachian tube dysfunction, 35 (17.5%) had throat pathology. In this study, “S” shaped deviation was more prone to be associated with ENT pathology.</p><p class="abstract"><strong>Conclusions:</strong> “S” shaped DNS was maximally associated with sinonasal pathology and there was a high correlation between the side of septal deviation to the side of ear pathology, particularly in asymptomatic DNS.</p><p> </p>
