In this paper, we present some results obtained from the simulation of low power 633, 780, 850, and 940 nm laser in the liver by Monte Carlo method, with the model of the liver, consisting of 5mm derm, 7mm subcutaneous fat, 5 mm muscle layer. Based on these results, we fabricated devices called “Laser Semiconductor Optoacupuncture and phototherapy Device” using 780 and 940nm semiconductor lasers to treat chronic hepatitis. We combined with the doctor in An Giang province to clinical practice for 50 voluntary patients with chronic hepatitis. We used a 650 nm wavelength intravascular semiconductor laser treatment clinically to provide high-quality blood to the patients’ liver. Treating the phototherapy of the skin with two semiconductor laser beams with 780 nm and 940 nm wavelengths directly affects the liver from the surface of the abdomen. At the moment, we use the treatment on acupoint with 940nm- wavelength laser. A treatment course consists of 20 times for the patients is treated continuously. The patients tested with the ALT and AST before and after treatment with 3 courses. We use the SPSS 23 statistical method to evaluate the outcomes of treatment. The clinical symptoms of the patients such as fatigue, nausea, indigestion, fever, jaundice, yellow eyes almost completely have gone out after treatment. Low-level laser therapy offers a good response in patients with moderate to severe hepatic impairment such as the AST of 56.380 ± 10.162 and 39.260 ± 4.869; The ALT of 56.540 ± 13.580 and 41,360 ± 7,488 for before- and after treatments, respectively. Low-level laser therapy for patients initially has good results, high therapeutic effectiveness, no catastrophic or side Effects, and the statistical significance is p < 0.001.This research applied the ethical principles of the Helsinki Declaration in human researches. The research was carried out using non-invasive methods on humans with the regulations of the University of Technology, Vietnam National University Ho Chi Minh City, and the relevant regulations.

Старение организма человека - закономерный физиологический процесс. В прошлом веке основными причинами обращения людей к пластическим хирургам служили врожденные и приобретенные деформации, а также возрастные изменения кожи лица. В наши дни внешний вид человека стал основным фактором его успешности в обществе. В работе определена мотивация людей пожилого и старческого возраста к выполнению пластических операций, а также проанализирована работа клиник по выполнению пластических операций людям старших возрастных групп. Установлено, что 96,4 % мужчин и 86,9 % женщин старших возрастных групп имеют достаточно полные представления о пластической хирургии. 27,7 % мужчин и 17,2 % женщин пожилого и старческого возраста хотели бы сделать себе пластическую операцию, однако женщины в 10,9 раза чаще обращаются к пластическим хирургам. Анализ работы клиник пластической хирургии показал устойчивую тенденцию увеличения обращаемости людей пожилого и старческого возраста, независимо от пола, к пластическим хирургам для выполнения операций, направленных, главным образом, на улучшение эстетики лица, шеи и коррекцию фигуры путем устранения гравитационного птоза и удаления избыточных подкожных жировых отложений на туловище. Основной причиной отказа от выполнения желаемой пациентом и рекомендуемой пластическим хирургом операции является недостаток денежных средств. Aging of the human body is a natural physiological process. In the last century, the main reasons for people turning to plastic surgeons were congenital and acquired deformities, as well as age-related changes in the skin of the face. Nowadays, a person’s appearance has become the main factor in his success in society. The work determines the motivation of elderly and senile people to perform plastic surgeries, and also analyzes the work of clinics in performing plastic surgeries for people of older age groups. It was found that 96,4 % of men and 86,9 % of women in older age groups have a fairly complete understanding of plastic surgery. Among the elderly and old people, 27,7 % of men and 17,2 % of women have a desire to perform plastic surgery for themselves, however, women are 10,9 times more likely to go to plastic surgeons. Analysis of the plastic surgery clinics showed a steady trend of increasing the number of elderly and senile people, regardless of gender, to plastic surgeons to perform operations aimed mainly at improving the aesthetics of the face, neck and body shaping by eliminating gravitational ptosis and removing excess subcutaneous fat. deposits on the trunk. The main reason for refusal to perform the operation desired by the patient and recommended by the plastic surgeon is the lack of funds.

Ehlers-Danlos syndrome (EDS) comprises a series of rare hereditary connective tissue diseases characterized by joint hypermobility, joint dislocation, and hyperextensibility of the skin, as well as cardiovascular involvement. EDS is often associated with chronic widespread physical pain, which can lead to psychological pain. Poor awareness and limited diagnosis of EDS and related symptoms result in decreased self-esteem and confusion regarding physical sensation. Furthermore, EDS imposes substantial psychological burden on patients due to exercise restriction, scars, keloids, and subcutaneous fat accumulation on the extremities, which leads to parental overprotection and bullying experiences from other children at school age. Recent large-scale studies have suggested that patients with EDS have a higher risk of mood disorders than the general population. Other cohort studies indicated high prevalence of anorexia nervosa, addiction, obsessive compulsive disorder, and anxiety disorder were found in patients with EDS. Case reports instead indicated that some psychiatric disorders were secondary symptoms due to physical problems from EDS. Therefore, psychiatrists must be more knowledgeable and proactive about EDS in their practice. We review the previous case reports and literature for patients with EDS, along with our own case of complicated psychiatric problems, which are strongly related to early stressful situations through childhood and adolescence. This is to aid general psychiatrists in the discussion of appropriate medical management in such infrequent, yet challenging conditions.

Abstract Objective: To explore the association between visceral obesity and short-term postoperative complications in patients with advanced ovarian cancer undergoing cytoreductive surgery.Methods: Medical records were reviewed for patients with ovarian cancer. Visceral fat area, subcutaneous fat area and total fat area were measured on a single slice at the level of L3/4 of a preoperative CT scan. Univariable and multivariable analyses were performed to investigate the correlation between visceral obesity and short-term complications and to analyze the risk factors for complications after surgery.Results: Of the 130 patients, 53.8% (70/130) were presented visceral obesity. Patients with visceral obesity were older than those with nonvisceral obesity (58.3 years old vs. 52.3 years old, p = 0.001). The proportion of patients with hypertension was slightly higher (37.1% vs. 11.7%, p = 0.001). The total fat area and subcutaneous fat area were higher in patients with visceral obesity (296.9 ± 72.1 vs. 173.1 ± 67.3, p < 0.001; 168.8 ± 55.5 vs. 121.6 ± 54.3, p < 0.001). Compared with patients in the nonvisceral obese group, patients in the visceral obese group were more likely to have postoperative fever (21/70 30.0% vs. 8/60 1.25%, p = 0.023), leading to a longer length of hospital stay (21 days vs. 17 days, p = 0.009). Time from surgery to adjuvant chemotherapy for patients with visceral obesity has been delayed (24 days vs. 20 days, p = 0.037). Multivariate analysis showed that visceral obesity (OR 4.770, p < 0.001) and operation time (OR 1.008, p < 0.001) were independent predictors of postoperative complications. Conclusion: Visceral obesity is an important risk factor for short-term postoperative complications in patients with advanced ovarian cancer undergoing cytoreductive surgery.

Abstract Background Lymphedema is a progressive, noncurable condition consisting of increases in subcutaneous fat and interstitial fluid in the limbs and fibrosis during later stages. The disease most commonly affects the limbs following injury to or removal of the lymph nodes. The aim of this study was to investigate the therapeutic outcomes of liposuction for cancer-related lower extremity lymphedema. Methods Sixty-two patients with cancer-related lymphedema in the unilateral lower extremity were recruited for this study, and all patients underwent liposuction. The volume of hemorrhage and lipids, the operation time, and the volume changes of the affected extremity were compared by applying the t tests, and the subjective feelings of patients were compared with the chi-square tests. Results The total lipid volume was 2539 ± 1253.5 ml, and the hemorrhage volume was 828 ± 311.8 ml. For the comparison of objective indices, (1) the percent volume differences (PVDs) before surgery, intraoperatively, and at the 3-month follow-up were 5.5 ± 12.2 vs. 11.6 ± 18.4 vs. 43.2 ± 23.7, P < 0.05, respectively; (2) greater lipid volumes and higher liposuction rates were observed for female patients, as was a smaller volume of hemorrhage; (3) greater hemorrhage volumes were observed in patients with a history of recurrent erysipelas; and (4) greater lipid volumes and liposuction rates (LRs) and smaller hemorrhage volumes were observed for stage II than for stage III patients. Conclusions Liposuction is an effective therapy for cancer-related lower extremity lymphedema. Sex, stage, and recurrent erysipelas history influence the course and effect of liposuction.

Obesity is associated with extensive expansion and remodeling of the adipose tissue architecture, including its microenvironment and extracellular matrix (ECM). Although obesity has been reported to induce adipose tissue fibrosis, the composition of the ECM under healthy physiological conditions has remained underexplored and debated. Here, we used a combination of three established techniques (picrosirius red staining, a colorimetric hydroxyproline assay, and sensitive gene expression measurements) to evaluate the status of the ECM in metabolically healthy lean (MHL) and metabolically unhealthy obese (MUO) subjects. We investigated ECM deposition in the two major human adipose tissues, namely the omental and subcutaneous depots. Biopsies were obtained from the same anatomic region of respective individuals. We found robust ECM deposition in MHL subjects, which correlated with high expression of collagens and enzymes involved in ECM remodeling. In contrast, MUO individuals showed lower expression of ECM components but elevated levels of ECM cross-linking and adhesion proteins, e.g., lysyl oxidase and thrombospondin. Our data suggests that subcutaneous fat is more prone to express proteins involved in ECM remodeling than omental adipose tissues. We conclude that a more dynamic ability to deposit and remodel ECM may be a key signature of healthy adipose tissue, and that subcutaneous fat may adapt more readily to changing metabolic conditions than omental fat.

Abstract Objectives Contemporary obstetrics has begun to appreciate the importance of diet in pregnancy, but guidelines are not based on robust data. The hypothesis that a whole grains diet improves pregnancy outcomes is tested in this study. We compared maternal and neonatal outcomes for a pregnancy diet containing 75% of total carbohydrates as refined grains with outcomes for a diet with 75% of total carbohydrates as whole grains. Methods This was a randomized interventional study in a clinic population over the last 4–7 months of normal pregnancy with extensive compliance measures. Besides obstetrical and neonatal outcomes, anthropometric measurements were done. In addition to food frequency questionnaires (FFQs), total plasma alkyl resorcinols, a unique quantitative measure of whole grains, were used as a measure of whole grain consumption. Results The data show effective compliance and no difference in outcomes between the diets with regard to maternal weight gain, birth weights, subcutaneous fat and glucose tolerance. Conclusions Ensuring compliance to a proper pregnancy diet resulted in satisfactory weight gain and normal outcomes even when the proportion of whole grains consumed is only 25% of total carbohydrates. www.ClinicalTrials.gov NCT03232762, Effects of Diet on Pregnancy Outcome and Child Obesity.

Background: Thigh fat is associated with high cardiometabolic risks, attenuate risk for dyslipidemia, and glucose intolerance. Aerobic exercise has been linked to fat metabolization due to the increase of free fatty acid oxidation and the preservation of muscle glycogen. Plaster therapy, a beauty treatment that allows the quick elimination or reduction of cellulite, flaccid skin, and localized fat, eliminating liquid from the body, producing improvements that are not only aesthetic but also health-wise, may be used to maximize fat loss in the thigh area and complement exercise. The aim of this study was to analyze the effect of plaster therapy in combination with aerobic exercise on thigh fat. Methods: Six female volunteers were randomly divided into an intervention group (TG; n = 3) performing an aerobic exercise with plaster therapy and a control group (CG; n = 3) performing only aerobic exercise. Subcutaneous fat was estimated by the analysis of skinfolds and thigh perimeters. Results: The treatment group demonstrated a significant decrease (p ≤ 0.05) in subcutaneous fat at the left and right perimeters and thigh skinfold measurements at the end of the 10-session protocol. Conclusion: Comparing skinfold measurements, both groups revealed a statistically significant decrease in the perimeter of both left and right thighs. Furthermore, skinfold measurements showed, for the treatment groups, significant statistical results in the thigh area. Exercise is indeed one of the most critical components for the metabolism of fat. Plaster therapy in combination with aerobic exercise seems to be effective for thigh fat reduction.
