Abstract A prospective researchwas performed on 50 patients submitted for radiological screening in Samarra general hospital complaining of chronic backache and sciatica. The study was completed for patients recommended by senior neurosurgeons, orthopedicians, &rheumatologist, to the radiological unit in Samarra public hospital from October 2016 to October 2017. Radiological studies were analyzed by senior radiologists of Samarra public hospital in conjunction with the recommending senior doctors. Patients who are operated on were all proven to have chronic Lateral Recess Syndrome both radiological and surgically.

ABSTRACTIntroductionSciatic pain secondary to nerve root compression occurs in approximately 1% of the general population in the United States, which represents enormous costs related to the treatment and loss of function of symptomatic individuals.Acute radicular pain is predominantly caused by herniation of the intervertebral disc but can also be caused by degenerative changes. Compressive and inflammatory mechanical factors are interrelated in the pathophysiology of symptoms.Although radiculopathy is self-limited in most cases, in its acute stage it is associated with painful symptoms and loss of essential function. The first-line treatment usually employed is a conservative approach, including a short rest period and use of common analgesics and nonsteroidal anti-inflammatory drugs.As an alternative to the conservative approach, the systemic administration of oral corticosteroids, such as prednisone, are widely used. However, the current literature shows contradictory results for this treatment in terms of improvement of pain and function.The present study hypothesizes that treatment with prednisone is effective in the treatment of acute sciatic pain in patients with social, clinical, and demographic characteristics favorable to this treatment.Objectivesthe main objective of the present study is to identify predictors, both clinical and imaging, of a positive response, in terms of both function and pain intensity, to prednisone treatment in patients with acute sciatic pain.Methods and analysisthe present study will include a cohort of patients, with a diagnosis of acute sciatica, who will receive treatment with oral prednisone. The pain and functional scores before and after treatment will be compared. Thereafter, the social, clinical, and radiographic characteristics of responsive patients will be compared to those of patients who did not respond well to the treatment.Ethics and disseminationthe study received the approval of Federal University of São Paulo Research Committee (4.232.193) and Research Ethics National Committee (CONEP).ARTICLE SUMMARYStrengths and limitations of this studyThe study is based on samples from a large population recruited from a reference facility for the treatment of acute sciatic pain.There is potential for optimization of treatment response with oral corticosteroids.The study will be conducted with a population with defined clinical characteristics.Due to the relatively short follow-up period, the project will not evaluate alternative treatments, such as surgery.

Sciatic pain treatment by Scottish souls. The speaker, on the basis of his observations in the treatment of 32 cases of chronic sciatic pain by Scottish souls, came to the conclusion that the latter represent an excellent means that enhances the exchange in the diseased nerve, being the best anti-neuralgic means in general with neuralgia. Out of 32 cases, he had 23 complete cures. 7 pain relief and only 2 without any result.

: Sciatica is a common back pain caused by irritation of the sciatic nerve. Current pharmacologic therapies have proven inadequate for many patients with sciatic pain. On the other hand, there is a global increasing demand toward the use and administration of natural medicaments for this disorder. Traditional Persian Medicine (TPM) is a school of medicine and a medicinal plant based resource for clinical studies put forward by Persian scholars. The aim of the present study is to gather and study the effectiveness of all medicinal plants from five main Persian pharmacopeias. Furthermore, different data bases such as PubMed and Scopus have been checked to derive relevant activities for these plants. In all, 99 medicinal plants related to 42 families have been authenticated. Asteraceae and Apiaceae were the most frequent families and Roots and Seeds were the most reported botanical parts. The employed routs of administration were oral (54%), topical (33%) and rectal (13%). Anti-inflammatory, analgesic activity and anti-nociceptive properties of medicines are known as some main mechanisms to manage sciatic pain. These functions are possessed by 30%, 15%, and 15% of the studied plants, respectively. Medicaments that can be introduced as lead agents for further investigation are Zingiber officinale Roscoe, Olea europaea L., Strychnos nux-vomica L and Artemisia vulgaris L which showed all of these properties in previous studies. Hence, conducting adducible clinical trials using these lead agents may lead to novel drugs with lesser undesirable and much more therapeutic effects on controlling sciatic pain.
